5 ENGLISH BLOM-SINGER INDWELLING VOICE PROSTHESES for Primary, Secondary, Replacement TEP Procedures Introduction The Blom-Singer voice prosthesis has helped thousands of laryngectomees worldwide to speak again. The original tracheoesophageal voice prosthesis was developed by Dr. Eric Blom and Dr. Mark Singer in Today, their method of TEP, tracheoesophageal puncture, is still recognized throughout the world as the leading standard for voice restoration following total laryngectomy. The Blom-Singer Indwelling voice prosthesis is an indwellingstyle, clinician-placed voice prosthesis. The product is designed for laryngectomees who are unable or unwilling to routinely remove, clean, and reinsert a patient-changeable voice prosthesis. The voice prosthesis and accessory items should only be used by individuals who have received proper training in the effective management and care of Blom-Singer voice prostheses. Management includes, but is not limited to, handling of the device, dislodgement of the device, and cleaning of the device. This Blom-Singer medical device may be used only once by one patient. It may not be reused. See Warnings, Precautions and Complications. Do not use the device if it becomes contaminated; discard and replace it with a newly packaged prosthesis. INDICATIONS The Blom-Singer Indwelling prosthesis is indicated for tracheoesophageal voice restoration following total laryngectomy, when placement, or replacement, of an Indwelling prosthesis is performed by a qualified, trained medical professional. CONTRAINDICATIONS The Blom-Singer Indwelling prosthesis is a medical product that should be used only by a qualified, medical professional with experience and training in its use and care F 5

6 HOW SUPPLIED The Blom-Singer Indwelling voice prosthesis is supplied in nonsterile or sterile offerings as a complete voice restoration system. Nonsterile Products: Included in each nonsterile offering is a voice prosthesis, secondary placement items, and voice prosthesis cleaning system for the patient. Do not sterilize or place in an autoclave as damage to the prosthesis may occur. Sterile Products: Conveniently included in each sterile offering is a sterile voice prosthesis, nonsterile secondary placement items, and nonsterile voice prosthesis cleaning system for the patient. PRODUCT DESCRIPTION Please refer to the diagrams located at the back of this instruction manual. Four distinct styles and the largest selection of sizes are available to allow the clinician to choose a voice prosthesis best suited for the needs of the patient. Nonsterile product is not intended for primary placement procedures. The Blom-Singer voice prosthesis is made of silicone, a biocompatible material used in many medical products. The gel cap used for insertion of the prosthesis is made of a vegetable base gelatin. Individual patient reactions to these materials may occur. The Blom-Singer voice prosthesis, (diagram 1), consists of a oneway silicone flap valve (diagram 1a), an esophageal retention flange (diagram 1b), a body that holds the valve assembly (diagram 1c), a tracheal retention flange (diagram 1d), and a neck strap (diagram 1e) F

Refer to the Instructions For Use packaged with the related accessory for complete instructions on how to use these devices in accordance with the manufacturer s intended use. Voice Prosthesis Cleaning System The Voice Prosthesis Cleaning System contains the necessary cleaning devices for careful cleaning of the voice prosthesis while in the TE puncture. Dilation/Sizing System The Blom-Singer Dilation/Sizing System is designed to facilitate dilating and measuring of the TE puncture tract in preparation for insertion of a Blom-Singer voice prosthesis. Included in the system is a puncture dilator that also prevents closure of the TE puncture and aspiration of fluids when the voice prosthesis is removed. A Dilation-Sizing device should not be placed in a small tracheostomal airway if its presence obstructs breathing. These devices are not valved voice prostheses and do not function like one F 7

8 Valved and Plug Inserts The valved and plug inserts are designed to temporarily stop the voice prosthesis from leaking until professional, medical attention can be obtained. The valved insert allows the user to continue voicing but the plug insert does not allow voicing. WARNINGS AND PRECAUTIONS The Blom-Singer Indwelling prosthesis is a medically prescribed device. Initial sizing and instructions for use must be provided by a qualified medical professional trained in the use of this particular device. InHealth relies on the qualified, trained medical professional to instruct patients on the use and care of this device and to provide patients with the Blom-Singer instructions for use supplied with the product. If the prosthesis is not functioning properly, the patient should have it evaluated by a qualified, trained medical professional as soon as possible. If there are tears, cracks, or structural damage to the prosthesis, cleaning device or accessories, discontinue use. Handle the device and the gel cap loading tool carefully to avoid damaging the prosthesis. Use only the specially sized gel caps provided by InHealth Technologies. The Gel Cap Insertion System is designed for use with Blom-Singer prostheses only. Do not use petroleum-based products such as Vaseline to lubricate the prosthesis as these products can damage silicone. Changes in the anatomy or medical status of the patient may lead to improper fitting and/or function of the device. It is recommended that the prosthesis and TEP be evaluated by the clinician at regular intervals. Voice Production To prevent post-operative complications, the patient should not begin speaking with the voice prosthesis until the clinician has indicated that it is safe to do so F

9 Blom-Singer Indwelling Voice Prostheses The voice prosthesis must be kept clean of mucus and organic matter in order for it to function properly to allow the patient to speak with a good quality of voice. In some users the inability to relax the muscles of the throat may account for their inability to speak fluently and, to speak with minimal effort. This problem requires professional assessment. Patients requiring postoperative radiation may have transient interruption of voice in the third or fourth week of treatment. The prosthesis can be left in place or a catheter can be inserted as a stent until radiation treatments are completed. Voice Prosthesis Dislodgement Care must be exercised during prosthesis insertion or removal to avoid injury to the TEP or accidental displacement of the prosthesis which could result in aspiration of the prosthesis into the trachea. Should aspiration occur, the patient should attempt to cough the prosthesis out of the trachea. Further medical attention may be necessary if coughing the prosthesis out is unsuccessful. Confirm gel cap dissolution and deployment of the esophageal retention flange per procedures provided to ensure device is securely retained in TEP. If the prosthesis is accidentally dislodged from the puncture, the patient should immediately place a Blom-Singer puncture dilator or suitable device, of the appropriate diameter, in the puncture to keep it from closing and leaking fluids. The prosthesis should be reinserted within 24 hours. Foreign objects should never be inserted into the prosthesis by the patient. The cleaning device is designed for only cleaning dried mucus and organic matter from the voice prosthesis. Inserting objects other than the supplied cleaning device may cause dislodgment of the voice prosthesis or its components and may cause these objects to be swallowed or inhaled. Care should be exercised when inserting or removing cleaning devices to avoid accidental displacement of the prosthesis which could result in aspiration of the prosthesis. In the unlikely event F 9

