CLINICAL BACKGROUND For technical assistance or ordering information contact : Tel : +32 (0) Fax : +32 (0) A Biological activities of insulin Insulin, a polypeptide hormone with a molecular weight of 5800 Da, is secreted by the beta cells of the islets of Langerhans from the pancreas. Insulin possesses a wide spectrum of biological actions. It stimulates cellular glucose uptake, glucose oxidation, glycogenesis, lipogenesis, proteogenesis and the formation of DNA and RNA. Insulin plays a key role in the regulation of plasma glucose levels (hepatic output inhibition, stimulation of peripheral glucose utilisation). The resulting hypoglycemic effects of insulin are counterbalanced by hormones with hyperglycemic effects (glucagon, growth hormone, cortisol, epinephrin). Insulin secretion is mainly controlled by the plasma glucose levels : hyperglycemia induces a prompt and important increase in circulating insulin levels. Neural influences, as well as various metabolic and hormonal factors (amino acids, glucagon, gastro intestinal hormone) also participate to the control of insulin secretion. Type I (insulin dependent : "juvenile") diabetes is due to a destruction of the beta cells, with a consequence of absolute lack of insulin. In type II (noninsulin-dependent : "maturity onset") diabetes, insulin resistance may play an important role; however after several years of evolution, beta-cells failure may occur, leading to a relative insulinopenia requiring, in some cases, insulin administration. Insulin resistance is associated with high circulation levels of the hormone. The most common case of insulin resistance is represented by obesity. Various endocrinopathies (acromegaly, Cushing syndrome) as well as rare cases of insulin receptor defects or cases with anti-insulin receptor antibodies are associated with glucose intolerance or even diabetes due to insulin resistance. The determination of plasma insulin levels is an important parameter in the diagnosis of hypoglycemia. Insulin levels are high in cases of insulinoma (beta-cell tumor). Functional postprandial hypoglycemia may also be associated with inappropriate insulin release to carbohydrate intake. Insulin levels are determined either in the fasting state or during dynamic test : a) stimulation test : carbohydrate rich meal, oral glucose tolerance test (OGTT), arginin infusion, tolbutamide or other sulfonylureas administration. b) inhibition test : fasting, somatostatine infusion B. Clinical application of insulin determination. Determination of the beta-cell reserve during glucose tolerance test or after a carbohydrate rich meal, as a guide for the instauration of insulin therapy;. Contribution to the diagnosis of insulin and non-insulin-dependent diabetes;. Characterisation and follow-up of states of glucose intolerance;. Diagnosis and study of cases of insulin resistance;. Diagnosis of insulinoma and other causes of hypoglycemia.

4 IV. PRINCIPLES OF THE METHOD VI. SUPPLIES NOT PROVIDED The DIAsource INS-EASIA is a solid phase Enzyme Amplified Sensitivity Immunoassay performed on breakable microtiterplates. The assay uses monoclonal antibodies (Ms) directed against distinct epitopes of insulin. Calibrators and samples react with the capture monoclonal antibody (M 1) coated on microtiter well and with a monoclonal antibody (M 2) labelled with horseradish peroxidase (HRP). After an incubation period allowing the formation of a sandwich: coated M 1 human insulin M 2 HRP, the microtiterplate is washed to remove unbound enzyme labelled antibody. Bound enzyme-labelled antibody is measured through a chromogenic reaction. Chromogenic solution (TMB ready for use) is added and incubated. The reaction is stopped with the addition of Stop Solution and the microtiterplate is then read at the appropriate wavelength. The amount of substrate turnover is determined colourimetrically by measuring the absorbance, which is proportional to the insulin concentration. A calibration curve is plotted and INS concentration in samples is determined by interpolation from the calibration curve. V. REAGENTS PROVIDED The following material is required but not provided in the kit: 1. High quality distilled water 2. Pipettes for delivery of: μl, 0 µl and 2 ml (the use of accurate pipettes with disposable plastic tips is recommended) 3. Vortex mixer 4. Magnetic stirrer 5. Washer for Microtiterplates 6. Microtiterplate reader capable of reading at 4 nm nm and 6 nm (bichromatic reading) VII. REAGENT PREPARATION A. Calibrators : Reconstitute the zero calibrator with 2.0 ml distilled water and other calibrators with 1 ml distilled water. B. Controls : Reconstitute the controls with 1 ml distilled water. C. Working Wash solution : Prepare an adequate volume of Working Wash solution by adding 199 volumes of distilled water to 1 volume of Wash Solution (200x). Use a magnetic stirrer to homogenize. Discard unused Working Wash solution at the end of the day. Reagents Microtiterplate with 96 anti INS (monoclonal antibodies) coated breakable wells. STORAGE AND EXPIRATION DATING OF REAGENTS Before opening or