1 NOVA Lite Monkey Oesophagus IFA Kit/Slides For In Vitro Diagnostic Use Product Code: 74145, 7415, 74155, 54145, , 5415, , CLIA Complexity: High Intended Use Monkey oesophagus sections are intended for use as a substrate for the screening of antibodies to intraepidermal antigens or basement membrane zones in human serum, as an aid in the diagnosis of pemphigus and bullous pemphigoid respectively. The sections can also be used for detection of endomysial antibodies as an aid in the diagnosis of coeliac disease. Summary and Explanation of the test Intra-epidermal IgG antibodies which characterise various clinical forms of pemphigus react with antigens present on the cell surface of epidermal keratinocytes. A positive result gives a characteristic chicken wire pattern. Tissue deposition of immunoglobulin occurs in almost all cases. It is recommended that both patient serum and tissue are always investigated with bullous disease. Basement membrane zone antibodies are classically found in bullous pemphigoid. In this disease direct biopsy reveals linear deposition of IgG and C3 along the dermo-epidermal junction. These autoantibodies are regularly, but not always, present in the serum of affected patients. However, antibody titre does not correlate with disease state and in clinical remission antibody may still be present in both tissues and serum. An antihuman IgG (H&L) FITC conjugate is provided in this kit to demonstrate both these autoantibodies. Endomysial antibodies are directed against reticulin-like fibres in connective tissue around smooth muscle fibers in the intestinal tracts of primates. An almost 1% sensitivity and specificity has been reported for IgA class endomysial antibodies active in coeliac disease 1,2. These antibodies are detected by the anti-human IgA FITC conjugate provided. Principles of the Procedure The kit uses an indirect immunofluorescence technique 3 where patient samples and appropriate controls are incubated with the sections. The unreacted antibodies are washed off and then appropriate fluorescein labeled conjugates are applied. Unbound conjugate is washed off as before. Slides are viewed with a fluorescence microscope and positive samples produce apple-green fluorescence which corresponds to areas of the section where autoantibody has bound. Reagents 1. Phosphate buffered saline (PBS II), provided as a 4-fold concentrate in liquid form. 4. Affinity purified sheep anti-human IgG (H&L), (monkey adsorbed) fluorescein conjugate and antihuman IgA fluorescein conjugate, containing.9% sodium azide. Prediluted, ready for use. 5. 1% Evans Blue, as an optional counterstain 6. Mounting medium, containing an anti-fading agent 7. Coverslips Kit only: 1. 2 Warnings/Precautions All donors of human serum supplied (kits only) have been serum tested and found to be negative for Hepatitis B surface antigen and antibodies to Hepatitis C virus and Human Immunodeficiency Virus (HIV 1 & 2). However, these tests cannot guarantee the absence of infective agents. Proper handling and disposal methods should be established as for all potentially infective material and only personnel adequately trained in such methods should be permitted to perform the procedures. The Evans Blue and the kit controls contain.9% sodium azide as a preservative and must be handled with caution do not ingest or allow contact with the skin or mucous membranes. If contact does occur wash with a large volume of water and seek medical advice. Explosive metal azides may by formed with the lead and copper plumbing; on disposal of reagent, flush with a large volume of water to prevent azide build up. This product should only be used by suitably trained persons for the purposes stated. Adherence to the given procedure is recommended. Storage Conditions Unopened kits should be stored at 2-8 C and can be used until the given expiry date. DO NOT FREEZE. Once slides are removed from a foil bag, they should be used immediately. Diluted PBS II buffer can be stored for up to one month at 2-8 C. All other reagents should be stored at 2-8 C. Specimen Collection Blood samples should be collected by venepuncture, allowed to clot naturally and the serum separated as soon as possible to prevent haemolysis. The serum may be stored at 2-8 C for up to 7 days prior to assay, or for prolonged storage, aliquoted and stored at -2 C or below 8. DO NOT freeze and thaw sera more than once. Avoid using lipaemic, haemolysed or microbially contaminated sera as decreased titres or unclear staining patterns may occur. Test Procedure Quality control Positive and negative controls should be used every time samples are tested. 1. Dilute PBS II concentrate. Dilute PBS II concentrate with distilled or deionized water (1 part PBS II concentrate + 39 parts distilled or deionized water) and mix. NB: only make up the entire amount of kit PBS II if the entire kit is to be used within one month. The PBS II is used for diluting patient samples and as a wash buffer. 2. Dilute patient samples. Dilute patient samples 1/1 by adding 2 L of serum to 18 L of PBS II buffer. 3. Substrate slides. Allow substrate slide(s) to reach room temperature (18-28 C) prior to removal from pouch(es). Label slides appropriately, place in the humid chamber and add one drop of positive and negative controls to the appropriate wells. Add 5 L of diluted patient sample to the remaining wells. 4. Slide incubation. Incubate slides for 3 minutes in a humid chamber at room temperature (18-28 C). (Moistened paper towels at the bottom of the chamber will maintain humidity.) 