10 that this should occur, the patient should attempt to cough the prosthesis out of the trachea and immediately seek medical attention if unsuccessful. Voice Prosthesis Leakage When the flap valve fails to close completely a few drops of fluid may pass back through the valve from the esophagus to the trachea, which may cause coughing. Recurrent leakage of the voice prosthesis should be evaluated. Gentle handling and pressure should always be used when cleaning the voice prosthesis to avoid deterioration of the device and damaging internal components which could cause leakage. Candida species Most users have a yeast (Candida albicans) in their mouth, throat, or esophagus; this is a commonly occurring condition. However, if the prosthesis is not properly cleaned in users who have this condition, the yeast may grow on the valve of the voice prosthesis. The yeast is identified by discolored spots on the prosthesis. Yeast deposits can cause valve deformation and failure, i.e., liquid leakage through the prosthesis and an increase in the pressure necessary to voice. Replacement of the prosthesis may be required more frequently. Some patients can be treated successfully for yeast with oral medication or mouth rinses. Device Removal The attached neck strap on an Indwelling prosthesis should only be removed after the esophageal retention flange position within the esophagus has been verified. Never attempt to insert or reinsert an Indwelling prosthesis that has the neck strap removed. Never remove one Indwelling prosthesis and insert another prosthesis without first dilating the TE puncture and remeasuring the tract to confirm correct prosthesis length. The patient should never attempt to remove the device or allow anyone other than a qualified, trained medical professional to remove the Indwelling prosthesis. The Indwelling prosthesis is not a permanent device and requires replacement periodically F

11 Blom-Singer Indwelling Voice Prostheses The Indwelling prosthesis may be left in place in the TE puncture until it ceases to function correctly, that is, until it leaks or is not providing adequate voice for speech. Many clinicians recommend routine follow-up evaluations not longer than 6 months. Removal of the Indwelling prosthesis should only be done by grasping the outer tracheal flange of the device securely with a locking hemostat. Pull gently and firmly until the prosthesis is fully withdrawn. Caution: Federal (USA) law restricts this device to sale by or on the order of a physician or a licensed speech pathologist. COMPLICATIONS Although rare, the following complications have been identified to occur with silicone prostheses of the Blom-Singer type. They include: stoma contamination or sepsis, which may require removal of the prosthesis and/or appropriate antibiotics; accidental aspiration of the prosthesis into the airway, which may require removal by a physician; occasional extrusion of the prosthesis, requiring replacement after dilation of the TE puncture and additional supervision of the stoma care regimen; puncture dilation resulting in leakage of fluids around the prosthesis; inflammatory reaction around the puncture site and formation of granulation tissue; dislodgment of the prosthesis and subsequent closure of the TE puncture; intractable leakage around the prosthesis, requiring surgical revision or closure of the puncture; dysphagia, or difficulty swallowing; tearing or other damage to the prosthesis from improper use; yeast deposits causing leakage through the prosthesis or valve incompetence; accidental ingestion of the prosthesis into the esophagus and/ or GI tract. INSTRUCTIONS FOR USE The following instructions are provided for primary and secondary/ replacement procedures. It is recommended that the video demonstration, Tracheoesophageal Voice Restoration After Total Laryngectomy, R.C. Hamaker M.D., F.A.C.S., Eric D. Blom, Ph.D., available from InHealth Technologies, be viewed before performing these procedures. Always measure the TE puncture tract before inserting a voice prosthesis to confirm correct voice prosthesis length F 11

12 Primary Voice Prosthesis Placement The following procedure is provided by the late Ronald C. Hamaker, M.D., F.A.C.S., as instructions for primary placement of the voice prosthesis at the time of the tracheoesophageal puncture during laryngectomy. Note: The use of a Blom-Singer Gel Cap Insertion System is not required for primary voice prosthesis placement. Prior to use of the voice prosthesis, check the valve mechanism to be sure it is intact and working properly. The flap valve, (diagram 8a), should close flat against the seating surface inside the prosthesis. Please refer to the diagrams located at the back of this instruction manual. 1. Grasp the neck strap on the sterile indwelling voice prosthesis with a right angle hemostat (diagram 8b). 2. Insert the tip of the hemostat with neck strap into the esophagus. Push the tip against the anterior wall 1.5 cm. from the cut edge of the trachea. Cut down horizontally on the tip through the trachea (diagram 9). 3. The hemostat tip with the neck strap is gently pushed through the tracheoesophageal wall into the trachea (diagram 10). 4. The neck strap is grasped with a second hemostat and the voice prosthesis is pulled through the puncture site (diagram 11). 5. The voice prosthesis is positioned in the tracheoesophageal puncture (diagram 12). 6. The neck strap on the voice prosthesis is detached and the wound is closed. Voicing is deferred until two weeks postoperatively to avoid the possibility of an air pressure induced fistula (diagram 13). Secondary Voice Prosthesis Placement The following procedure is provided by Eric D. Blom, Ph.D. as instructions for secondary voice prosthesis placement, or replacement of the voice prosthesis, using the Blom-Singer Gel Cap Insertion System. Please refer to the diagrams located at the back of this instruction manual F