reconstitution, all kits components are stable until the expiry date, indicated on the vial label, if kept at 2 to 8 C. Unused wells must be stored, at 2-8 C, in a sealed bag containing a desiccant until expiration date. After reconstitution, calibrators and controls are stable for 1 week at 2 to 8 C. For longer storage periods, aliquots should be made and kept at -20 C. Avoid successive freeze thaw cycles. The concentrated Wash Solution is stable at room temperature until expiration date. Freshly prepared Working Wash solution should be used on the same day. After its first use, the conjugate is stable until expiry date, if kept in the original well-closed vial at 2 to 8 C. Alterations in physical appearance of kit reagents may indicate instability or deterioration. Zero calibrator in human serum and thymol IX. Serum must be kept at 2-8 C. If the test is not run within 24 hours, storage in aliquots at -20 C is recommended. Avoid subsequent freeze thaw cycles. Prior to use, all samples should be at room temperature. It is recommended to vortex the samples before use. Do not use haemolysed samples. X. Use the zero calibrator for sample dilutions μiu of the calibrator preparation is equivalent to 1 μiu of 2 nd IRP 66/304. A. Handling notes Do not use the kit or components beyond expiry date. Do not mix materials from different kit lots. Bring all the reagents to room temperature prior to use. Thoroughly mix all reagents and samples by gentle agitation or swirling. Perform calibrators, controls and samples in duplicate. Vertical alignment is recommended. Use a clean plastic container to prepare the Wash Solution. In order to avoid cross-contamination, use a clean disposable pipette tip for the addition of each reagent and sample. For the dispensing of the Chromogenic Solution and the Stop Solution avoid pipettes with metal parts. High precision pipettes or automated pipetting equipment will improve the precision. Respect the incubation times. To avoid drift, the time between pipetting of the first calibrator and the last sample must be no longer than 30 minutes. Prepare a calibration curve for each run, do not use data from previous runs. Dispense the Chromogenic Solution within 15 minutes following the washing of the microtiterplate. During incubation with Chromogenic Solution, avoid direct sunlight on the microtiterplate.

5 B. Procedure 1. Select the required number of wells for the run. The unused wells should be resealed in the bag with a desiccant and stored at 2-8 C. 2. Secure the wells into the holding frame. 3. Pipette µl of each Calibrator, Control and Sample into the appropriate wells. 4. Pipette µl of anti-ins-hrp conjugate into all the wells. 5. Incubate for 30 minutes at room temperature 6. Aspirate the liquid from each well. 7. Wash the plate 3 times by: dispensing 0.4 ml of Wash Solution into each well aspirating the content of each well 8. Pipette 100 µl of Chromogenic Solution the into each well within 15 minutes following the washing step. 9. Incubate the microtiterplate for 15 minutes at room temperature. 10. Pipette 100 µl of Stop Reagent into each well. 11. Read the absorbances at 4 nm (reference filter 630 nm or 6 nm) within 1 hour and calculate the results as described in section XI. C. Read the plate at 4 nm against a reference filter set at 6 nm (or 630 nm). 2. Calculate the mean of duplicate determinations. 3. On semi-logarithmic or linear graph paper plot the OD values (ordinate) for each calibrator against the corresponding concentration of INS (abscissa) and draw a calibration curve through the calibrator points by connecting the plotted points with straight lines. 4. Read the concentration for each control and sample by interpolation on the calibration curve. 5. Computer assisted data reduction will simplify these calculations. PERFORMANCE AND LIMITATIONS A. Detection Limit Twenty zero calibrators were assayed along with a set of other calibrators. The detection limit, defined as the apparent concentration two standard deviations above the average OD at zero binding, was 0.17 μiu/ml. B. Specificity Cross-reactive hormones were added to a high value calibrator (100 μiu/ml or 4 ng/ml). The apparent INS response was measured. As shown hereafter, animal insulins (except rat insulin) cross-react whereas human, pork and beef proinsulins present no cross-reaction. E. INTERNAL QUALITY CONTROL If the results obtained for Control 1 and/or Control 2 are not within the range specified on the vial label, the results cannot be used unless a satisfactory explanation for the discrepancy has been given. If desirable, each laboratory can make its own pools of control samples, which should be kept frozen in aliquots. Acceptance criteria for the difference between the duplo results of the samples should rely on Good Laboratory Practises It is recommended that Controls be routinely assayed as unknown samples to measure assay variability. The performance of the assay should be monitored with quality control charts of the controls. It is good practise to check visually the curve fit selected by the computer.