5. PBS II wash. Remove slides from humid chamber and rinse briefly with PBS II squeeze bottle. Do not squirt directly on to the wells. Place slides in a rack and immerse in PBS II and agitate or stir for 5-1 minutes. 6. Addition of fluorescein conjugate. Shake off excess PBS II. Return slides to humid chamber and immediately cover each well with a drop of the appropriate fluorescein conjugate. The antihuman IgG (H&L) AFF FITC is for use in detecting skin autoantibodies and the anti-human IgA ( chain) AFF FITC for use in detecting endomysial autoantibodies. DO NOT LEAVE WELLS UNCOVERED FOR LONGER THAN 15 SECONDS. 7. Slide incubation. Incubate slides for 3 minutes in a humid chamber at room temperature (18-28 C) in the dark. 8. PBS II wash. Wash again as described in step Optional counterstain. Add 2-3 drops of 1% Evans Blue per 1mL of PBS II prior to slide immersion. 1. Mounting with coverslip. Remove one slide at a time from PBS II wash. Quickly dry around the wells and add a drop of mounting medium to each well. Carefully lower the slide onto the coverslip, avoiding air bubbles, but if present do not attempt to remove. Wipe excess medium from around edge of coverslip. 11. View slides under fluorescence microscope. Slides may be stored for up to 3 days at 2-8 C, in the dark, without significant loss of fluorescence. Results Quality Control The positive controls should give a bright apple-green staining pattern in the intercellular cement of the epithelium ( chicken-wire pattern ) for pemphigus and reticulin-like fibres in connective tissue around smooth muscle for the endomysial control. The negative control should show dull green staining in all the tissue, with no discernible fluorescence. If the controls do not appear as described, the test is invalid and should be repeated. Interpretation of Results See reference 5 for colour photographic examples of these patterns. Results are reported as positive or negative. 3

NB: Each laboratory should establish at which point a positive result is considered clinically significant. Limitations of the Procedure 1. The light source, filters and optics of different makes of fluorescence microscopes will influence the sensitivity of the assay. The performance of the microscope is significantly influenced by correct maintenance especially centering of the mercury vapour lamp and changing of the lamp after the recommended period of time. 2. Monkey oesophagus tissue is inherently prone to auto-fluorescence, independent of the manufacturing process used to produce the tissue slides. This tends to be most pronounced in the lamina propria and submucosal regions. Care should therefore be taken when interpreting samples giving weak positive staining patterns in the muscularis mucosa, i.e. endomysial and smooth muscle staining. 3. Antinuclear (ANA), anti-mitochondrial (AMA), anti-smooth muscle (ASMA) and skeletal muscle antibodies may react with the oesophagus substrate. The presence of these autoantibodies should be confirmed on the appropriate substrate. 4. Cross reactivity due to anti-a and anti-b blood group antibodies may occur, as oesophagus mucosa can contain certain blood group substances 6. In some patients the staining pattern can mimic pemphigus antibodies, although blood group antibodies will also stain capillaries in the oesophageal musculature. 5. Due to the short distance between wells on the 1-well slides it is possible for cross contamination of samples and controls to occur. Care must be taken, especially at the washing stages, to ensure that this does not happen. 6. This test alone should not be considered diagnostic. All other factors including the clinical history of the patients and other serological or biopsy results must also be taken into account. 7. Suitability for use with other manufacturers' IFA reagents has not been assessed but use with such reagents should not necessarily be excluded. Slides sold separately are classified as Analyte Specific Reagents. Except as a component of the kit, analytical and performance characteristics are not established. Expected Values Monkey oesophagus slides were used to test pemphigus and pemphigoid skin disease patients as well as 2 random blood donors. 5 Specific Performance Characteristics A comparison study was performed on 43 serum samples (35 clinical, 8 normal) using this kit and two commercially available reference methods, one for pemphigus and pemphigoid positive samples, and the other endomysial positive samples. Overall, there was good agreement between all of the kits. For pemphigus, 1 out of 13 samples tested positive on both kits. Two of the discrepant samples were borderline positive on the kit but negative on the reference kit. For pemphigoid, all four positive samples tested agreed, although the staining was slightly weaker on the competitor kit. For the endomysial positive samples tested, all eighteen were positive by both methods. The eight normal samples tested were negative on all three kits. Summary of the Procedure 1. Dilute PBS II with distilled or deionized water. 2. Dilute patient sera 1/1 with PBS II. 3. Equilibrate substrate slides to room temperature (18-28 C). 4. Apply 5 L positive and negative controls and diluted patient sera to appropriate wells. 5. Incubate in a humid chamber for 3 minutes. 6. Wash for 5-1 minutes in PBS II. 7. Shake off excess PBS and immediately cover each well with a drop of conjugate. 8. Incubate as in Step Wash as Step Mount. 