13 Blom-Singer Indwelling Voice Prostheses Note: All secondary procedures must only be done using a bright light to illuminate the tracheostoma and puncture. Insertion or removal of the prosthesis in the TE puncture should only be done with a bright light focused directly on the stoma. Use of gloves and protective eye wear is recommended for the following procedure. The Indwelling prosthesis and its accessories should always be thoroughly clean and dry to help avoid introducing contaminants into the tracheostoma, TE puncture, or esophagus. Have the following Blom-Singer items prepared for aseptic insertion of the prosthesis: Blom-Singer Indwelling Prosthesis Blom-Singer Gel Cap Insertion System Blom-Singer Dilator-Sizer Blom-Singer Voice Prosthesis Sizer (or Dilator-Sizer) Blom-Singer Puncture Dilator (or Dilator-Sizer) Blom-Singer Prosthesis Inserter Dilate and Measure Please refer to the Blom-Singer Dilation/Sizing System Instructions For Use for complete product details. Optional: The Blom-Singer Sizer or Puncture Dilator may be used as an alternate procedure. Refer to the instructions for use supplied with the product. 1. Dilate the TEP with a Blom-Singer Dilator-Sizer of the appropriate diameter. Gently insert the Dilator-Sizer into the puncture completely to the neck strap (diagram 2) and leave in place for five minutes. 2. Carefully withdraw the Dilator-Sizer until very gentle resistance is detected indicating that the esophageal wings are seated against the anterior wall of the esophagus (diagram 2a). Note: Pulling too firmly on the Dilator-Sizer may squeeze or compress the TE tract resulting in an erroneous measurement that indicates it is shorter than it actually is. 3. Note the marker that is closest to the entrance of the TE puncture. Read the size number on the Dilator-Sizer and select F 13

14 a Blom-Singer Indwelling prosthesis of corresponding size. If the measurement is between two markers, select the longer one. 4. Fully insert the Dilator-Sizer and fasten the neck strap with adhesive tape on to the patient s neck. Leave the Dilator-Sizer in the puncture to dilate 5 minutes, or longer if necessary, in order to allow for easy insertion of the selected Indwelling prosthesis. Prior to insertion of the Blom-Singer Indwelling prosthesis, remove the tape from the neck strap, grasp the dilator securely, and carefully withdraw it from the puncture. Immediately insert the Indwelling prosthesis (see below). Prosthesis Preparation Note: Prior to use of the Indwelling prosthesis, check the valve mechanism to be sure it is intact and working properly. The flap valve, (diagram 1a), should close flat against the seating surface inside the prosthesis. Prepare the prosthesis for easy, atraumatic insertion by utilizing the Blom-Singer Gel Cap Insertion System. Use only the specially sized gel caps provided with this product. The Gel Cap Insertion System is designed for use with Blom- Singer Prostheses only; do not use with any other type devices. The following steps describe the hand folding method of gel cap insertion. 1. Remove the prosthesis from the package. Always handle the prosthesis with clean, gloved hands. Be certain that your hands and the prosthesis are completely dry before loading the gel cap. Failure to do so may cause premature dissolving of the gel cap. 2. Remove the Gel Cap Insertion System items from the package: gel cap, flange introducer, and inserter tool. 3. Fold and load the prosthesis into the unprinted gel cap (discard the printed end) according to the following steps. 4. Fold the esophageal retention flange tightly in half toward the center of the device (diagram 4a). 5. Fold the two outer edges of the esophageal retention flange tightly against the folded flange (diagram 4b) and hold the prosthesis in this folded position F

15 Blom-Singer Indwelling Voice Prostheses 6. Insert the folded portion of the esophageal retention flange into the gel cap (diagram 4c). Gently tuck and push the folded prosthesis as far as possible into the gel cap. 7. Use the flange introducer to gently tuck the remaining portion of the folded esophageal retention flange completely into the gel cap (diagram 4d). Do not use any sharp or serrated instruments to avoid damaging the prosthesis. 8. The esophageal retention flange should be folded in a forward position and completely enclosed inside the gel cap (diagram 4e). Prosthesis Insertion Warning: Caution should be exercised to avoid accidental displacement of the prosthesis which could result in aspiration of the prosthesis into the trachea. Should aspiration occur, the patient should attempt to cough the prosthesis out of the trachea. Further medical attention may be necessary if coughing the prosthesis out is unsuccessful. 1. Place the prosthesis on the inserter and lock the neck strap onto the safety peg (diagram 3). 2. Caution the patient not to swallow as you remove the previously inserted Dilator-Sizer to avoid aspiration of saliva into the trachea prior to inserting the prosthesis. 3. Apply a light coating of water-soluble lubricant (oil-free) to the tip of the gel-capped end of the prosthesis and immediately insert the prosthesis completely into the TE puncture by aligning the tip of the prosthesis partially in the puncture with the neck strap oriented upwards (diagram 5). Note: Do not use petroleumbased products such as Vaseline to lubricate the prosthesis as these products can damage silicone. 4. Hold the prosthesis in this position of complete insertion for at least three minutes. This allows ample time for the gel cap to dissolve and release the esophageal retention flange (diagram 1b) within the esophagus. Patients should be instructed to swallow their saliva or warm water to facilitate dissolving the gel cap. Flange Deployment of Dual Valve Prosthesis To accommodate flange deployment of the dual valve, the prosthesis inserter is designed so that it does not fully insert F 15

16 into the prosthesis (diagram 7). Within three minutes the inserter should visibly move forward into the prosthesis; the clinician should see this forward movement indicating that the gel cap has dissolved and that the esophageal retention flange has deployed. Caution: If the prosthesis does not insert easily on the first attempt, do not continue to try to insert it. Instead, insert a clean puncture dilator, of the appropriate diameter, for a few minutes to dilate the TE tract, and then try again. 5. Detach the prosthesis neck strap (diagram 1e) from the safety peg on the inserter. Place a finger against the neck strap and carefully withdraw the inserter from the prosthesis with a twisting motion. Warning: Caution should be exercised when removing the inserter to avoid accidental displacement of the prosthesis which could result in aspiration of the prosthesis. Secure the prosthesis with adhesive tape across the neck strap. It is recommended to orient the prosthesis with the neck strap up. 6. Have the patient take a breath, occlude the stoma, and produce voice. Be certain there is no leakage through the prosthesis by having the patient drink water. Confirmation of Esophageal Retention Flange Deployment The following procedure for confirming deployment of the esophageal retention flange is recommended by Eric D. Blom, Ph.D. After insertion of the prosthesis in the puncture tract and completion of the 3-minute waiting period for gel cap dissolution and esophageal retention flange deployment, repeatedly rotate the prosthesis on the inserter while it is in the puncture. If the esophageal retention flange has deployed and has properly seated against the mucosa of the anterior esophageal wall, the prosthesis on the inserter will rotate easily in the puncture tract. If the gel cap has dissolved and incorrectly deployed the esophageal retention flange inside the puncture tract rather than inside the lumen of the esophagus, considerable resistance to prosthesis rotation will be felt. A second method for confirming gel cap dissolution and esophageal retention flange deployment is to assess the capacity F

