INTENDED USE This immunoassay is for the quantitative detection of IgG, IgA or IgM antibodies to Helicobacter pylori in human serum. These reagents are for in vitro diagnostic use only. SUMMARY OF THE TEST Helicobacter pylori has been found in biopsy samples from gastric epithelium from patients showing active type B 1,2,3,4,5 gastritis. Although the route of infection is unknown, reports have shown that H. pylori infection is associated with chronic gastritis. 6,7 A correlation has been found between the presence of H. pylori and gastric lesions in some cases of duodenal ulcers. 8 Complete resolution of gastritis after eradication of the organism has also been reported. 9 H. pylori is a gram negative, curved, spiral-shaped rod, µm in width by µm in length. It colonizes the deep portion of the mucous gel layer and the apical surface of the gastric mucosal epithelial cells. It may also be located in the junction between adjacent mucosal epithelial cells. The presence of H. pylori can be detected by both invasive and non-invasive methods. Invasive methods include testing biopsy specimens by culture, histology and rapid urease. Due to the patchy distribution of H. pylori in tissue, false negative results are common. Non-invasive procedures include serum tests for antibodies against H. pylori and the urea breath test which utilizes radiolabelled urea. The mechanisms by which H. pylori causes disease is not well understood. It can survive in the gastric mucosa, possibly for several years. H. pylori is also common in individuals who are considered healthy, with no clinical symptoms of disease. PRINCIPLE OF THE PROCEDURE The GAP IgG, IgA and IgM kits are quantitative ELISA assays for the detection of antibodies specific for H. pylori in human serum. Partially purified H. pylori antigens are immobilized on the wells of a microtiter plate and diluted patient serum is added to the wells. The antibodies in the serum, specific to H. pylori, will bind to the antigen on the wells. Non-specific antibodies are washed away. Antihuman IgG, IgA or IgM enzyme conjugate is added to the wells and binds to the antibody-antigen complex. Excess enzyme conjugate is washed way. The color is developed by the addition of an enzyme substrate. The intensity of the color is directly proportional to the amount of antibody present in the serum sample. KIT COMPONENTS PLA H.pylori = Antigen-Coated Wells 96 microtiter wells coated with inactivated H. pylori antigens. CAL 0-4 = Calibrator Set 5 vials, 1 ml each of zero, 12.5, 25, 50 and 100 units/ml of IgG, IgA or IgM antibodies against H. pylori in buffered human serum. CTRL + - = Control Set 1 ml each positive and negative for IgG, IgA or IgM antibodies against H. pylori in buffered human serum. DIL SPE 25X = Sample Diluent Concentrate 20 ml of phosphate buffered saline with Tween. CONJ ENZ = Enzyme Conjugate 10 ml of goat antihuman IgG, IgA or IgM horseradish peroxidase conjugate in a TRIS/BSA buffer. SUBS A TMB = Substrate Solution A 8 ml TMB in an acetate buffer with DMSO. SUBS B H2O2 = Substrate Solution B 8 ml hydrogen peroxide in an acetate buffer. BUF WASH 50X = Wash Concentrate (50x) 20 ml phosphate buffered saline with Tween. 2 Store all kit components at 2 8 C Calibrators, controls and serum diluent contain 0.09% sodium azide as a preservative. ADDITIONAL EQUIPMENT NEEDED Microplate reader Microplate washer Pipettes capable of dispensing 25, 50 and 100 µl Volumetric pipettes: 5 and 10 ml Graduated cylinders: 500 and 1000 ml Test tubes: 13 x 100 mm Plastic film Deionized water PRECAUTIONS The components of this product contain material of human origin which has been tested by FDA approved methods and found to be non-reactive for Hepatitis B Surface Antigen (HbsAg), HIV-1/2 and HCV antibodies. However, no test method can offer complete assurance of safety. These components have NOT been tested for antibodies to HTLV-1 or HTLV-II. For these reasons it is recommended that the components of this product be considered potentially infectious and handled with the same precautions used with patient samples. Some reagents in this kit contain sodium azide as a preservative. Sodium azide may react with lead and copper plumbing to form explosive metal azides. When disposing of these reagents, always flush with large volumes of water to prevent azide build-up. Dimethyl sulfoxide and H 2 SO 4 are skin and eye irritants. Avoid skin and eye contact. Wear gloves and safety glasses. If skin or eye contact occurs, flush with water for a minimum of 5 minutes. REAGENT PREPARATION AND STORAGE Sample Diluent Solution: Add the entire contents of the vial of Sample Diluent concentrate into a 500 ml container. Rinsing the vial with water to remove any precipitated crystals, add 480 ml of deionized water. Mix thoroughly. Wash Solution: Add the entire contents of the Wash Concentrate into a 1 liter container. Rinsing the vial with water to remove any precipitated crystals, add 980 ml of deionized water. Mix thoroughly. Working Substrate Solution. As an example, for 6 microwell strips, mix 3.0 ml of Substrate Solution A with 3.0 ml of Substrate Solution B. Store in the dark or cover with