11. View slide under fluorescence microscope. Die Gewebeschnitte eignen sich auch für den Nachweis von Endomysium-Antikörpern bei der Diagnose von Zöliakie. Zusammenfassung und Erklärung des Tests Intraepidermale IgG-Antikörper sind für verschiedene klinische Formen von Pemphigus charakteristisch und reagieren mit Antigenen, die auf der Zelloberfläche von epidermalen Keratinozyten vorhanden sind. Ein positives Ergebnis ergibt ein typisches retikulär-wabenartiges Muster. In fast allen Fällen kommt es zu einer Immunglobulinablagerung im Gewebe. Es empfiehlt sich, beim bullösen Pemphigoid stets Patientenserum wie auch -gewebe zu untersuchen. Antikörper gegen die Basalmembranzone finden sich typischerweise beim bullösen Pemphigoid. In der direkten Biopsie werden bei dieser Erkrankung lineare Ablagerungen von IgG und C3 entlang der dermo-epidermalen Junktionszone nachgewiesen. Diese Autoantikörper sind in der Regel, jedoch nicht immer, im Serum der betroffenen Patienten vorhanden. Der Antikörpertiter korreliert nicht mit dem Krankheitsbild, d.h. bei einer Remission kann der Antikörper nach wie vor im Serum und Gewebe vorhanden sein. Dieses Kit enthält ein Anti-Human-IgG (H+L)-FITC- Konjugat für den Nachweis beider Autoantikörper. Endomysium-Antikörper sind gegen Retikulinähnliche" Fasern des Bindegewebes rund um Fasern der glatten Muskulatur im Intestinaltrakt von Primaten gerichtet. Für aktive Endomysium-Antikörper der Klasse IgA wird eine Sensitivität und Spezifität von nahezu 1% bei Zöliakie berichtet 1,2. Diese Antikörper werden anhand des im Kit enthaltenen Anti-Human-IgA-FITCH-Konjugats nachgewiesen. Testprinzip Der Test stützt sich auf ein indirektes Immunfluoreszenzverfahren 3, bei dem Patientenproben und die entsprechenden Kontrollen auf den Gewebeschnitten inkubiert werden. Ungebundene Antikörper werden durch Waschen entfernt, anschließend wird das entsprechende Fluorescein-markierte Konjugat aufgetragen. Ungebundenes Konjugat wird wie zuvor durch Waschen entfernt. Die Objektträger werden unter einem Fluoreszenzmikroskop ausgewertet. Positive Proben ergeben eine apfelgrüne Fluoreszenz, die den Bereichen des Gewebeschnittes mit gebundenem Antikörper entspricht. Reagenzien 1. Primaten-Ösophagus-Gewebeschnitte auf Objektträgern (5 oder 1 Vertiefungen) mit Trockenmittel. Nur Kits und 7415: 1. Zwei Positivkontrollen (aus Humanserum) mit,9% Natriumazid. Eine ergibt das retikulärwabenartige Pemphigus-Muster und die andere das Retikulin-ähnliche Endomysium-Muster auf Primaten-Ösophagus-Gewebeschnitten. Vorverdünnt, gebrauchsfertig. 2. Negativkontrolle, negativ für alle Autoantikörper, mit,9% Natriumazid. Vorverdünnt, gebrauchsfertig. 3. Phosphatgepufferte Salzlösung (PBS II), als 4-fach-Konzentrat in flüssiger Form. 4. Affinitätsgereinigtes (Schaf) Anti-Human-IgG (H+L)-Fluorescein-Konjugat (Primaten-adsorbiert) und Anti-Human-IgA-Fluorescein-Konjugat mit,9% Natriumazid. 7 Sicherheitshinweise / Vorsichtsmaßnahmen Alle Spender des in diesen Kits enthaltenen Humanserums wurden getestet und als negativ für Hepatitis- B-Oberflächenantigen und Antikörper gegen HCV und HIV (1 & 2) befunden. Infektionserreger können mit diesen Tests jedoch nicht vollständig ausgeschlossen werden. Daher sind angemessene Handhabungs- und Entsorgungsverfahren wie für jedes andere potenziell infektiöse Material zu befolgen. Diese Verfahren dürfen nur von entsprechend qualifiziertem Personal durchgeführt werden. Das Evans-Blau und die Kit-Kontrollen enthalten,9% Natriumazid als Konservierungsstoff und sind mit entsprechender Vorsicht handzuhaben - nicht schlucken und Kontakt mit der Haut bzw. Schleimhäuten vermeiden. Im Falle eines Kontaktes mit reichlich Wasser abwaschen und Arzt konsultieren. Natriumazid kann in Blei- oder Kupferrohrleitungen explosive Metallazide bilden. Spülen Sie nach einer Entsorgung von Reagenzien mit reichlich Wasser nach, um mögliche Azidablagerungen zu verhindern. Dieses Produkt darf ausschließlich von entsprechend geschulten Personen für seinen vorgesehenen Zweck verwendet werden. Es empfiehlt sich die Einhaltung des angegebenen Verfahrens. Lagerung Ungeöffnete Kits sind bei 2-8 C zu lagern und bis zum angegebenen Verfalldatum haltbar. NICHT EINFRIEREN. Nach der Entnahme der Objektträger aus einem Folienbeutel müssen sie sofort verwendet werden. Verdünnter PBS II-Puffer kann bei 2-8 C bis zu einem Monat gelagert werden. Alle anderen Reagenzien sind bei 2-8 C zu lagern. Probenentnahme Blutproben sind mittels Venenpunktion zu entnehmen. Gerinnen lassen und Serum so rasch wie möglich abtrennen, um Hämolyse zu vermeiden. Das Serum kann bei 2-8 C bis zu 7 Tage vor dem Test gelagert werden; bei längerer Lagerung aliquotieren und bei mindestens -2 C lagern 8. Serum NICHT öfter als 1x einfrieren und auftauen. Vermeiden Sie lipämische, hämolytische oder mikrobiell kontaminierte Seren, da sich niedrigere Titer oder uneindeutige Fluoreszenzmuster ergeben könnten. Testverfahren Qualitätskontrolle Positiv- und Negativkontrollen sollten bei jedem Testen von Proben mitgeführt werden. 1. PBS II-Konzentrat verdünnen. Verdünnen Sie das PBS II-Konzentrat mit destilliertem bzw. entionisiertem Wasser (1 Teil PBS II-Konzentrat + 39 Teile destilliertes bzw. entionisiertes Wasser) und mischen Sie. Hinweis: Bereiten Sie nur die gesamte Menge an PBS II zu, wenn das Testkit innerhalb von einem Monat aufgebraucht wird. Der PBS II-Puffer wird zum Verdünnen der Patientenproben und als Waschpuffer verwendet. 2. Patientenproben verdünnen. Patientenproben 1:1 verdünnen, indem Sie 2 l Serum zu 18 l PBS II-Puffer hinzufügen. 