17 Blom-Singer Indwelling Voice Prostheses to produce tracheoesophageal voice. If the voice prosthesis is correctly positioned and the esophageal retention flange deployed, airflow diverted from the trachea by digital occlusion of the tracheostoma will open the flap valve and pass up the esophagus to initiate fluent voice. Proper Valve Function Confirmation of Dual Valve Prosthesis Use a small wax curette or equivalent gauge instrument to gently open the tracheal valve while the patient swallows their saliva or water. The clinician should see the properly positioned esophageal valve at the other end and no leakage. If the proper deployment and positioning of the esophageal retention flange cannot be confirmed by one or both of the methods described above, the clinician may need to determine gel cap dissolution and esophageal retention flange deployment by manually opening, with gentle contact, the one-way flap valve in the esophageal end of the voice prosthesis. Note: The following method is not applicable to the Dual Valve prosthesis. Direct a bright light into the prosthesis to visualize the flap valve at the proximal end. Slowly and carefully advance the shaft end (non-cotton tip) of a long cotton swab into the voice prosthesis until you observe that gentle contact opens the flap valve. A flap valve that does not open with gentle contact indicates that the undeployed retention flange is preventing valve opening. If the assessment methods described above fail to provide satisfactory verification of correct esophageal retention flange deployment, remove the prosthesis, dilate and remeasure the puncture tract, and repeat the prosthesis insertion and confirmation process. Neck Strap Detachment The Indwelling prosthesis is designed to include the optional detachment of the neck strap following confirmation that the prosthesis esophageal retention flange is securely positioned within the esophageal lumen against the anterior wall of the esophagus. Removal of the neck strap is an optional procedure F 17

18 Once deployment of the esophageal retention flange has been confirmed, detach the neck strap from the tracheal retention flange at the area of reduced neck strap width, where it meets the tracheal flange, by cutting carefully with scissors (diagram 6). Note: if you are uncertain that the esophageal retention flange has properly unfolded, and the prosthesis is in place inside the esophageal lumen, after using the confirmation steps described above, do not detach the neck strap from the prosthesis. Securely tape the neck strap to the neck and observe the patient for a few days for retention ability. The neck strap should not be detached from the Indwelling prosthesis if: use of a laryngectomy tube may potentially dislodge the prosthesis; a patient has previously been wearing a prosthesis that is larger in diameter than the Indwelling prosthesis currently being inserted; the diameter of the TE puncture is dilated significantly larger than the prosthesis diameter; or, the health of the stoma tissue or physical condition of the patient is in question. Note: It is important not to detach the neck strap from the prosthesis in the instances cited above. Tape the neck strap to the peristomal skin and observe security of prosthesis retention for an extended period of time. Removal of the Prosthesis The Indwelling prosthesis is not a permanent device and requires replacement periodically. The Indwelling prosthesis may be left in place in the TE puncture until it ceases to function correctly, that is, until it leaks or is not providing adequate voice for speech. Many clinicians recommend routine follow-up evaluations not longer than 6 months. 19 Blom-Singer Indwelling Voice Prostheses Insert a Dilator-Sizer of the appropriate diameter, and tape it in position for five minutes prior to inserting a new Indwelling prosthesis. Warning: The patient should never attempt to remove the device or allow anyone other than a qualified, trained medical professional to remove the Indwelling prosthesis. Never remove one Indwelling prosthesis and insert another prosthesis without first dilating the TE puncture and remeasuring the tract to confirm correct prosthesis length. VOICE PROSTHESIS CLEANING AND CARE INSTRUCTIONS The following instructions should be made clear to the patient as part of the routine care and cleaning of the Indwelling voice prosthesis. The following information is a brief overview of the proper care and maintenance of the voice prosthesis. Please refer to the Voice Prosthesis Cleaning System instructions for use supplied with this product for complete instructions on how to use the cleaning devices. The voice prosthesis will continue to function and may last longer if it is cleaned daily with the supplied cleaning device per the recommended instructions. The cleaning device provides a means for careful cleaning of the voice prosthesis while in the patient s TE puncture. The cleaning device is designed for use only in cleaning organic matter from the voice prosthesis. Caution: Use only supplied cleaning device. Do not insert foreign objects into voice prosthesis. Inserting objects other than the supplied cleaning device may cause dislodgment of the voice prosthesis or its components. The cleaning device and user s hands should always be thoroughly clean to prevent contaminants from entering into the tracheostoma, puncture, or esophagus. Cleaning of the indwelling voice prosthesis should only be done in front of a mirror with a bright light focused directly on the stoma so that the open end of the prosthesis is clearly visible F 19