aluminum foil. Use within 1 hour. The wash solution, sample diluent solution and open kit reagents are stable for 60 days, when stored at 2 8 C. SPECIMEN COLLECTION AND STORAGE The specimen should be serum. The usual precautions for venipuncture should be observed. Do not use anticoagulants or preservatives. Do not use grossly hemolyzed or grossly lipemic specimens. Serum may be stored at 2-8 C for up to 10 days; for longer periods, serum should be stored at 20 C. SPECIMEN PREPARATION Each serum sample should be diluted with the Sample Diluent Solution. Add 25 µl of the serum sample to 5 ml of Sample Diluent Solution. Mix thoroughly. Controls and calibrators are supplied prediluted and are ready to use. PROCEDURE 1. All reagents and samples should be brought to room temperature (18-25 C) before the start of the assay. Assemble the desired number of microwell strips into the strip holder. 2. Pipet 100 µl of each calibrator, control and diluted patient sample into each well. The calibrators and controls are prediluted and ready to use. Cover the plate with plastic film and incubate for 1 hour at C. 3. Decant the wells and wash 3 times with 300 µl of Wash Solution. Blot thoroughly after each wash step. 4. Add 100 µl of Enzyme Conjugate to each well. Cover the plate with plastic film and incubate for 30 minutes at C. 5. Prepare the Working Substrate Solution. See REAGENT PREPARATION for more information. 6. Decant the wells and wash 3 times with 300 µl of Wash Solution. Blot thoroughly after each wash step. 7. Add 100 µl of the Working Substrate Solution to each well. Cover the plate with plastic film and incubate for 10 minutes in the dark at C. Working Substrate Solution: Within 1 hour of use, mix equal volumes of Substrate Solution A and Substrate Solution B. Prepare only the required amount. Each microwell strip will require a minimum of 800 µl of GAP IgG, IgM, IgA H. pylori ELISA Instruction Manual Page BR_eng DEC2003

3 8. Immediately after incubate, add 50 µl of Stop Solution to each well. Mix the reagents by gently tapping the plate. 9. Using a wavelength of 450 nm, measure the optical density of each well. The plate should be read within 30 minutes of adding the Stop Solution. Use quadratic curve fitting for the calibration curve and for calculating results. QUALITY CONTROL In keeping with good laboratory practice, the Positive and Negative Controls should be run in parallel with patient specimens. Failure to obtain the appropriate values for controls may indicate imprecise manipulations, improper sample handling or deterioration of reagents. GAP IgG EXPECTED VALUES An independent study was performed at four separate clinics using the GAP IgG H. pylori assay on 173 patients who presented with clinical symptoms of gastritis. The following table indicates the percentage of those patients that had a positive result on the GAP IgG test separated by diagnosis. Biopsy samples taken from these patients were tested by culture, urease and/or histological stains. The GAP results were compared to the biopsy results. 4 BETWEEN-RUN PRECISION Between-run precision was determined by measuring three serum samples on ten separate runs, using four different operators. GAP-IgG Mean Standard Deviation Coefficient of Variation (%) GAP-IgA Mean Standard Deviation (U/mL Coefficient of Variation (%) GAP IgM Mean Standard Deviation (U/mL Coefficient of Variation (%) LIMITATIONS OF THE PROCEDURE The GAP IgG, IgA and IgM assays are quantitative tests to detect the presence of antibodies specific for H. pylori and does not indicate the titer of the antibody. A positive test result does not distinguish between colonization or infection by H. pylori and does not indicate the presence of gastrointestinal disease. A negative test result does not preclude the presence of H. pylori. Colonization may be present but in its very early stages or the antibody titer may be too low for the test to detect. Persons over 50 years of age have a higher likelihood of past encounters with H. pylori and may have low but detectable levels of antibodies specific for H. pylori without clinical symptoms. 10,11,12 Additionally, the incidence of seropositivity can vary by population. 13 The GAP IgG, IgA and IgM H. pylori assays should be used only for patients reporting clinical signs and symptoms related to gastritis. These tests should not be used on patients who are asymptomatic. DISTR. ANWENDUNG Quantitativer Enzymimmunoassay zum Nachweis von spezitischen IgG, IgA oder IgM Antikörper gegen Helicobacter pylori. Alle Reagenzien sind nur zur in-virtodiagnostik bestimmt. EINLEITUNG Helicobacter pylori wurde in Biopsiematerial aus den Epithelzellen des Magens von Patienten gefunden, die an 1,2,3,4,5 einer aktiven Typ-B-Gastritis erkrankt waren. Obwohl der Infektionsweg unbekannt ist, zeigen Untersuchungen und Studienergebnisse deutlich, Daß H. pylori Infektionen mit chronischen Gastritiden assöziieren. 6,7 Auch wurde ein Zusammenhang zwischen dem Auftreten von H. pylori und Magenfunktionss-törungen bei Zwölffingerdarmgeschwüren festgestellt. 8 Nach der Ausrottung des Keimes konnte in vielen Fällen eine vollständige Ausheilung der Gastritis verzeichnet werden. 