3. Substrat-Objektträger. Lassen Sie den/die Objektträger vor der Entnahme aus dem Beutel Raumtemperatur annehmen (18-28 C). Objektträger entsprechend beschriften, in die feuchte Kammer legen und je einen Tropfen Positiv- und Negativkontrolle in die entsprechenden Vertiefungen geben. Geben Sie 5 l der verdünnten Patientenprobe in die restlichen Vertiefungen. 4. Objektträger inkubieren. Inkubieren Sie die Objektträger 3 Minuten bei Raumtemperatur (18-28 C) in einer feuchten Kammer. (Angefeuchtete Papiertücher auf dem Kammerboden sorgen für ausreichende Feuchtigkeit.) 5. Mit PBS II waschen. Nehmen Sie die Objektträger aus der feuchten Kammer und spülen Sie sie kurz mit der PBS II-Spritzflasche ab. Den Strahl nicht direkt auf die Vertiefungen richten. Geben Sie die Objektträger in ein Rack, tauchen Sie dieses in ein PBS II-Pufferbad ein und schütteln bzw. rühren Sie 5-1 Minuten lang. 6. Fluorescein-Konjugat zugeben. Überschüssigen PBS II-Puffer ablaufen lassen. Legen Sie die Objektträger zurück in die feuchte Kammer und bedecken Sie jede Vertiefung sofort mit einem Tropfen des entsprechenden Fluorescein-Konjugats. Das Anti-Human-IgG (H&L)-AFF-FITC dient für den Nachweis von Haut-Autoantikörpern und das Anti-Human-IgA ( -Kette)-AFF-FITC für den Nachweis von Endomysium-Autoantikörpern. DIE VERTIEFUNGEN NICHT LÄNGER ALS 15 SEKUNDEN UNBEDECKT LASSEN. 7. Objektträger inkubieren. Inkubieren Sie die Objektträger 3 Minuten in einer feuchten Kammer bei Raumtemperatur (18-28 C) im Dunkeln. 8. Mit PBS II waschen. Wiederholen Sie den Waschschritt unter Punkt Optionale Gegenfärbung. Geben Sie 2-3 Tropfen Evans-Blau 1% pro 1ml PBS II vor dem Eintauchen der Objektträger hinzu. 1. Deckgläser aufsetzen.nehmen Sie die Objektträger nacheinander aus dem PBS II-Pufferbad. Um die Vertiefungen herum rasch abtrocknen und einen Tropfen Eindeckmedium in jede Vertiefung geben. Das Deckglas vorsichtig auf den Objektträger legen, dabei Luftbläschen möglichst vermeiden. Versuchen Sie nicht, etwaige vorhandene Luftblasen zu entfernen. Überschüssiges Eindeckmedium mit Filterpapier entfernen. 11. Objektträger unter einem Fluoreszenzmikroskop auswerten. Die Objektträger können bis zu 3 Tage bei 2-8 C im Dunkeln ohne signifikanten Fluoreszenzverlust aufbewahrt werden. Ergebnisse Qualitätskontrolle Die Positivkontrollen sollten ein leuchtend-apfelgrünes Fluoreszenzmuster im interzellulären Zement des Epithels (retikulär-wabenartiges Muster) für Pemphigus und Retikulin-ähnliche Fasern im Bindegewebe um die glatte Muskulatur für Endomysium ergeben. Die Negativkontrolle sollte eine mattgrüne Anfärbung des gesamten Gewebes ohne erkennbare Fluoreszenz ergeben. Erscheinen die Kontrollen nicht wie oben erläutert, ist der Test ungültig und muss wiederholt werden. 8

9 Auswertung der Ergebnisse Siehe Quellenangabe 5 für Farbfotos as Beispiele für diese Muster. Die Ergebnisse werden als positiv oder negativ protokolliert. Pemphigus-Antikörper positiv Anfärbung des interzellulären Zements des mehrschichtigen Epithels. Pemphigoid-Antikörper positiv Anfärbung der Basalmembranzone entlang der dermo-epidermalen Junktionszone. Endomysium-Antikörper positiv 4 Anfärbung der Retikulin-ähnlichen Fasern im Bindegewebe rund um Fasern der glatten Muskulatur. Hinweis: Jedes Labor sollte für sich festlegen, wann ein positives Ergebnis als klinisch relevant gilt. Grenzen des Verfahrens 1. Die Lichtquelle, Filter und Optik von Fluoreszenzmikroskopen verschiedener Hersteller wirken sich auf die Sensitivität des Tests aus. Die Mikroskopleistung hängt in hohem Maße von einer ordnungsgemäßen Instandhaltung ab, v.a. Zentrierung der Quecksilberdampflampe und Ersatz der Lampe nach der empfohlenen Zeitdauer. 2. Primaten-Ösophagus-Gewebe ist, unabhängig von dem verwendeten Herstellungsprozess für die Gewebe-Objektträger, von Natur aus anfällig für Autofluoreszenz. Diese Autofluoreszenz ist in der Lamina propria und den submukösen Regionen besonders ausgeprägt. Deshalb sollten Proben, die schwach positive Fluoreszenzmuster (speziell Endomysium und glatte Muskulatur) in der Lamina muscularis mucosae erzeugen, mit Vorsicht interpretiert werden. 3. Antinukleäre Antikörper (ANA), antimitochondriale Antikörper (AMA), Antikörper gegen glatte Muskulatur (ASMA) und Antikörper gegen Skelettmuskel können mit dem Ösophagusgewebe reagieren. Das Vorhandensein dieser Autoantikörper sollte auf dem entsprechenden Substrat bestätigt werden. 4. Kreuzreaktivität aufgrund von Anti-A -and Anti-B-Blutgruppen-Antikörpern kann auftreten, da die Ösophagusschleimhaut bestimmte Blutgruppensubstanzen enthalten kann 6. Bei einigen Patientenseren kann durch diese Blutgruppen-Antikörper ein Pemphigus-Muster vorgetäuscht werden, wobei jedoch auch die Kapillaren in der Ösophagus-Muskulatur angefärbt werden. 5. Aufgrund des geringen Abstandes zwischen den Vertiefungen bei Objektträgern mit 1 Vertiefungen besteht eine gewisse Verschleppungsgefahr. Deshalb muss, speziell bei den Waschschritten, besonders sorgfältig gearbeitet werden, um eine Verschleppung zu vermeiden. 6. Dieser Test allein ist nicht als diagnostischer Beweis ausreichend. Alle anderen Faktoren, wie etwa Anamnese des Patienten und andere serologische Ergebnisse bzw. Ergebnisse aus Biopsien, sind ebenfalls zu berücksichtigen. 