20 Never use facial or toilet tissue to dry a cleaning device as particles of lint or fabric may be deposited onto the device that can be inhaled into the airway upon reinsertion. Do not use solvents or petroleum-based products for cleaning or lubricating the device. These materials may damage silicone or cause the device to not work properly. SPECIAL ORDER PRODUCTS If this instruction manual accompanies a Special Order Product, there may be differences in the physical characteristics between the product and the product descriptions described in this instruction manual. Η πρωτότυπη τραχειοοισοφαγική φωνητική πρόθεση σχεδιάστηκε από τους Δρ. Eric Blom και Δρ. ΑΝΤΕΝΔΕΙΞΕΙΣ Η Μόνιμη φωνητική πρόθεση Blom-Singer είναι ένα ιατρικό προϊόν, το οποίο πρέπει να χρησιμοποιείται μόνο από έναν εκπαιδευμένο επαγγελματία υγείας με εμπειρία και τα κατάλληλα προσόντα σχετικά με τη χρήση και τη φροντίδα του. ΤΡΟΠΟΣ ΔΙΑΘΕΣΗΣ Η Μόνιμη φωνητική πρόθεση Blom-Singer διατίθεται σε μη αποστειρωμένες ή αποστειρωμένες συσκευασίες ως ολοκληρωμένο σύστημα αποκατάστασης της φωνής Μη αποστειρωμένα προϊόντα: Σε κάθε μη αποστειρωμένη συσκευασία περιλαμβάνεται μία φωνητική πρόθεση, αντικείμενα δευτερεύουσας τοποθέτησης και σύστημα καθαρισμού φωνητικής πρόθεσης για τον ασθενή. Μην αποστειρώσετε ή τοποθετήσετε την πρόθεση σε έναν κλίβανο υγρής αποστείρωσης καθώς ενδέχεται να προκληθεί ζημιά. Αποστειρωμένα προϊόντα: Σε κάθε αποστειρωμένη βολική συσκευασία περιλαμβάνεται μία αποστειρωμένη φωνητική πρόθεση, μη αποστειρωμένα αντικείμενα δευτερεύουσας τοποθέτησης και μη αποστειρωμένο σύστημα καθαρισμού φωνητικής πρόθεσης για τον ασθενή. ΠΕΡΙΓΡΑΦΗ ΠΡΟΪΟΝΤΟΣ Παρακαλούμε ανατρέξτε στα διαγράμματα που βρίσκονται στο τέλος του παρόντος εγχειριδίου οδηγιών. Σύστημα καθαρισμού φωνητικής πρόθεσης Το Σύστημα καθαρισμού φωνητικής πρόθεσης περιλαμβάνει τις απαραίτητες συσκευές καθαρισμού για τον προσεκτικό καθαρισμό της φωνητικής πρόθεσης ενώ βρίσκεται στην τραχειοοισοφαγική (TE) παρακέντηση. Σύστημα διαστολής/μέτρησης μεγέθους Tο Σύστημα διαστολής/μέτρησης μεγέθους Blom-Singer είναι σχεδιασμένο για να διευκολύνει τη διαστολή και τη μέτρηση του πόρου της TE παρακέντησης κατά την προετοιμασία για την εισαγωγή μίας φωνητικής πρόθεσης Blom-Singer. ΠΡΟΕΙΔΟΠΟΙΗΣΕΙΣ ΚΑΙ ΠΡΟΦΥΛΑΞΕΙΣ Η Μόνιμη φωνητική πρόθεση Blom-Singer είναι μία ιατρικά συνταγογραφούμενη συσκευή. Η αρχική ρύθμιση του μεγέθους και οι οδηγίες χρήσης πρέπει να δοθούν από έναν επαγγελματία υγείας με τα κατάλληλα προσόντα, ο οποίος είναι εκπαιδευμένος ως προς τη χρήση της συγκεκριμένης συσκευής. 26 ως προς τη χρήση και τη φροντίδα αυτής της συσκευής και τη διάθεση των οδηγιών χρήσης της μόνιμης φωνητικής πρόθεσης Blom-Singer στους ασθενείς, οι οποίες διατίθενται με το προϊόν. Εάν η πρόθεση δεν λειτουργεί σωστά, ο ασθενής πρέπει να ζητήσει να γίνει αξιολόγησή της από έναν καταρτισμένο επαγγελματία υγείας με τα κατάλληλα προσόντα το συντομότερο δυνατόν. Εάν υπάρχουν σχισίματα, ρωγμές ή δομικές ζημιές στην πρόθεση, τις συσκευές καθαρισμού ή τα εξαρτήματα, σταματήστε τη χρήση τους. Χειριστείτε προσεκτικά τη συσκευή και το εργαλείο γεμίσματος της κάψουλας γέλης ώστε να αποφύγετε την πρόκληση ζημιάς στην πρόθεση. Χρησιμοποιήστε μόνο τις ειδικών διαστάσεων κάψουλες γέλης που διατίθενται από την InHealth Technologies. Το Σύστημα εισαγωγής για τις κάψουλες γέλης είναι σχεδιασμένο για χρήση μόνο με τις προθέσεις Blom Singer. Μη χρησιμοποιείτε προϊόντα με βάση το πετρέλαιο όπως η βαζελίνη (Vaseline ) για τη λίπανση της πρόθεσης, καθώς αυτά τα προϊόντα μπορεί να καταστρέψουν τη σιλικόνη. Αλλαγές ως προς την ανατομία ή την ιατρική κατάσταση του ασθενή ενδέχεται να οδηγήσουν σε ακατάλληλη προσαρμογή και/ή λειτουργία της συσκευής. Συνίσταται η τακτική αξιολόγηση της φωνητικής πρόθεσης και της TEP από έναν κλινικό ιατρό. Παραγωγή φωνής Για την πρόληψη των μετεγχειρητικών επιπλοκών, ο ασθενής δεν πρέπει να αρχίσει να μιλάει με τη φωνητική πρόθεση μέχρι την ενημέρωση από τον ιατρό ότι είναι ασφαλές να το κάνει. Η φωνητική πρόθεση πρέπει να διατηρείται καθαρή από βλέννη και οργανικές ύλες ώστε να λειτουργεί σωστά και να βοηθάει τον ασθενή να μιλάει με καλή ποιότητα ομιλίας. Σε μερικούς χρήστες, η αδυναμία χαλάρωσης των μυών του λάρυγγα ενδέχεται να ευθύνεται για την αδυναμία τους να μιλήσουν με ευχέρεια και χωρίς ιδιαίτερη προσπάθεια. Απαιτείται επαγγελματική αξιολόγηση για αυτό το πρόβλημα F