6 XV. REFERENCE INTERVALS XVIII. SUMMARY OF THE PROTOCOL The range of insulin levels in 29 subjects with normal oral glucose tolerance tests, was 5 to 19 µiu/ml, the range is based on 2.5 to 97.5 percentiles of the dataset. These values are given only for guidance; each laboratory should establish its own normal range of values. CALIBRATORS (µl) SAMPLE(S) CONTROLS (µl) XVI. PRECAUTIONS AND WARNINGS Safety For in vitro diagnostic use only. The human blood components included in this kit have been tested by European approved and/or FDA approved methods and found negative for HBsAg, anti- HCV, anti-hiv-1 and 2. No known method can offer complete assurance that human blood derivatives will not transmit hepatitis, AIDS or other infections. Therefore, handling of reagents, serum or plasma specimens should be in accordance with local safety procedures. All animal products and derivatives have been collected from healthy animals. Bovine components originate from countries where BSE has not been reported. Nevertheless, components containing animal substances should be treated as potentially infectious. Avoid any skin contact with all reagents, Stop Solution contains HCl. In case of contact, wash thoroughly with water Do not smoke, drink, eat or apply cosmetics in the working area. Do not pipette by mouth. Use protective clothing and disposable gloves. Calibrators (0-5) Samples, Controls Anti-INS-HRP conjugate - Incubate for 30 min at room temperature. CONTEXTE CLINIQUE A Activités biologiques de l insuline L insuline, une hormone polypeptidique avec un poids moléculaire de 5800 Da, est sécrétée par les cellules β des îlots de Langerhans dans le pancréas. L insuline possède un large spectre d actions biologiques. Elle stimule la prise de glucose cellulaire, l oxidation du glucose, la glycogenèse, la lipogenèse, la protéogenèse et la formation de l ADN et de l ARN. L insuline joue un rôle clef dans la régulation des niveaux de glucose dans le plasma (Inhibition de production hépatique, stimulation d'utilisation de glucose périphérique). Les effets de hypoglycémique résultant de l'insuline sont contrebalancés par des hormones avec des effets de hyperglycémie (glucagon, l'hormone de croissance, cortisol, l'adrénaline). La sécrétion d'insuline est principalement contrôlée par les niveaux de glucose dans le plasma : l'hyperglycémie incite une augmentation prompte et importante du niveau d'insuline circulante. Des influences neurales, aussi bien que des facteurs métaboliques et hormonaux divers (des acides aminés, glucagon, l'hormone gastro-intestinale) participent aussi au contrôle de sécrétion d'insuline. Le diabète de Type I (insuline dépendant : "juvénile") est provoqué par la destruction des cellules β, avec pour conséquence l absence totale d insuline. Dans le diabète de type II (non-insulino-dépendant : "Début de la maturité ), la résistance à l insuline peut jouer un rôle important; toutefois après plusieurs années d évolution, un manque de cellules β peut arriver, conduisant à une demande relative d insulinopenia, dans certains cas, à l administration d insuline. La résistance à l insuline est associée à des niveaux de circulation élevés de l hormone. Le cas le plus répandu de résistance à l insuline est représenté par l obésité. De nombreuses maladies endocrinologiques (acromégalie, le syndrome de Cushing) aussi bien que les rares cas de défaut de récepteur d'insuline ou les cas impliquant les anticorps anti-récepteur à insuline sont associés à une intolérance au glucose ou même au diabète en raison de la résistance à l'insuline. La détermination de niveaux d'insuline dans le plasma est un paramètre important dans le diagnostic de l hypoglycémie. Les niveaux d insuline sont élevés dans les cas d «insulinoma» (tumeur des cellules β). L hypoglycémie fonctionnelle postprandiale peut aussi être associée a une libération inappropriée d insuline et à la consommation d hydrates de carbone. Les niveaux d insuline sont déterminés soit à jeun soit pendant un essai dynamique: a) test de stimulation: repas riche en hydrates de carbone, test de tolérance de glucose oral (OGTT), infusion d arginine, administration de tolbutamide ou autres sulfonylurées. b) test d inhibition : diète, infusion de somatostatine B. Application clinique du dosage de l insuline. Détermination des réserves des cellules β pendant le test de tolérance au glucose ou après un repas riche en hydrates de carbone, comme référence pour l établissement de la thérapie à l insuline;. Contribution au diagnostic des diabètes d insuline et non-insulino-dépendant;. Caractérisation et suivi des états d intolérance au glucose;. Diagnostic et étude des cas de résistance à l insuline;. Diagnostic de l insulinoma et des autres causes d hypoglycémie.