7. Die Eignung zur Verwendung mit IFA-Reagenzien anderer Hersteller wurde nicht untersucht, ist jedoch nicht zwangsläufig ausgeschlossen. Separat erhältliche Objektträger sind als analytspezifische Reagenzien" klassifiziert. Die analytischen und leistungsbezogenen Eigenschaften wurden lediglich als Bestandteil des Kits bestimmt. Erwartungswerte Patienten mit Pemphigus- und Pemphigoid-Erkrankungen sowie 2 zufällig ausgewählte Blutspender wurden anhand von Primaten-Ösophagus-Objektträgern getestet. 10 Infektiöse Diarrhoe Kleinkind-Diarrhoe Rezidivierende Diarrhoe Milcheiweißunverträglichkeit Kontrollgruppe (Hauterkrankungen) Lineare IgA-Dermatose Nicht-Dermatitis herpetiformis Hauterkrankungen Die Daten wurden verschiedenen Studien entnommen, Chorzelski et al /21 /17 /124 /6 /41 /18 Spezifische Leistungsdaten Eine Vergleichsstudie mit 43 Seren (35 klinische und 8 normale Spender) wurde mit diesem Kit und zwei anderen im Handel erhältlichen Referenzmethoden, eine für den Nachweis von Pemphigus- und Pemphigoid-Antikörpern, die andere zum Nachweis von Endomysium-Antikörpern, durchgeführt. Die Übereinstimmung zwischen den Testkits war insgesamt gut. 1 von 13 Proben ergaben bei beiden Kits ein positives Ergebnis für Pemphigus. Zwei der abweichenden Proben waren grenzwertig positiv bei diesem Kit und negativ beim Referenzkit. Für Pemphigoid stimmten alle vier getesteten positiven Proben überein, wobei die Anfärbung beim Konkurrenzprodukt etwas schwächer war. Für Endomysium ergaben alle 18 positiven Proben bei beiden Methoden ein positives Ergebnis. Die acht normalen Proben ergaben bei allen drei Testkits ein negatives Ergebnis. Kurzarbeitsanleitung 1. PBS II mit destilliertem oder entionisiertem Wasser verdünnen. 2. Patientenseren 1:1 mit PBS II verdünnen. 3. Objektträger auf Raumtemperatur (18-28 C) erwärmen lassen. 4. Je 5 l Positiv- und Negativkontrollen sowie verdünnte Patientenseren in die entsprechenden Vertiefungen geben Minuten in einer feuchten Kammer inkubieren Minuten in PBS II waschen. 7. 11 NOVA Lite Monkey Oesophagus IFA Kit/Slides Per uso diagnostico in vitro Codice prodotto: 74145, 7415, 74155, 54145, , 5415, , Complessità CLIA: elevata Finalità d'uso L'utilizzo di sezioni di esofago di scimmia come substrato è indicato per lo screening degli anticorpi contro gli antigeni intraepidermici o le zone della membrana basale nel siero umano nonché come sussidio nella diagnosi rispettivamente di pemfigo e pemfigoide bolloso. Le sezioni possono inoltre essere usate per il rilevamento di anticorpi anti-endomisio quale ausilio nella diagnosi di celiachia. Riassunto e spiegazione del test Gli anticorpi IgG intraepidermiche che caratterizzano diverse forme cliniche di pemfigo reagiscono con gli antigeni presenti sulla superficie cellulare dei cheratinociti epidermici. Se il risultato del test è positivo, si ha un tipico modello a rete. Il deposito tissutale di immunoglobuline avviene praticamente in tutti i casi. Nelle malattie bollose si raccomanda di analizzare il siero sia i tessuti del paziente. Gli anticorpi diretti verso la membrana basale si riscontrano tradizionalmente nel pemfigoide bolloso. In questa patologia, la biopsia diretta rivela un deposito lineare di IgG e C3 lungo la giunzione dermoepidermica. Questi autoanticorpi sono presenti di norma, anche se non sempre, nel siero dei pazienti affetti dalla patologia. Tuttavia, il titolo anticorpale non è correlato allo stato patologico e durante la remissione si possono ancora riscontrare degli anticorpi sia nei tessuti sia nel siero. Il kit fornisce un coniugato di anti-igg umane (H&L) FITC per ricercare entrambi gli autoanticorpi. Anticorpi anti-endomisio vengono diretti contro le fibre simili alla reticolina nel tessuto connettivo attorno alle fibre di muscolo liscio nei tratti intestinali dei primati. È stata riferita una sensibilità e una specificità vicina al 1% per gli anticorpi antiendomisio di classe IgA attivi nella celiachia 1,2. Questi anticorpi vengono rilevati dal coniugato anti-iga umane FITC fornito. Principi della metodica Il kit impiega una tecnica di immunofluorescenza indiretta 3 in cui i campioni dei pazienti e i relativi controlli sono incubati sui vetrini con substrato. Gli anticorpi senza reazioni vengono lavati e quindi vengono applicati i coniugati appropriati marcati con fluoresceina. Il coniugato non legato viene eliminato con il lavaggio. I vetrini vengono letti con un microscopio a fluorescenza. I campioni positivi presentano una fluorescenza di calore verde mela che corrisponde alle aree della sezione dove l'autoanticorpo si è legato. Reagenti 1. Sezioni di esofago di scimmia su vetrini con 5 o 1 pozzetti con essiccante Solo kit e 7415: 1. 12 Avvertenze/Precauzioni Il siero di tutti i donatori umani fornito (solo kit) è stato testato ed è risultato negativo all'antigene di superficie dell'epatite B, agli anticorpi del virus dell'epatite C e del HIV 1 e 2. Tuttavia, questi test non possono garantire l'assenza di agenti infettivi. Devono essere stabiliti metodi di trattamento e di eliminazione adeguati corretti per tutti i materiali potenzialmente infettivi e le procedure devono essere eseguite soltanto da personale esperto ed autorizzato. Il Blu di Evan e i controlli del kit contengono lo,9% di sodio azide come conservante e devono essere maneggiati con cautela: non ingerire né permettere il contatto con la pelle o le membrane mucose. In caso di contatto, lavare con acqua abbondante e rivolgersi al medico. A contatto con tubature di piombo o rame possono formarsi azidi metallici esplosivi; quando si smaltisce il reagente, lavare con acqua abbondante per impedire l'accumulo di azide. Questo prodotto deve essere usato solo da coloro che sono addestrati