9 H. pylori ist ein Gram-negatives, gekrümmtes, spiralförmiges Stäbchenbakterium mit einer Breite von 0,2 0,8 µm und einer Länge von 0,5 5,0 µm. Es bildet Kolonien unter der Oberfläche der Schleimhaut des Antrums und wurde in großer Zahl in Canaliculi und Tubulovesikeln gefunden. Es ist auch in den Verbin-dungsbereichen zwischen den angrenzenden Mucosaepithelzellen lokalisiert. Die Anwesenheit von H. pylori kann sowohl durch invasive als auch durch nicht-invasive Methoden fest-gestellt werden. Zu den invasiven Methoden zählt die Entnahme von Biopsiematerial bei einer Gastroskopie mit anschließender Kultivierung der Keime, histologischer Untersuchungen und der Urease Schnelltest. Aufgrund der unregelmäßigen Ausbreitung von H. pylori im Gewebe werden häufig falsch negative Ergebnisse gefunden. Zu den nicht-invasiven Methoden gehört die Bestimmung von Antikörpern gegen H. pylori aus dem Serum und der Urease Atemtest, bei dem mit C 13 markierter Harnstoff eingesetzt wird. Der Mechanismus, durch den H. pylori Krankheiten verursacht, ist nicht vollkommen geklärt. H. pylori kann in der Magenschleimhaut bis zu mehreren Jahren überleben. Auch tritt das Bakterium bei gesunden Personen auf, die keinerlei klinische Symptome aufweisen. TESTPRINZIP Der GAP IgG, IgA bzw, IgM Test ist ein quantitativer ELISA Assay zur spezifischen Bestimmung von H. pylori Antikörpern in humanem Serum. Die Wandungen der Vertiefungen der Mikrotiterplatte sind mit gereinigten gruppenspezifischen H. pylori Antigenen beschichtet. Das verdünnte Patientenserum wird in diese Vertiefungen pipettiert. Die spezifischen IgG, IgA bzw. IgM H. pylori Antikörper im Serum binden an das spezifische Antigen in der Mikrotiterplatte. Nichtspezifische Antikörper werden durch die Waschschritte entfernt. Anti-humanes IgG, IgA bzw. IgM enzymkonjugat wird in die Vertiefungen pipettiert und bindet an die Antigen-Antikörper-Komplexe. Überflüssiges Enzymkonjugat wird ausgewaschen. Nach Zugabe eines Enzyumsubstrates kommt es zu einer Anfärbung der Lösung. Die Farbintensität ist direkt proportional zu der Konzentration an spezifischen IgG, IgA bzw. IgM Antikörpern im Patientenserum. TESTKOMPONENTEN PLA H.pylori = Antigen beschichtete Mikrotiterstreifen 12 Streifen a 8 Vertiefungen, beschichtet mit H. pylori Antigen. CAL 0-4 = Kalibratoren IgG, - IgA bzw, IgM positiven Human-serums in Serum verdünnungsreagenz in folgenden Konzentrationen: 0, 12.5, 25, 50 und 100 Einheiten/mL. CTRL + - = Positve und Negative Kontrollen 2 Fläschen mit je 1 ml H. pylori positiv-bzw, negative- Humanserum. DIL SPE 25X = Probenverdünnungsreagenz (25 x Konzentrat) 20 ml bestehend aus PBS mit PEG und Tween. CONJ ENZ = Enzymkonjugat 10 ml eines Antikörper- Enzym-Konjugats Ziege-Anti-Human-IgG, IgA bzw, IgM Antikörper markiert mit Meerrettichperoxidase (HRPO) in TRIS/BSA-Puffer. 6 SUBS A TMB = Substratlösung A 8 ml TMB in einem Acetat/DMSO-Puffer. SUBS B H2O2 = Substratlösung B 8 ml H 2 O 2 in einem Acetat/Puffer. BUF WASH 50X = Waschpuffer (50 x Konzentrat) 20 ml bestehend aus PBS mit Tween. SOLN STOPPING = Stoplösung 6 ml 1N H 2 SO 4 Lagerung bei 2-8 C Jeder Kalibrator, Kontrollen und Probenverdünnungsreagenz enthält 0,09% Natriumazid. ZUSÄTZLICHE ERFORDERLICHE Pipette oder Dispensor zum Transfer von Flüssigkeiten von 50 bis 200 µl. Mikrotiterplatten-Wascher Mikrotiterplatten-Lesegerät Mikropipetten Einmalspitzen für 25, 50 und 100 µl. Pipetten: 5 und 10 ml pipette Meßzylinder: 500 ml und 1L, graduiert. Reagenzgläser fur die Serumzugabe. Saugfähiges Papier Kunststoff-Folie Destilliertes Wasser VORSICHTSMASSNAHMEN Die Matrix der Kalibratoren besteht aus Humanserum, das auf HIV Antikörper und HbsAg getestet und für negativ befunden wurde. Diese Komponenten wurden NICHT auf Antikörper gegen HCV, HIV-2, HTLV-1 oder HTLV-II getestet. Da es jedoch kein Testverfahren gibt, das mit 100% iger Sicherheit das Vorhandensein von HIV, HBV oder anderer infekitöser Partikel ausschlie ßt, müssen diese Reagenzien als positiv infektios betrachtet und entsprechend behandelt werden. Einige Reagenzien enthalten Natriumazid also Konservierungsmittel. Verschlucken und Berührun mit der Haut oder den Schleimhäuten vermeiden. Natriumazid kann mit Bleioder Kuferrohren expolsive Metallizde bilden kann. Bei der Entsorgung bitte die entsprechenden Richtlinien beachten. Dimethylsulfoxid kann zu Verletzungen der Haut und der Augen führen. Vermeiden Sie den Kontakt mit Haut und Augen. Tragen Sie Handschuhe und eine Schutzbrille. Falls Dimethylsulfoxid auf die Haut oder in die Augen gerät, spülen Sie die betroffenen Stellen mindestens 5 Minuten mit Wasser. VORBEREITUNG DER REAGENZIEN Verdünnungspuffer für Serumproben: Den gesamten Inhalt der Konzentrat-Flasche in einen 500 ml Behälter mit 480 ml destilliertem oder entionisiertem Wasser geben (eventuell vorhandene Kristalle ausspülen). Gründlich mischen. Waschpuffer: Den gesamten Inhalt der Flasche mit dem Waschpuffer in einen 1 L. Behälter mit 980 ml dest oder entionisiertem Wasser geben (eventuell vorhanden Kristalle ausspülen). Gründlich mischen. Substratlösung: Substratlösung A und B im Verhältnis 1:1 in einem 13 x 100 mm