8 IV. PRINCIPES DU DOSAGE La DIAsource INS-EASIA est une «Enzyme Amplified Sensitivity Immunoassay» en phase solide effectuée sur des plaques de microtitration sécables. La trousse utilise des anticorps monoclonaux (Ms) dirigés contre différents épitopes de l insuline. Les calibrateurs et les échantillons réagissent avec l anticorps de capture monoclonal (M 1) recouvrant les puits et avec un anticorps monoclonal (M 2) marqué avec la peroxydase (HRP). Après une période d incubation permettant la formation d un sandwich: M 1 recouvert insuline humaine M 2 HRP, la micro-plaque est lavée afin d enlever l anticorps libre marqué enzymatiquement. L anticorps lié marqué enzymatiquement est mesuré avec une réaction chromogénique. Une solution chromogénique (TMB prêt à utiliser) est ajoutée et incubée. La réaction est arrêtée avec l addition de Solution d arrêt et la micro-plaque est alors lue à la longueur d onde appropriée. La quantité de remplacement de substrat est déterminée colorimétriquement par la mesure de l absorbance, qui est proportionnelle à la concentration en insuline. Une courbe de calibration est dessinée et la concentration en INS dans les échantillons est déterminée par interpolation de la courbe de calibration. V. REACTIFS FOURNIS Reactifs Micro-plaque de titration sécable avec 96 puits recouvert d anti INS (anticorps monoclonal) Conjugué: anti-ins marqué avec de l HRP (anticorps monoclonal) dans un tampon TRIS-HCl avec de l'albumine bovine et du thymol CAL HRP Calibrateur zéro dans du sérum humain et du thymol 0 96 tests Kit Code Couleur Reconstitution 96 puits bleu Prêt à l emploi 1 flacon 6 ml 1 flacon lyophilisé Rouge Jaune Prêt à l emploi Ajouter 2,0 ml d eau distillée VII. PREPARATION DES REACTIFS A. Calibrateurs : Reconstituer le calibrateur zéro avec 2,0 ml d eau distillée et les autres calibrateurs avec 1 ml d eau distillée.. B. Contrôles : Reconstituer les contrôles avec 1 ml d eau distillée. C. Solution de Lavage : Préparer un volume adéquat de Solution de Lavage en ajoutant 199 volumes d eau distillée à 1 volume de Solution de Lavage (200x). Utiliser un agitateur magnétique pour homogénéiser. Eliminer la Solution de Lavage non utilisée à la fin de la journée. VIII. STOCKAGE ET DATE D EXPIRATION DES REACTIFS Avant l ouverture ou la reconstitution, tous les composants de la trousse sont stables jusqu à la date d expiration, indiquée sur l étiquette, si la trousse est conservée entre 2 et 8 C. Des puits inutilisées doivent être gardées, à 2-8 C, dans un sachet cacheté contenant un desiccant jusqu à la date d expiration. Après reconstitution, les calibrateurs et les contrôles sont stables pendant 7 jours entre 2 et 8 C. Pour de plus longues périodes de stockage, des aliquots devront être réalisés et ceux-ci seront gardés à 20 C pendant 3 mois. Eviter des cycles de congélation et décongélation successifs. La Solution de Lavage concentrée est stable à température ambiante jusqu à la date d expiration. La Solution de Lavage préparée doit être utilisée le jour même. Après la première utilisation, le conjugué est stable jusqu à la date d expiration, si celui-ci est conservé entre 2 et 8 C dans le flacon d origine