in modo corretto per gli scopi indicati. Si raccomanda un rigido rispetto del protocollo della procedura. Condizioni di conservazione I kit non aperti devono essere conservati a 2-8 C e possono essere usati fino alla data di scadenza indicata. NON CONGELARE. Una volta estratti dall'involucro in alluminio, i vetrini devono essere utilizzati immediatamente. Il tampone PBS II diluito può essere conservato fino a un mese a 2-8 C. Tutti gli altri reagenti devono essere conservati a 2-8 C. Raccolta dei campioni I campioni di sangue devono essere raccolti mediante venipuntura, deve essere permesso loro di coagulare naturalmente e il siero deve essere separato il prima possibile per impedire l'emolisi. Il siero deve essere conservato a 2-8 C per massimo 7 giorni prima del test oppure, per periodi prolungati, deve essere conservato a una temperatura pari a -2 C o inferiore 8. NON congelare e scongelare il siero più di una volta. Evitare di usare sieri lipemici, emolizzati o contaminati con microbi in quanto possono verificarsi titoli diminuiti o quadri di colorazione non chiari. Procedura del test Controllo di qualità Ogni volta che vengono testati campioni devono essere utilizzati campioni positivi e negativi. 1. Diluire il concentrato di PBS II. Diluire il concentrato di PBS II con acqua distillata o deionizzata (1 parte di concentrato PBS II + 39 parti di acqua distillata o deionizzata) e miscelare. NB: preparare l'intera quantità del kit PBS II solo se si prevede di utilizzarlo completamente entro un mese. Il tampone PBS II viene usato per diluire i campioni dei pazienti e come tampone di lavaggio. 2. Diluire i campioni dei pazienti. Diluire i campioni dei pazienti 1/1 dispensando 2 l di siero in 18 l di PBS II. 3. Vetrini di substrato. Attendere che i vetrini di substrato raggiungano la temperatura ambiente (18-28 C) prima di rimuoverli dalle buste. Etichettare accuratamente i vetrini, collocarli nella camera umida e dispensare i controlli positivi e negativi nei pozzetti appropriati. Dispensare 5 l di campione del paziente diluito nei pozzetti restanti. 4. Incubazione del vetrino. Incubare i vetrini per 3 minuti in una camera umida a temperatura ambiente (18-28 C). (Delle salviette in carta inumidite sul fondo della camera manterranno l'umidità.) 5. Lavaggio con PBS II. Rimuovere i vetrini dalla camera umida e lavarli brevemente con la spruzzetta di PBS II. Non spruzzare direttamente sui pozzetti. Posizionare i vetrini in un rack, immergerli in PBS II e agitare o mescolare per 5-1 minuti. 6. Aggiunta di coniugato fluorescente. Eliminare l'eccesso di PBS II scuotendo. Collocare nuovamente i vetrini nella camera umida e coprire immediatamente ogni pozzetto con una goccia di coniugato fluorescente appropriato. Il reagente anti-igg umane (H&L) AFF FITC serve per rilevare autoanticorpi cutanei mentre il reagente anti-iga umane (catena ) AFF FITC viene usato per rilevare gli autoanticorpi endomisiali. NON LASCIARE I POZZETTI SCOPERTI PER PIU' DI 15 SECONDI. 7. Incubazione del vetrino. Incubare i vetrini per 3 minuti in una camera umida a temperatura ambiente (18-28 C), al riparo dalla luce. 8. Tampone PBS II. Ripetere il lavaggio come descritto al Punto Colorazione di contrasto opzionale. Dispensare 2-3 gocce di Blu di Evan all'1% per 1 ml di PBS II prima dell'immersione del vetrino. 1. Montaggio con il coprivetrino. Rimuovere un vetrino per volta dal lavaggio con PBS II. Asciugare rapidamente attorno ai pozzetti e aggiungere una goccia di soluzione di montaggio in ogni pozzetto. Abbassare attentamente il vetrino sul coprivetrino, evitando le bolle d'aria; qualora queste fossero presenti, non tentare di rimuoverle. Asciugare il mezzo in eccesso attorno al bordo del coprivetrino. 11. Osservare i vetrini al microscopio a fluorescenza. I vetrini possono essere conservati per un massimo di 3 giorni a 2-8 C, al riparo dalla luce, senza che si verifichi una perdita significativa di fluorescenza. Risultati Controllo della qualità I controlli positivi devono presentare un pattern di colore verde mela brillante nel cemento intercellulare dell'epitelio ("a rete") per il pemfigo e fibre "simili a reticolina" nel tessuto connettivo attorno al muscolo liscio per il controllo endomisiale. Il controllo negativo produce una colorazione verde spento in tutti i tessuti, senza alcuna fluorescenza discernibile. Se i controlli non appaiono come descritto, il test non è valido e deve essere ripetuto. 13

14 Interpretazione dei risultati Vedere il riferimento 5 per esempi fotografici a colori di questi pattern. I risultati vengono riportati come positivi o negativi. Positivo agli anticorpi del pemfigo Colorazione del cemento intercellulare dell'epitelio stratificato. Positivo agli anticorpi del pemfigoide Colorazione della zona della membrana basale lungo la giunzione dermo-epidermica. Positivo agli anticorpi endomisiali 4 Colorazione delle fibre "simili a reticolina" nel tessuto connettivo attorno alle fibre di muscolo liscio. NB: ogni laboratorio deve definire a che punto un risultato positivo debba essere considerato clinicamente significativo. Limiti della procedura 1. La fonte luminosa, i filtri e le ottiche di microscopi di fluorescenza diversi influenzano la sensibilità del test. Le prestazioni del microscopio vengono influenzate in modo significativo da una corretta manutenzione specialmente se si centra la lampada al vapore di mercurio e si cambia la lampada dopo il periodo di tempo raccomandato. 