Reagenzglas mischen. Für je 6 Mikrotiterstreifen (1/2 Platte) 3 ml Substratlösung A and 3 ml Substratlösung B mischen. Vor Gebrauch gründlich mischen. Die gebrauchsfertige Substratlösung innherhalb einer Stunde verwenden. Lagern Sie die Lösung im Dunkeln oder decken Sie sie bis zum Einsatz mit Aluminiumfolie ab. Die Waschlösung, die Lösung zur Verdünnung der Proben und die geöffneten Reagenzien im Testkit sind bei einer Lagertemperatur von 2-8 C bis zu 10 Tagen aufbewahrt werden. Kann die Probe innerhalb dieses Zeitraumus nicht untersucht werden, sollte sie bei -20 C eingefroren werden. PROBENENTNAHME Das Probenmaterial sollte Serum sein, das möglichst rasch nach der Sammlung vom Blutkuchen getrennt wurde. Hämolyse vermeiden. Serumproben können bei 2-8 C eingefroren werden. VERDÜNNUNG DER SERUMPROBEN Exakt 25 µl (0,025 ml) Patientenserum werden zu 5ml Serumprobem- Verdünnungpuffer (siehe Punkt 1 dieses Abschnitts) in einem Reagenzglas pipettiert. Durch Schüttein oder Vortexen gründlich mischen. Die Kontrollen werden unverdünnt direktin die entsprechenden Vertiefungen der Mikrotiterstreifen pipettiert. Es ist keine Verdünnung der Kontrollen vor Gebrauch erforderlich. TESTDURCHFÜHRUNG 1. Die erforderliche Anzahl Mikrotiterstreifen in die Halterung einsetzen. 2. Je 100 µl des IgG, IgA, bzw, IgM-Kalibratoren Kontrollen und verdünnten Patientenproben in die entsprechenden Vertiefungen pipettieren. Die Kontrollen werden unverdünnt direkt in die entsprechenden Vertiefungen der Mikrotiterstreifen pipettiert. Die Platte mit Plastikfolie und 60 Minuten bei C inkubieren. 7 3. Kippen Sie die Lösung aus der Mikrotiterplatte aus und waschen Sie 3 mal mit 300 µl Waschlösung. Trocknen Sie die Platte nach jedem Waschschritt auf einem Fließpapier ab µl der Anti-IgG, IgA bzw, IgM-enzymkonjugat. Lösung in alle Vertiefungen pipettieren. Die Platte mit Plastikfolie abdecken und 30 Minuten bei C inkubieren. 5. Die Herstellung der Substrat-Arbeitslösung sollte vor dem nachsten Arbeitsschritt erfolgen. 6. Kippen Sie die Lösung aus der Mikrotiterplatte aus und waschen Sie 3 mal mit 300 µl Waschlösung. Trocknen Sie die Platte nach jedem Waschschritt auf einem Fließpapier ab µl der frisch hergestellten Substratlösung in alle Vertiefungen pipettieren. Die Platte 10 Minuten bei C im Dunkeln inkubieren. 8. Nach genau 10 Minuten, schnell 50 µl Stoplösung in jede Vertiefung pipettieren. Mischen Sie die Reagenzien durch vorsichtiges Aufklopfen der Platte. 9. Die Extinktion in allen Vertiefungen bei 450 nm bestimmen. Die Extinktionsmessung sollte innherhalb von 30 Minute nach Zugabe der Stoplösung erfolgen. Stellen Sie die Standardkurve mit einer quadratischen Funktion dar und berechnen Sie hieraus die Ergebnisse. QUALITÄTSKONTROLLE Zur Sicherheit sollten Positiv- und Negativ-Kontrollen bei jedem Testlauf parallel mit den Patientenproben eingesetzt werden. Werden die angegebenen Kontrollwerte nicht gefunden, kann dies ein Hinweis auf eine ungenaue Handhabung bei der Testdurchführung sein, oder auf einen Verfall der Reagenzien hindeuten. Diese Probe sollte zusammen mit der ersten Probe analysiert werden. IgG ERWARTETE WERTE An vier verschiedenen Krankenhäusern wurden klinische Studienmit demtest durchgeführt. Insegesamt wurden 173 Proben von Patienten untersucht, die über klinische Anzeichen und Symptome, verursacht durch Magenerkrankungen, berichteten. In der folgenden Tabelle wird die prozentuale Anzahl der Patienten dargestellt, die ein positives GAP IgG Testergebnis nach entsprechender Diagnose aufgewiesen haben. Application Ce test immunoenzymatique est utilisé pour le dépistage quantitatif des anticorps IgG, IgA ou IgM spécifiques de Helicobacter pylori. Ces réactifs doivent être utilisés uniquement pour un diagnostic in vitro. But du dosage Helicobacter pylori a été retrouvée dans des échantillons de biopsies d'épithélium gastrique démontrant une gastrite type B active 1,2,3,4,5. Bien que l'origine de l'infection soit inconnue, des rapports démontrent que l'infection à H. pylori est associée à la gastrite chronique. 6,7 Une corrélation a été démontrée entre la présence de H. pylori et les lésions gastriques dans certains cas d'ulcères du duodénum. 8 La guérison complète de la gastrite après éradication de l'organisme a aussi été rapportée. 9 H. pylori est un bâtonnet en spirale, recourbé et gram négatif de 0.2 à 0.8 µm de large et 0.5 à 5 µm de long. Il colonise la partie profonde de la couche de gel muqueux et la surface apicale des cellules épithéliales de la muqueuse gastrique. Il peut aussi se retrouver dans l'interstice entre deux cellules épithéliales de la muqueuse. La présence de H. pylori peut être détectée par des méthodes invasives ou non-invasives. Parmi les méthodes invasives, l'analyse peut être réalisée à partir de biopsie, culture, histologie ou test à l'uréase rapide. Les résultats faussement négatifs sont courants parce que le H. pylori est distribué de façon très inégale dans les tissus. Parmi les méthodes non-invasives il y a aussi le dépistage d'anticorps anti-h. pylori et le test à l'urée en cours d'inspiration à l'aide d'urée radioactive. Le mécanisme d'infection par H. pylori n'est pas bien