correctement fermé. Des altérations dans l apparence physique des réactifs de la trousse peuvent indiquer une instabilité ou une détérioration. IX. PREPARATION ET STABILITE DE L ECHANTILLON Les échantillons de sérum doivent être gardés entre 2 et 8 C. Si le test n est pas réalisé dans les 24 heures, un stockage en aliquots à -20 C est recommandé. Eviter des cycles de congélation et décongélation successifs. Avant l utilisation des échantillons, ceux-ci doivent être à température ambiante. On recommande de vortexer les échantillons avant de les utiliser. Ne pas utiliser d échantillons hémolysés CAL Calibrateur N = 1 à 5 (cfr. Ajouter 1 ml d eau distillée X. MODE OPERATOIRE A. Notes de manipulation Ne pas utiliser la trousse ou ses composants après avoir dépassé la date d expiration. Ne pas mélanger du matériel provenant de trousses de lots différents. Mettre tous les réactifs à température ambiante avant utilisation. Mélangez tous les réactifs et les échantillons sous agitation douce. Réaliser les calibrateurs, les contrôles et les échantillons en double. Un alignement vertical est recommandé. Utiliser un récipient en plastic propre pour préparer la Solution de Lavage. Pour éviter toute contamination croisée, utiliser une nouvelle pointe de pipette pour l addition de chaque réactif et échantillon. Pour la distribution de solution du chromogène et de la Solution d arrêt, éviter des pipettes avec des parties en métal. CHROM STOP TMB SOLN MATERIELS NON FOURNIS Le matériel mentionné ci-dessous est requis mais non fourni avec la trousse: 1. Eau distillée d une haute qualité 2. Pipettes pour distribuer: μl, 0 μl et 2 ml (l utilisation de pipettes précises et de pointes en plastique est recommandée) 3. Agitateur vortex 4. Agitateur magnétique 5. Laveur de micro-plaques 6. Lecteur de micro-plaques capable de lire à 4 nm et 6 nm (lecture bichromatique) Des pipettes de haute précision ou un équipement de pipetage automatique permettent d augmenter la précision. Respecter les temps d incubation. Afin d éviter des anomalies, le délai entre le pipetage du premier calibrateur et celui du dernier échantillon ne peut pas dépasser 30 minutes. Préparer une courbe d étalonnage pour chaque nouvelle série d expériences, ne pas utiliser les données d expériences précédentes. Distribuer la solution du chromogène dans les 15 minutes après le lavage de la micro-plaque de titration. Eviter exposition à la lumière du soleil lors de l incubation avec la solution du chromogène. B. Mode opératoire 1. Sélectionner le nombre de puits nécessaires pour le test. Les puits inutilisées doivent être cachetées de nouveau dans le sachet avec un desiccant et gardées à 2-8 C. 2. Placer les puits dans le support. 3. Pipeter µl de chaque Calibrateur, Contrôle et Echantillon dans les puits appropriés. 4. Pipeter µl du conjugué anti-ins-hrp dans tous les puits. 5. Incuber pendant 30 minutes à température ambiant. 6. Aspirer le liquide de chaque puits. 7. Laver la plaque 3 fois en: distribuant 0.4 ml de la Solution de Lavage dans chaque puits aspirant le contenu de chaque puits 8. Pipeter 100 µl de la solution chromogénique dans chaque puits dans les 15 minutes après la phase de lavage.