2. Il tessuto di esofago di scimmia tende intrinsecamente all'autofluorescenza, indipendentemente dal processo usato per produrre i vetrini. Questo tende a essere più pronunciato nella lamina propria e nelle regioni sottomucose. Occorre pertanto prestare attenzione quando si interpretano i campioni che producono pattern di colorazione debolmente positivi nella mucosa muscolare, p.es. colorazione endomisiale e del muscolo liscio. 3. Gli anticorpi antinucleo (ANA), anti-mitocondrio (AMA), anti-muscolatura liscia (ASMA) e antimuscolatura striata possono reagire con il substrato di esofago. La presenza di questi autoanticorpi deve essere confermata su substrato specifico. 4. Può verificarsi una cross-reattività causata da anticorpi anti-gruppo A e B del sangue in quanto la mucosa dell'esofago può contenere determinate sostanze del gruppo sanguigno 6. In alcuni pazienti, il pattern di colorazione può mimare gli anticorpi anti-pemfigo benché gli anticorpi antigruppo sanguigno coloreranno anche i capillari nella muscolatura esofagea. 5. Data la ridotta distanza tra le pareti sui vetrini con 1 pozzetti, è possibile che si verifichi una contaminazione crociata dei campioni e dei controlli. Occorre prestare attenzione, in particolare durante le fasi di lavaggio, per garantire che questo non avvenga. 6. Questo test da solo non deve essere considerato diagnostico. Occorre tenere in considerazione tutti gli altri fattori tra cui l'anamnesi clinica dei pazienti e altri risultati sierologici o di biopsie. 7. L'adattabilità all'uso con reagenti IFA di altri produttori non è stata valutata, ma l'uso tali reagenti non deve essere necessariamente escluso. In generale, la correlazione tra i kit è risultata buona. Per il pemfigo, 1 su 13 campioni dosati sono risultati positivi in entrambi i kit. Las secciones también pueden utilizarse para la detección de anticuerpos antiendomisio y como ayuda para el diagnóstico de la celiaquía. Resumen y explicación de la prueba Los anticuerpos IgG intraepidérmicos, que caracterizan a varias formas clínicas de pénfigo, reaccionan con los antígenos de la superficie celular de los queratinocitos epidérmicos. Un resultado positivo muestra un patrón característico en tela metálica. Se produce depósito tisular de inmunoglobulina en casi todos los casos. Se recomienda investigar siempre el suero de los pacientes y también el tejido en caso de enfermedad ampollosa. Los anticuerpos frente a la membrana basal se encuentran habitualmente en el penfigoide ampolloso. En esta enfermedad, la biopsia directa revela el depósito lineal de IgG y C3 a lo largo de la unión dermoepidérmica. Normalmente, aunque no siempre, estos autoanticuerpos están presentes en el suero de los pacientes afectados. Sin embargo, el título de anticuerpos no se correlaciona con el estado de la enfermedad y, en caso de remisión clínica, el anticuerpo puede seguir presente tanto en los tejidos como en el suero. Con este kit se suministra un conjugado de IgG antihumano (H y L) con FITC para poner de manifiesto ambos autoanticuerpos. Los anticuerpos antiendomisio se dirigen contra las fibras de tipo reticulina del tejido conectivo situadas alrededor de las fibras de músculo liso en el tracto intestinal de los primates. Se ha comunicado una sensibilidad y especificidad de casi el 1% para los anticuerpos antiendomisio de clase IgA activos en la celiaquía 1,2. Estos anticuerpos son detectados mediante el conjugado de IgA antihumano con FITC proporcionado. Principios del procedimiento El kit emplea una técnica de inmunofluorescencia indirecta 3 en la que las muestras de los pacientes y los controles adecuados se incuban junto con las secciones. Advertencias/precauciones Todos los sueros de donantes humanos suministrados han sido analizados y han dado un resultado negativo para el antígeno de superficie de la hepatitis B y para los anticuerpos contra el virus de la hepatitis C y contra el virus de la inmunodeficiencia humana (VIH 1 y VIH 2). Sin embargo, estos análisis no pueden garantizar la ausencia de agentes infecciosos. Deben establecerse métodos adecuados de manipulación y eliminación, al igual que para cualquier material potencialmente infeccioso, y los procedimientos correspondientes debe realizarlos únicamente personal que tenga la formación adecuada en tales métodos. El azul de Evans y los controles del kit contienen un,9% de azida sódica como conservante y debe manipularse con precaución; no debe ingerirse y debe evitarse el contacto con la piel o con membranas mucosas. Condiciones de conservación Los kits no abiertos deben conservarse a 2-8 C y utilizarse antes de la fecha de caducidad indicada. NO DEBEN CONGELARSE. Los portaobjetos deben utilizarse inmediatamente después de extraerlos de su bolsa de aluminio. El tampón de PBS II diluido puede almacenarse como máximo durante un mes a 2-8 C. Todos los demás reactivos deben conservarse a 2-8 C. Recogida de muestras Las muestras de sangre deben recogerse mediante venopunción y deben dejarse coagular de forma natural; el suero debe separarse cuanto antes para evitar que se produzca hemólisis. Procedimiento de ensayo Control de calidad Deben utilizarse controles positivos y negativos cada vez que se analicen muestras. 1. Diluya el concentrado de PBS II. Diluya el concentrado de PBS II con agua destilada o desionizada (1 parte de concentrado de PBS II + 39 partes de agua destilada o desionizada) y mézclelo. Nota: Prepare todo el volumen de PBS II del kit únicamente si va a utilizarse todo el kit en el plazo de un mes. El tampón de PBS II se utiliza para diluir las muestras de los pacientes y como tampón de lavado. 