compris. Il peut survivre dans la muqueuse gastrique, possiblement pendant plusieurs années. H. pylori est aussi commun chez les individus considérés en bonne santé, sans symptômes cliniques de la maladie. Principe du test Les kits GAP IgG, IgA et IgM sont des dosages ELISA quantitatifs pour le dépistage d'anticorps spécifiques de H. pylori dans le sérum humain. Les sérums dilués des patients sont déposés dans les puits de microtitration recouverts d'antigènes de H. pylori partiellement purifiés. Les anticorps du sérum, spécifiques de H. pylori, réagiront avec les antigènes sur les puits. Les anticorps non spécifiques sont éliminés par lavage. Un conjugué enzymatique anti-igg, IgA et IgM humain est ajouté et se fixe au complexe antigène-anticorps. Le conjugué enzymatique excédentaire est éliminé par lavage. Après addition du substrat enzymatique, l'intensité de la coloration est proportionnelle à la quantité d'anticorps présent dans le sérum patient. Précautions Les composants d'origine humaine utilisés dans la préparation de ce produit ont été testés selon des méthodes approuvées par la FDA et trouvés non-réactifs en antigènes de surface de l'hépatite B (HBsAg), en anticorps anti-hiv-1/2 et en anticorps anti-hcv. Ces composants n'ont pas été testés pour les anticorps anti- HCV, HIV-2, HTLV I ou HTLV-II. Du fait qu'aucune méthode ne peut garantir de façon absolue l'absence d'agents infectieux, les composants de ce produit doivent être considérés comme potentiellement infectieux et manipulés avec les mêmes précautions d'usage que les échantillons patients. Certains réactifs du kit contiennent de l'azoture de sodium comme conservateur. L'azoture de sodium peut réagir avec le plomb et le cuivre des canalisations pour former des azides métalliques explosifs. Après évacuation de ces produits dans les canalisations, faire couler de l'eau abondamment afin d'éviter toute accumulation d'azoture. Le diméthyl sulfoxide et H 2 SO 4 sont irritants pour la peau et les yeux. Eviter le contact avec la peau et les yeux. Porter des gants et des lunettes de protection. S'il y a contact avec la peau ou les yeux, laver avec de l'eau au moins 5 minutes. Préparation des réactifs Diluant pour échantillon de sérum : Ajouter la totalité du contenu dans un récipient de 500 ml. Rincer le flacon à l'eau pour éliminer les cristaux. Ajouter 480 ml d'eau désionisée. Mélanger soigneusement. Tampon de lavage : Ajouter la totalité du contenu dans un récipient de 1 L. Rincer le flacon à l'eau pour éliminer les cristaux. Ajouter 980 ml d'eau désionisée. Mélanger soigneusement. Solution de substrat : Mélanger les solutions de substrat A et de substrat B volume à volume. Préparer seulement la quantité nécessaire. Pour 48 puits, ajouter 3 ml de la solution de substrat A à 3 ml de la solution de substrat B. Mélanger soigneusement. Employer cette solution dans l'heure qui suit la reconstitution. Conserver dans le noir ou recouvert d'un papier aluminium. La solution de lavage, le diluant pour échantillon et les réactifs de la trousse sont stables 60 jours après le premier usage si entreposé à 2-8 C. Prélèvement des échantillons Les échantillons doivent être des échantillons de sérum. Le sérum doit être séparé des cellules aussitôt que possible après prélèvement. Ne pas utiliser d'échantillons hémolysés ou hyperlipémiques. Les échantillons de sérum peuvent être gardés au réfrigérateur (2-8 C) jusqu'à 10 jours. Si les échantillons de sérum ne peuvent être analysés dans cette période, ils doivent être conservés à -20 C. Dilution de l'échantillon de sérum Chaque échantillon de sérum doit être dilué avec du diluant pour échantillon. Ajouter 25 µl de chaque sérum de patient à 5 ml de diluant pour échantillon. Mélanger soigneusement. Les contrôles et les calibrateurs sont fournis pré-dilués et prêts à l'emploi. Dosage 1. Tous les réactifs et les échantillons doivent être portés à température ambiante (18-25 C) avant utilisation. Sélectionner le nombre de puits nécessaire aux dosages. 2. Pipeter 100 µl d'étalons, d'échantillons de contrôles et de patients dilués dans les puits correspondants. Les contrôles et les calibrateurs sont pré-dilués et prêts à l'emploi. Recouvrir la plaque avec un film plastique et incuber pendant 60 minutes à 24 ± 2 C. 3. Après incubation, jeter le contenu de tous les puits dans l'évier. Laver les puits trois fois avec 300 µl de tampon de lavage. Sécher soigneusement sur papier absorbant avant de réaliser l'étape suivante. 4. Ajouter 100 µl de conjugué enzymatique dans tous les micropuits. Recouvrir la plaque avec un film plastique et incuber pendant 30 minutes à 24 ± 2 C. 5. La solution de substrat doit être préparée avant de réaliser l'étape suivante. Voir le paragraphe Préparation des réactifs pour plus d'information. 6. Après incubation, jeter le contenu de tous les puits dans l'évier. Laver les puits trois fois avec 300 µl de tampon de lavage. Sécher soigneusement sur papier absorbant avant de réaliser l'étape suivante. 7. Ajouter 100 µl de la solution de substrat nouvellement préparée dans tous les puits. Incuber la plaque dans l'obscurité à 24 ± 2 C pendant 10 minutes. 8. 