27 Μονιμη Φωνητικη Προθεση Blom-Singer Οι ασθενείς που χρειάζονται μετεγχειρητική ακτινοβολία ενδέχεται να παρουσιάσουν παροδική διακοπή της φωνής κατά την τρίτη ή τέταρτη εβδομάδα της θεραπείας. Η πρόθεση μπορεί να παραμείνει στη θέση της ή να εισαχθεί ένας καθετήρας ως στεντ μέχρι την ολοκλήρωση των ακτινολογικών θεραπειών. Μετατόπιση φωνητικής πρόθεσης Πρέπει να δίνεται προσοχή κατά την εισαγωγή ή αφαίρεση της πρόθεσης για να αποφύγετε τον τραυματισμό της TEP ή την τυχαία μετατόπιση της πρόθεσης, η οποία θα μπορούσε να επιφέρει την εισρόφηση της φωνητικής πρόθεσης στην τραχεία. Σε περίπτωση που προκληθεί εισρόφηση, ο ασθενής πρέπει να προσπαθήσει να βήξει ώστε να βγει η πρόθεση από την τραχεία. Ενδέχεται να απαιτείται επιπλέον ιατρική παρακολούθηση εάν δεν είναι επιτυχής η αφαίρεση της πρόθεσης μέσω του βηξίματος. Βεβαιωθείτε ότι η διάλυση της κάψουλας γέλης και η τοποθέτηση του οισοφαγικού πτερυγίου συγκράτησης γίνονται σύμφωνα με τις αντίστοιχες διαδικασίες, ώστε να διασφαλιστεί η ασφαλής διατήρηση της συσκευής στην TEP. Εάν η πρόθεση μετατοπιστεί τυχαία από την παρακέντηση, ο ασθενής πρέπει να τοποθετήσει αμέσως τον διαστολέα παρακέντησης Blom-Singer ή κάποια άλλη κατάλληλη συσκευή με σωστή διάμετρο στην παρακέντηση, για να αποτρέψει το κλείσιμο και τη διαρροή υγρών από αυτή. Η πρόθεση πρέπει να εισαχθεί ξανά εντός 24 ωρών. Δεν πρέπει να εισάγονται ποτέ ξένα αντικείμενα στην πρόθεση από τον ασθενή. Η συσκευή καθαρισμού είναι σχεδιασμένη μόνο για τον καθαρισμό αποξηραμένης βλέννης και οργανικής ύλης από τη φωνητική πρόθεση. Η εισαγωγή άλλων αντικειμένων εκτός της παρεχόμενης συσκευής καθαρισμού μπορεί να προκαλέσει μετατόπιση της φωνητικής πρόθεσης ή τμημάτων της και ενδέχεται να προκαλέσει την κατάποση ή την εισπνοή αυτών των αντικειμένων. Πρέπει να είστε προσεκτικοί όταν εισάγετε ή αφαιρείτε συσκευές καθαρισμού για να αποφύγετε την τυχαία μετατόπιση της πρόθεσης, η οποία θα μπορούσε να επιφέρει την εισρόφηση F 27

28 της πρόθεσης. Στην απίθανη περίπτωση που αυτό συμβεί, ο ασθενής πρέπει να προσπαθήσει να βήξει ώστε να βγει η φωνητική πρόθεση από την τραχεία και, εάν δεν τα καταφέρει, να αναζητήσει αμέσως ιατρική βοήθεια. Διαρροή φωνητικής πρόθεσης Όταν η βαλβίδα με καπάκι δεν μπορεί να κλείσει εντελώς, μπορεί να τη διαπεράσουν μερικές σταγόνες υγρού από τον οισοφάγο μέχρι την τραχεία, κάτι που ενδέχεται να προκαλέσει βήχα. Πρέπει να γίνει αξιολόγηση της επαναλαμβανόμενης διαρροής της φωνητικής πρόθεσης. Να γίνονται πάντα ήπιοι χειρισμοί και να ασκείται μικρή πίεση κατά τον καθαρισμό της φωνητικής πρόθεσης, ώστε να αποφύγετε την αλλοίωση της συσκευής και την καταστροφή των εσωτερικών τμημάτων, τα οποία θα μπορούσαν να προκαλέσουν διαρροή. Είδη Candida Oι περισσότεροι ασθενείς έχουν έναν ζυμομύκητα (Candida albicans) στη στομία, τον λάρυγγα ή τον οισοφάγο τους. Αυτή η κατάσταση παρουσιάζεται συχνά. Ωστόσο, εάν η πρόθεση δεν καθαρίζεται σωστά σε χρήστες που εμφανίζουν αυτήν την κατάσταση, ο ζυμομύκητας μπορεί να αναπτυχθεί στη βαλβίδα της φωνητικής πρόθεσης. Ο ζυμομύκητας αναγνωρίζεται από αποχρωματισμένα σημεία στην πρόθεση. Οι εναποθέσεις ζυμομυκήτων ενδέχεται να προκαλέσουν παραμόρφωση και βλάβη της βαλβίδας, δηλ. διαρροή υγρού μέσω της πρόθεσης και αύξηση της απαραίτητης για την ομιλία πίεσης. Ενδέχεται να πρέπει να γίνεται συχνότερη αντικατάσταση της πρόθεσης. Μερικοί ασθενείς μπορούν να θεραπευτούν επιτυχώς από τους ζυμομύκητες με από του στόματος χορηγούμενα φάρμακα ή στοματικά διαλύματα. Αφαίρεση συσκευής Η προσαρτημένη ταινία λαιμού σε μία μόνιμη πρόθεση πρέπει να αφαιρείται μόνο αφού έχει επιβεβαιωθεί η τοποθέτηση του οισοφαγικού πτερυγίου συγκράτησης εντός του οισοφάγου. Μην προσπαθείτε ποτέ να εισάγετε ή να επανεισάγετε μία μόνιμη πρόθεση από την οποία έχει αφαιρεθεί η ταινία λαιμού F