2. Diluya las muestras de los pacientes. Diluya las muestras de los pacientes a 1/1 añadiendo 2 L de suero a 18 L de tampón de PBS II. 3. Portaobjetos con sustrato. Deje que los portaobjetos con sustrato alcancen la temperatura ambiente (18-28 C) antes de extraerlos de las bolsas. Etiquete los portaobjetos adecuadamente, colóquelos en la cámara de humedad y añada una gota de los controles positivo y negativo a los pocillos apropiados. Añada 5 L de muestra de paciente diluida a los demás pocillos. 4. Incubación de los portaobjetos. Incube los portaobjetos durante 3 minutos en una cámara de humedad a temperatura ambiente (18-28 C). (La humedad puede mantenerse colocando toallitas de papel humedecidas en la parte inferior de la cámara.) 5. Lavado con PBS II. Retire los portaobjetos de la cámara de humedad y aclárelos brevemente mediante el frasco comprimible de PBS II. No aplique el PBS II directamente sobre los pocillos. Coloque los portaobjetos en una gradilla y, a continuación, sumérjalos en PBS II y agítelos o remuévalos durante 5-1 minutos. 6. Adición del conjugado de fluoresceína. Incube los portaobjetos durante 3 minutos en una cámara de humedad a temperatura ambiente (18-28 C) y en la oscuridad. 8. Lavado con PBS II. Repita el lavado tal como se describe en el paso Contratinción opcional. Añada 2-3 gotas de azul de Evans al 1% por cada 1 ml de PBS II antes de realizar la inmersión de los portaobjetos. 18

19 1. Montaje con cubreobjetos. Retire los portaobjetos del lavado con PBS II de uno en uno. Seque rápidamente la zona de alrededor de los pocillos y añada una gota de medio de montaje a cada pocillo. Deposite con cuidado el portaobjetos sobre el cubreobjetos; evite que se formen burbujas de aire, pero si las hay no intente retirarlas. Elimine el exceso de medio de montaje de los bordes del cubreobjetos. 11. Observe los portaobjetos con un microscopio de fluorescencia. Los portaobjetos pueden conservarse durante un máximo de 3 días a 2-8 C, en la oscuridad, sin pérdida significativa de fluorescencia. Resultados Control de calidad Los controles positivos deberían dar un patrón de tinción de color verde manzana brillante en el cemento intercelular del epitelio (patrón en tela metálica) para el pénfigo y fibras de tipo reticulina en el tejido conectivo de alrededor del músculo liso para el control de endomisio. El control negativo debe presentar una tinción de color verde apagado en todo el tejido, sin fluorescencia discernible. Si los controles no cumplen estos criterios, el análisis no es válido y debe repetirse. Interpretación de los resultados En la referencia bibliográfica número 5 pueden consultarse ejemplos de estos patrones en fotografías en color. Los resultados se notifican como positivo o negativo. Nota: Cada laboratorio debe establecer el punto en el que un resultado positivo se considera clínicamente significativo. Limitaciones del procedimiento 1. La fuente de luz, los filtros y las ópticas de las diferentes marcas de microscopios de fluorescencia influyen en la sensibilidad del análisis. El rendimiento del microscopio depende de un modo significativo de que se le efectúe un correcto mantenimiento, en especial el centrado de la lámpara de vapor de mercurio y el cambio de la lámpara después del período de tiempo recomendado. 2. El tejido de esófago de mono es inherentemente propenso a la autofluorescencia, con independencia del proceso de fabricación de los portaobjetos de tejido. Esta tendencia es más pronunciada en las regiones de la lámina propia y de la submucosa. Por tanto, debe tenerse cuidado al interpretar las muestras que presentan patrones de tinción positivos débiles en la capa muscular de la mucosa, es decir tinción de endomisio y de músculo liso. 3. Loa anticuerpos antinucleares (ANA), antimitocondriales (AMA), anti-músculo liso (ASMA) y antimúsculo esquelético pueden reaccionar con el sustrato de esófago. La presencia de estos autoanticuerpos debe ser confirmada en el sustrato apropiado. 4. Se puede producir reactividad cruzada debido a los anticuerpos de grupo sanguíneo anti-a y anti-b, ya que la mucosa de esófago puede contener ciertas sustancias de grupo sanguíneo. 6 En algunos pacientes, el patrón de tinción puede imitar al de los anticuerpos de pénfigo, aunque los anticuerpos de grupo sanguíneo también tiñen los capilares de la musculatura esofágica. 5. Debido a la pequeña distancia entre los pocillos en los portaobjetos de 1 pocillos, es posible que se produzca contaminación cruzada de las muestras y los controles. Para el penfigoide, las cuatro muestras positivas analizadas concordaron, aunque la tinción fue ligeramente más débil en el kit de referencia. Para las muestras de endomisio positivas analizadas, todas las dieciocho fueron positivas con ambos métodos. Las ocho muestras normales analizadas fueron negativas en los tres kits. Resumen del procedimiento 1. Diluya el PBS II con agua destilada o desionizada. 2. Diluya los sueros de los pacientes a 1/1 con PBS II. 3. Equilibre los portaobjetos de sustrato a temperatura ambiente (18-28 C). 4. Dispense 5 L de los controles positivo y negativo y los sueros diluidos de los pacientes a los pocillos apropiados. 5. Incube en una cámara de humedad durante 3 minutos. 6. Lave durante 5-1 minutos con PBS II. 7. Agite para eliminar el exceso de PBS y cubra inmediatamente cada uno de los pocillos con una gota de conjugado. 8. Incube igual que en el paso Lave como en el paso Realice el montaje. 11.