12 chacun des micropuits. Mélanger les réactifs en tapotant la plaque. 9. Lire les absorbances à 450 nm. La microplaque doit être lue dans les 30 minutes suivant l'addition de la solution d'arrêt. Utiliser un algorithme pour courbe quadratique pour la courbe étalon et le calcul des résultats. Contrôle de la qualité Selon les bonnes pratiques de laboratoire, un contrôle positif et un contrôle négatif doivent être dosés en parallèle avec les sérums des patients. L'obtention de résultats incorrects pour les contrôles peut indiquer une erreur de manipulation, la détérioration des réactifs ou une mauvaise manipulation des échantillons. Le tableau suivant présente le pourcentage de patients, par diagnostic, ayant un résultant positif avec le test GAP IgG. Sensibilité et spécificité Des essais cliniques du test GAP IgG, IgA et IgM ont été réalisés dans quatre centres. Des patients présentant des symptômes de gastrite ont été testés. Les échantillons de biopsies de ces patients ont été analysés par culture, coloration histologique et/ou test d'uréase. Les résultats du GAP ont été comparés à ceux de l'étude histologique. USO PROPUESTO Este inmunoanálisis se usa para la detección cuantitativa de anticuerpos IgG, IgA o IgM contra el Helicobacter pylori en suero humano. Estos reactivos sólo deben emplearse para diagnóstico in vitro. RESUMEN DE LA PRUEBA Se ha encontrado Helicobacter pylori en muestras de biopsia del epitelio gástrico de pacientes que presentan gastritis activa tipo B. 1,2,3,4,5 Aunque se desconoce la vía de contagio, algunos informes han mostrado que la infección por H. pylori se asocia con gastritis crónica. 6,7 Se ha encontrado correlación entre la presencia de H. pylori y las lesiones gástricas en algunos casos de úlcera duodenal. 8 También se ha documentado la resolución completa de la gastritis después de la erradicación del microorganismo. 9 El H. pylori es un bacilo gramnegativo curvo, en forma de espiral, de 0,2 a 0,8 µm de ancho y de 0,5 a 5,0 µm de largo. Coloniza la porción profunda de la capa gelatinosa de la mucosa y la superficie apical de las células epiteliales de la mucosa gástrica. También se puede encontrar en la unión entre células epiteliales adyacentes de la misma. La presencia del H. pylori se puede detectar por métodos invasivos y no invasivos. Los invasivos comprenden el análisis de muestras de biopsia con cultivo, examen histológico y prueba de ureasa rápida. Debido a la distribución irregular del H. pylori en los tejidos, los resultados falsos negativos son frecuentes. Los procedimientos no invasivos comprenden las pruebas séricas de detección de anticuerpos contra el H. pylori y la prueba de aliento con urea marcada con isótopos radiactivos. No se conocen bien los mecanismos de producción de la enfermedad del H. pylori. Puede sobrevivir en la mucosa gástrica posiblemente durante varios años. También se encuentra a menudo en personas que se consideran sanas y que no presentan síntomas clínicos de enfermedad. PRINCIPIO DEL PROCEDIMIENTO Los estuches de GAP IgG, IgA e IgM son pruebas de ELISA cuantitativo para la detección de anticuerpos específicos contra el H. pylori en suero humano. Se inmovilizan antígenos parcialmente purificados de H. pylori en los pocillos de una placa de microtitulación y en ellos se añade suero diluido del paciente. Los anticuerpos específicos contra el H. pylori que se encuentren en el suero se fijarán a los antígenos de los pocillos. Los anticuerpos inespecíficos se retiran durante el lavado. En los pocillos se añade la IgG, IgA o IgM antihumana conjugada con la enzima, la cual se fija al complejo formado por el anticuerpo y el antígeno. El exceso del conjugado enzimático se retira en el lavado. Al agregar un sustrato de la enzima aparece un color cuya intensidad es directamente proporcional a la cantidad de anticuerpo presente en la muestra de suero. COMPONENTES DEL ESTUCHE PLA H. Sin embargo, ningún método de análisis puede dar una garantía absoluta de inocuidad. Estos componentes no se han sometido a pruebas para detectar anticuerpos contra el HTLV-1 ni el HTLV-2. Por esta razón se recomienda considerar que son potencialmente infecciosos y manejarlos con las mismas precauciones que se emplean con las muestras de pacientes. Algunos reactivos de este estuche contienen el conservante azida de sodio. La azida de sodio puede reaccionar con las tuberías de plomo y cobre y producir azidas metálicas explosivas. Cuando sea necesario desecharlos, se deben siempre aclarar con bastante agua para prevenir la acumulación de azidas. El dimetilsulfóxido y el H 2 SO 4 son irritantes de la piel y de los ojos. Se debe evitar el contacto de los reactivos con estos órganos. Usar guantes y anteojos de seguridad. Si hay contacto con la piel o los ojos, enjuagar con agua durante por lo menos 5 minutos. PREPARACIÓN Y CONSERVACIÓN DE LOS REACTIVOS Solución diluyente para muestras: Verter el contenido completo del frasco de concentrado de diluyente en un recipiente de 500 ml de capacidad. Agregar 480 ml de agua desionizada, enjuagando el frasco para retirar los cristales que se hayan precipitado. Mezclar completamente. Solución de lavado: Agregar el contenido completo del concentrado