29 Μονιμη Φωνητικη Προθεση Blom-Singer Μην αφαιρείτε ποτέ μία μόνιμη πρόθεση και εισάγετε μία άλλη πρόθεση χωρίς να πραγματοποιήσετε πρώτα διαστολή του πόρου της TE παρακέντησης και μέτρηση του σωλήνα, ώστε να επιβεβαιώσετε το σωστό μήκος της πρόθεσης. Ο ασθενής δεν πρέπει να προσπαθήσει ποτέ να αφαιρέσει τη συσκευή μόνος του ή να επιτρέψει σε κάποιον άλλο να αφαιρέσει την πρόθεση εκτός από έναν εκπαιδευμένο επαγγελματία υγείας με τα κατάλληλα προσόντα. Η μόνιμη πρόθεση δεν αποτελεί μία σταθερή συσκευή και απαιτείται η περιοδική αντικατάστασή της. Η μόνιμη πρόθεση μπορεί να παραμείνει στην TE παρακέντηση μέχρι να σταματήσει να λειτουργεί σωστά, δηλαδή μέχρι να εμφανιστεί διαρροή ή να μην παρέχει επαρκή φωνή για την ομιλία. Πολλοί ιατροί συνιστούν τις τακτικές αξιολογήσεις παρακολούθησης σε διάστημα που δεν ξεπερνά τους 6 μήνες. Η αφαίρεση της μόνιμης πρόθεσης πρέπει να γίνεται μόνο ενώ κρατάτε σε ασφαλή θέση το εξωτερικό τραχειακό πτερύγιο της συσκευής με μια αιμοστατική λαβίδα ασφάλισης. Τραβήξτε απαλά και σταθερά μέχρι να αφαιρεθεί εντελώς η πρόθεση. Προσοχή: Ο Ομοσπονδιακός νόμος (ΗΠΑ) επιτρέπει την πώληση της παρούσας συσκευής από ή κατόπιν εντολής κάποιου ιατρού ή λογοπαθολόγου με άδεια. ΕΠΙΠΛΟΚΕΣ Παρόλο που είναι σπάνιες, έχει αναγνωριστεί η εμφάνιση των παρακάτω επιπλοκών με τις προθέσεις σιλικόνης τύπου Blom Singer. Περιλαμβάνουν τα εξής: επιμόλυνση ή σήψη της στομίας, η οποία μπορεί να οδηγήσει στην αφαίρεση της πρόθεσης και/ή στην λήψη των κατάλληλων αντιβιοτικών, τυχαία εισρόφηση της πρόθεσης στις αεροφόρους οδούς, η οποία ενδέχεται να απαιτεί την αφαίρεση από έναν ιατρό, περιστασιακή εξώθηση της πρόθεσης, η οποία απαιτεί την αντικατάστασή της κατόπιν διαστολής της TE παρακέντησης και επιπλέον παρακολούθηση του προγράμματος φροντίδας της στομίας, διαστολή της παρακέντησης με αποτέλεσμα τη διαρροή υγρών γύρω από την πρόθεση, φλεγμονώδη αντίδραση γύρω από την περιοχή της παρακέντησης και σχηματισμός κοκκιώδους ιστού, μετατόπιση της πρόθεσης και επακόλουθο κλείσιμο της TE F 29

30 παρακέντησης, μη ελεγχόμενη διαρροή γύρω από την πρόθεση, η οποία απαιτεί τη χειρουργική επανεξέταση ή το κλείσιμο της παρακέντησης, δυσφαγία ή δυσκολία κατάποσης, σχισίματα ή άλλες ζημιές στη φωνητική πρόθεση λόγω ακατάλληλης χρήσης, εναποθέσεις ζυμομυκήτων τα οποία προκαλούν διαρροή μέσω της πρόθεσης ή δυσλειτουργία της βαλβίδας, τυχαία κατάποση της πρόθεσης στον οισοφάγο και/ή στον γαστρεντερικό σωλήνα. ΟΔΗΓΙΕΣ ΧΡΗΣΗΣ Οι ακόλουθες οδηγίες παρέχονται για αρχικές και δευτερεύουσες επεμβάσεις/επεμβάσεις αντικατάστασης. Αρχική τοποθέτηση φωνητικής πρόθεσης Η παρακάτω διαδικασία δίνεται από τον Ronald C. Hamaker, M.D., F.A.C.S., ως οδηγίες για την αρχική τοποθέτηση φωνητικής πρόθεσης κατά την στιγμή της τραχειοοισοφαγικής παρακέντησης στη διάρκεια της λαρυγγεκτομής. Σημείωση: Δεν απαιτείται η χρήση ενός συστήματος εισαγωγής για τις κάψουλες γέλης Blom-Singer για την αρχική τοποθέτηση της φωνητικής πρόθεσης. Πριν από τη χρήση της φωνητικής πρόθεσης, ελέγξτε τον μηχανισμό της βαλβίδας για να βεβαιωθείτε ότι είναι άθικτη και λειτουργεί σωστά. Η βαλβίδα με καπάκι, (διάγραμμα 8a), πρέπει να κλείσει επίπεδα στην επιφάνεια απόθεσης μέσα στην πρόθεση. Πιέστε το άκρο έναντι του πρόσθιου τοιχώματος 1,5 cm από το άκρο της τραχείας. Κόψτε το οριζόντια στο άκρο μέσω της τραχείας (διάγραμμα 9). 3. Το άκρο της αιμοστατικής λαβίδας με την ταινία λαιμού πιέζεται απαλά μέσω του τραχειοοισοφαγικού τοιχώματος μέσα στην τραχεία (διάγραμμα 10). 4. Η ταινία λαιμού συγκρατείται με μισ δεύτερη αιμοστατική λαβίδα και η φωνητική πρόθεση έλκεται μέσω του σημείου παρακέντησης (διάγραμμα 11). 5. Η φωνητική πρόθεση τοποθετείται στην τραχειοοισοφαγική παρακέντηση (διάγραμμα 12). 6. Η ταινία λαιμού που βρίσκεται στη φωνητική πρόθεση αποσπάται και το τραύμα κλείνεται. Η ομιλία αναβάλλεται για δύο εβδομάδες μετά την εγχείρηση ώστε να αποφευχθεί η δημιουργία προκαλούμενου από την πίεση του αέρα συριγγίου (διάγραμμα 13). Δευτερεύουσα τοποθέτηση φωνητικής πρόθεσης Η παρακάτω διαδικασία δίνεται από τον Eric D. Blom, Ph.D. ως οδηγίες για τη δευτερεύουσα τοποθέτηση φωνητικής πρόθεσης ή την αντικατάσταση της φωνητικής πρόθεσης, χρησιμοποιώντας το Σύστημα εισαγωγής για τις κάψουλες γέλης Blom-Singer. Παρακαλούμε ανατρέξτε στα διαγράμματα που βρίσκονται στο τέλος του παρόντος εγχειριδίου οδηγιών. Σημείωση: Όλες οι δευτερεύουσες διαδικασίες πρέπει να γίνονται μόνο χρησιμοποιώντας ένα δυνατό φως για να φωτίζετε την τραχειοστομία και την παρακέντηση. Η εισαγωγή ή η αφαίρεση της πρόθεσης στην TE παρακέντηση πρέπει να γίνεται μόνο έχοντας ένα δυνατό φως στραμμένο απευθείας προς την στομία. Συνιστάται η χρήση γαντιών και προστατευτικών για τα μάτια για την ακόλουθη διαδικασία. Η μόνιμη πρόθεση και τα εξαρτήματά της πρέπει να είναι πάντα πλήρως καθαρά και στεγνά ώστε να αποφευχθεί η εισαγωγή μικροβίων μέσα στην τραχειοστομία, την TE παρακέντηση ή τον οισοφάγο F 31

Προαιρετικό: Ως εναλλακτική διαδικασία μπορεί να χρησιμοποιηθεί ο μετρητής μεγέθους ή ο διαστολέας παρακέντησης Blom-Singer. Ανατρέξτε στις οδηγίες χρήσης που παρέχονται με το προϊόν. 1. Διαστείλετε την TEP παρακέντηση με έναν διαστολέα-μετρητή μεγέθους Blom-Singer Dilator-Sizer με τη σωστή διάμετρο. Εισάγετε απαλά τον διαστολέα-μετρητή μεγέθους εντελώς μέσα στην παρακέντηση μέχρι την ταινία λαιμού (διάγραμμα 2) και αφήστε τον σε αυτό το σημείο για πέντε λεπτά. 2. Απομακρύνετε προσεκτικά τον διαστολέα-μετρητή μεγέθους μέχρι να ανιχνευθεί μικρή αντίσταση, η οποία υποδεικνύει ότι τα πτερύγια του οισοφάγου είναι τοποθετημένα επάνω στο πρόσθιο τοίχωμα του οισοφάγου (διάγραμμα 2a). Σημείωση: Εάν τραβήξετε τον διαστολέα-μετρητή μεγέθους πολύ σφιχτά μπορεί να προκαλέσει στρίψιμο ή συμπίεση του πόρου της TE παρακέντησης, με αποτέλεσμα λανθασμένη μέτρηση, η οποία υποδεικνύει ότι είναι πιο κοντό από ότι είναι πραγματικά. 3. Σημειώστε τον δείκτη που βρίσκεται πιο κοντά στην TE παρακέντηση. Διαβάστε τον αριθμό του μεγέθους στον διαστολέα-μετρητή μεγέθους και επιλέξτε μία μόνιμη πρόθεση F

33 Μονιμη Φωνητικη Προθεση Blom-Singer Blom-Singer με το αντίστοιχο μέγεθος. Εάν η μέτρηση είναι μεταξύ δύο τιμών, επιλέξτε την πιο μεγάλη. 4. Εισάγετε πλήρως τον διαστολέα-μετρητή μεγέθους και στερεώστε την ταινία λαιμού με λευκοπλάστ στον λαιμό του ασθενή. Αφήστε τον διαστολέα-μετρητή μεγέθους στην παρακέντηση για να τη διαστείλει για 5 λεπτά, ή περισσότερο εφόσον χρειάζεται, ώστε να επιτρέψει την εύκολη εισαγωγή της επιλεγμένης μόνιμης πρόθεσης. Πριν την εισαγωγή της μόνιμης πρόθεσης Blom-Singer, αφαιρέστε το λευκοπλάστ από την ταινία λαιμού, κρατήστε τον διαστολέα σταθερά και αφαιρέστε τον προσεκτικά από την παρακέντηση. Εισάγετε αμέσως τη μόνιμη πρόθεση (βλ. παρακάτω). Προετοιμασία πρόθεσης Σημείωση: Πριν από τη χρήση της μόνιμης πρόθεσης, ελέγξτε τον μηχανισμό της βαλβίδας για να βεβαιωθείτε ότι είναι άθικτη και λειτουργεί σωστά. Η βαλβίδα με καπάκι, (διάγραμμα 1a), πρέπει να κλείσει επίπεδα στην επιφάνεια απόθεσης μέσα στην πρόθεση. Προετοιμάστε την πρόθεση για εύκολη εισαγωγή χωρίς τραυματισμούς, χρησιμοποιώντας το Σύστημα εισαγωγής για τις κάψουλες γέλης Blom Singer. Χρησιμοποιήστε μόνο τις ειδικών διαστάσεων κάψουλες γέλης με αυτό το προϊόν. Το Σύστημα εισαγωγής για τις κάψουλες γέλης είναι σχεδιασμένο για χρήση μόνο με τις προθέσεις Blom-Singer. Μην το χρησιμοποιείτε με συσκευές άλλου τύπου. Τα παρακάτω βήματα περιγράφουν τη μέθοδο διπλώματος του χεριού για την εισαγωγή της κάψουλας γέλης. 1. Αφαιρέστε την πρόθεση από τη συσκευασία. Πάντα να χειρίζεστε την πρόθεση με καθαρά χέρια φορώντας γάντια. Βεβαιωθείτε ότι τα χέρια σας και η πρόθεση είναι τελείως στεγνά πριν τοποθετήσετε την κάψουλα γέλης. Εάν δεν το κάνετε, η κάψουλα γέλης ενδέχεται να διαλυθεί πριν την ώρα που πρέπει. 2.