para lavado en un recipiente de 1 litro de capacidad. Agregar 980 ml de agua desionizada, enjuagando el frasco para retirar los cristales que se hayan precipitado. Mezclar completamente. Solución de trabajo del sustrato: En la hora anterior al momento de usar los reactivos, mezclar volúmenes iguales de solución A y de solución B del sustrato. Preparar solamente la cantidad necesaria. Cada tira de micropocillos necesita un mínimo de 800 µl de solución de trabajo del sustrato. Por ejemplo, para 6 tiras de micropocillos se deben mezclar 3,0 ml de solución A con 3,0 ml de solución B. Conservar en la oscuridad o cubrir con papel de aluminio. Usar antes de 1 hora. La solución de lavado, la solución diluyente para muestras y los reactivos del estuche abierto tienen una estabilidad de 60 días cuando se conservan entre 2 y 8 ºC. RECOLECCIÓN Y CONSERVACIÓN DE MUESTRAS La prueba debe realizarse en suero. Se deben tener en cuenta las precauciones acostumbradas en la venopunción. No usar anticoagulantes ni conservantes, ni emplear muestras muy hemolizadas ni muy lipémicas. El suero se puede conservar entre 2 y 8 ºC por un máximo de 10 días. Si es necesario conservarlo durante un período mayor, deberá almacenarse a 20 C. PREPARACIÓN DE LA MUESTRA Cada muestra de suero se debe diluir con la solución diluyente. Agregar 25 µl de la muestra de suero a 5 ml de solución diluyente. Mezclar completamente. Los controles y los calibradores vienen prediluidos y listos para usar. PROCEDIMIENTO 1. Todos los reactivos y muestras deben estar a temperatura ambiente (entre 18 y 25 ºC) antes de comenzar la prueba. Colocar el número deseado de tiras de micropocillos en el soporte para tiras. 2. Pipetear 100 µl de cada calibrador, control y muestra diluida de los pacientes en los pocillos respectivos. Los calibradores y los controles vienen prediluidos y listos para usar. Cubrir la placa con una película de plástico e incubarla durante 1 hora entre 22 y 26 ºC. 3. Decantar el contenido de los pocillos y lavar 3 veces con 300 µl de solución lavadora. Eliminar completamente el exceso de líquido después de cada lavado. 4. Agregar 100 µl de conjugado enzimático a cada pocillo. Cubrir la placa con una película de plástico e incubarla durante 30 minutos entre 22 y 26 ºC. 5. Preparar la solución de trabajo del sustrato. Véase PREPARACIÓN DE LOS REACTIVOS. 6. Decantar el contenido de los pocillos y lavar 3 veces con 300 µl de solución lavadora. Eliminar completamente el exceso de líquido después de cada lavado. 7. Agregar 100 µl de la solución de trabajo del sustrato a cada pocillo. Cubrir la placa con una película de plástico e incubarla durante 10 minutos en la oscuridad entre 22 y 26 ºC. 8. Inmediatamente después de la incubación, agregar a cada pocillo 50 µl de la solución para detener la reacción. Mezclar los reactivos dando golpecitos leves en la placa. 9. Medir la densidad óptica de cada pocillo a una longitud de onda de 450 nm. 16 reacción. Ajustar los datos a una curva cuadrática para obtener la curva de calibración y calcular los resultados. CONTROL DE CALIDAD De acuerdo con las prácticas correctas de laboratorio, los controles positivos y negativos se deben analizar al tiempo con las muestras de los pacientes. El hecho de no obtener los valores adecuados para los controles puede indicar manipulaciones imprecisas, manejo incorrecto de la muestra o deterioro de los reactivos. La tabla siguiente indica el porcentaje de pacientes que tuvieron un resultado positivo en la prueba GAP IgG, clasificados por diagnóstico. USO PREVISTO Test quantitativo per la determinazione in vitro di anticorpi IgG, IgA o IgM specifici per Helicobacter pylori. Questi reagenti sono per uso diagnostico in vitro. INTRODUZIONE E SCOPO DEL TEST L Helicobacter pylori è stato trovato nell epitelio gastrico da biopsie di pazienti che presentavano gastrite attiva di tipo B. 1,2,3,4,5 Sebbene il percorso dell infezione sia sconosciuto, differenti studi clinici hanno mostrato che l infezione da H. pylori è associata alle gastriti croniche. 6,7 E stata dimostrata una correlazione tra la presenza dell H. pylori e lesioni gastriche in alcuni casi di ulcere duodenali. 8 Inoltre sono state riportate complete guarigioni di gastriti dopo l eradicazione dell H. pylori. 9 L H. pylori è un batterio gram negativo, curvo, spiraliforme, largo 0,2 0,8 µm e lungo 0,5 5 µm in grado di colonizzare la profonda porzione delle mucose e la superficie apicale delle cellule epitaliali della mucosa gastica. Inoltre può essere localizzato fra le giunzioni delle cellule adiacenti. La presenza dell H. pylori può essere rilevata sia con metodi invasivi che non invasivi. Metodi invasivi includono I test culturali, I test istologici e di ricerca rapida dell ureasi da campioni bioptici. Frequenti risultati falsamente negativi sono causati dalla macchie nei tessuti dell H. pylori. Procedure non invasive includono il test sierologico per la ricerca degli anticorpi anti- H. pylori. Gli anticorpi presenti nel campione reagiscono immunologicamente con gli antigeni purificati adesi alle pareti dei pozzetti. Dopo rimozione della frazione non legata, gli anticorpi-legati vengono dosati per mezzo di una reazione con un secondo anticorpo diretto contro le IgG, IgA o le IgM umane, marcato con enzima. Viene aggiunto il substrato specifico e l intensità del colore che si genera è direttamente proporzionale alla concentrazione di anticorpi specifici per l H. pylori. ATTREZZATURE RICHIESTE Lettore per micropiastre Lavatore per micropiastre Micropipette: per dispensare 25, 50 e 100 µl Pipette: per dispensare da 5 e 10 ml Cilindri graduati rispettivamente 500 ml et 1 L Provette per la diluzione dei campioni Foglio di plastica Acqua deionizzata PRECAUZIONI Ogni singola unità di sangue da donatore utilizzata nella preparazione di questo materiale è stata analizzata con metodo, approvato dalla FDA (Food and Drug Administration), per l individuazone di anticorpi contro l HIV-1 e per l antigene di superficie dell epatite B, ed ha fornito risultati negativi (non ripetutamente reattivi). Questi componenti non sono stati testat per HCV, HIV-2, HTLV-1 or HTLV-2. Dal momento che nessun test è in grado di offrire assoluta certezza per quanto riguarda l assenza di agenti infettivi, questi reagenti devono essere tratti come se fossero effettivamento in grado di trasmettere agenti infettivi. Alcuni reagenti in dotazione al kit contengono sodio azide come conservante. Che può reagire con il rame ed il piombo delle tubature idrauliche, dando luogo alla formazione di azidi metalliche esplosive. Durante l eliminazione di questi reagenti far scorrere grandi volumi d acqua all interno delle tubature per evitare l accumulo di questi composti. Il DMSO é un irritante per occhi e pelle. Evitare il contatto con la pelle e con gli occhi. Indossare guanti e occhiali di protezione. Se pelle ed occhi venissero a contatto con il reattivo, lavare abbondantemente la parte interessata con acqua per almeno 5 minuti. PREPARAZIONE DEI REAGENTI Tampone per la Diluizione dei Compioni: Il tampone concentrato per la diluizione dei campioni può presentare dei cristalli. Aggiungere l intero contenuto del flacone a 480 ml di acqua deionizzata (trasferire anche i cristalli eventualmente presenti.) Mescolare bene. Stabilità dopo diluizione: scandenza del kit a 2 8 C. Tampone di Lavaggio: Il tampone di lavaggio concentrato può presentare dei cristalli. Aggiungere l intero contenuto del flacone a 980 ml di acqua deionizzata (trasferire anche i cristalli eventualmente presenti.) Mescolare bene. Stabilità dopo diluizione: scandenza del kit a 2-8 C. Soluzione di Substrato: Mescolare le soluzioni di substrato A e B in parti uguali in una provetta (13 x 100 mm). Per ottenere una quantità sufficiente per 48 pozzetti, pipettare 3 ml di substrato A in 3 ml di substrato B. Mescolare bene. Utillizzare la soluzione entro un ora dalla preparazione. La soluzione di lavaggio, il diluente per i campioni ed i reagenti del kit aperto sono stabili per 60 giorni se conservati a 2-8 C. PRELIEVO DEI CAMPIONI Raccogliere circa 5 ml di sangue venoso in una provetta per siero. Separare il siero per centrifugazione. I campioni di siero possono essere conservati a 2 8 C per 10 giorni. Per periodi di conservazione più lunghi, congelare a 20 C. PREPARAZIONE DEI CAMPIONI Pipettare in una provetta 25 µl di campione in 5 ml di tampone per la diluizione dei campioni. Mescolare bene per inversione o su vortex. Non è necessario prediluire i sieri di controllo prima dell uso, i sieri di controllo vanno direttamente dispensati nei pozzetti. FASI DEL TEST 1. Disporre un numero sufficiente di strisce nell apposito sostegno. 2. Dispensare 100 µl di calibratori, controlli e campioni diluiti nei pozzette appropriati. Non è necessario prediluire i sieri di controllo prima dell uso. I sieri di controllo vanno direttamente dispensati nei pozzetti. Coprire la piastra con un foglio di plastica e incubare per 60 minuti a 24 ± 2 C. 3. Scartare il contenuto dei pozzetti. Lavare ogni pozzetto 3 volte con 300 µl di soluzione di lavaggio. Capovolgere la piastra su carta assorbente. 4. Aggiungere 100 µl di coniugato enzimatico in ogni pozzetto. Coprire la piastra con un foglio di plastica e incubare per 30 minuti a 24 ± 2 C. 5. La soluzione di substrato deve essere preparata in questa fase. 6. Scartare il contenuto dei pozzetti. Lavare ogni pozzetto 3 volte con 300 µl di soluzione di lavaggio. 20 7. Dispensare in tutti i pozzetti 100 µl della soluzione di substrato appena preparata. Coprire la piastra con un foglio di plastica e incubrare per 10 minuti, al buio, a 24 ± 2 C. 8. Dopo esattemente 10 minuti, aggiungere velocemente 50 µl di soluzione bloccante ad ogni pozzetto. Miscelare i reagenti battendo leggermente la piestra. 9. Leggere l assorbanza a 450 nm. La micropiastra deve essere letta entro 30 minuti dal-l aggiunta della soluzione bloccante. Utilizzare una funzione di calcolo adequate (es. quadratica) per le costruzione della curva e per il calcolo dei resultati. CONTROLLO DI QUALITA In conformità con la buona pratica di laboratorio, i controlli positivo e negativo devono essere dosati in parallelo ai campioni. Risultati inaccurati dei controlli possono essere dovuti ad imprecise dispensazioni, inadeguata manipolazione dei campioni o deterioramento dei reagenti.