9 9. Incuber la micro-plaque pendant 15 minutes à température ambiante. 10. Pipeter 100 µl du Réactif d arrêt dans chaque puits. 11. Lire les absorbances à 4 nm (filtre de référence 630 nm ou 6 nm) endéans l heure et calculer les résultats comme décrits dans la section XI. XI. CALCUL DES RESULTATS 1. Lire la plaque à 4 nm contre un filtre de référence mis à 6 nm (ou 630 nm). 2. Calculer la moyenne de chaque détermination réalisée en double. 3. Dessiner sur un graphique linéaire ou semi-logarithmique les cpm (ordonnées) pour chaque calibrateur contre la concentration correspondante en INS (abscisses) et dessiner une courbe de calibration à l aide des points de calibration, en connectant les points avec des lignes droites. 4. Lire la concentration pour chaque contrôle et échantillon par interpolation sur la courbe de calibration. 5. L analyse informatique des données simplifiera les calculs. Si un système d analyse de traitement informatique des données est utilisé, il est recommandé d utiliser la fonction «4 paramètres» du lissage de courbes. D. 10 XVI. PRECAUTIONS ET AVERTISSEMENTS Sécurité Pour utilisation en diagnostic in vitro uniquement. Les composants de sang humain inclus dans ce kit ont été évalués par des méthodes approuvées par l Europe et/ou la FDA et trouvés négatifs pour HBsAg, l anti-hcv, l anti-hiv-1 et 2. Aucune méthode connue ne peut offrir l'assurance complète que des dérivés de sang humain ne transmettront pas d hépatite, le sida ou toute autre infection. Donc, le traitement des réactifs, du sérum ou des échantillons de plasma devront être conformes aux procédures locales de sécurité. Tous les produits animaux et leurs dérivés ont été collectés d'animaux sains. Les composants bovins proviennent de pays où l ESB n'a pas été détectée. KLINISCHER HINTERGRUND A Biologische Aktivitäten von Insulin Insulin, ein Polypeptidhormon mit einem Molekulargewicht von 5800 Da, wird von den Betazellen der Langerhansschen Inseln der Bauchspeicheldrüse sezerniert. Insulin besitzt ein weites Spektrum biologischer Aktivitäten. Es stimuliert die zelluläre Glucoseaufnahme, die Oxidation von Glucose, die Glykogenese, die Lipogenese, die Proteogenese und die Bildung von DNA und RNA. Insulin spielt eine Schlüsselrolle in der Regulation des Plasmaglucosespiegels (Hemmung der Freisetzung aus der Leber, Stimulation der peripheren Glucoseverbrennung). Die daraus resultierenden hypoglykämischen Effekte von Insulin werden von Hormonen mit hyperglykämischer Wirkung kompensiert (Glukagon, Wachstumshormon, Cortisol, Adrenalin). Die Insulinsekretion wird hauptsächlich über den Plasmaglucosespiegel kontrolliert: Hyperglykämie induziert einen sofortigen und wichtigen Anstieg des Insulinspiegels im Blutkreiskauf. Neuronale Einflüsse sowie verschiedene metabolische und hormonelle Faktoren (Aminosäuren, Glukagon, Gastrointestinales Hormon) spielen ebenfalls eine Rolle in der Regulation der Insulin Sekretion. Typ I (insulinabhängiger : "juveniler") Diabetes resultiert aus einer Zerstörung der Betazellen, was ein absolutes Fehlen von Insulin zur Folge hat. Bei Typ II (nicht-insulin-abhängiger : "Alters-") Diabetes, kann eine Insulinresistenz eine wichtige Rolle spielen; jedoch kann nach einigen Jahren Krankengeschichte eine Zerstörung der Betazellen erfolgen, die zu einer relativen Insulinopenie führt und in einigen Fällen Insulingabe benötigt. Insulinresistenz ist mit einem hohen Blutspiegel dieses Hormons gekoppelt. Am häufigsten kommt Insulinresistenz bei Fettsucht vor. Verschiedene Endokrinopathien (Akromegalie, Cushing Syndrom), sowie seltene Fälle von Insulinrezeptordefekten oder Fälle mit Anti-Insulin-Rezeptorantikörpern sind mit Glukoseintoleranz oder sogar Diabetes aufgrund einer Insulinresistenz gekoppelt. Die Bestimmung des Plasmainsulinspiegels ist ein wichtiger Parameter bei der Diagnose der Hypoglykämie. Der Insulinspiegel ist hoch bei einem Insulinom (Betazelltumor). Eine funktionelle postprandiale Hypoglykämie kann ebenfalls zu einer unangemessenen Insulinausschüttung nach Kohlenhydrataufnahme führen. Insulinspiegel werden entweder nüchtern oder mithilfe eines dynamischen Tests bestimmt : a) Stimulationstest: Kohlenhydratreiche Nahrung, oraler Glucosetoleranztest (OGTT), Arginin-Infusion, Einnahme von Tolbutamid oder anderen Sulfonylharnstoffen. b) Inhibitionstest : Fasten, Somatostatin-Infusion B. Klinische Anwendung der Insulinbestimmung. zur Bestimmung der Betazellreserve bei einem Glucosetoleranztest oder nach einem kohlenhydratreichen Mahl, als Leitlinie für die Etablierung einer Insulintherapie;. als Beitrag zur Diagnose des Insulin- und nicht-insulin-abhängigen Diabetes;. zur Charakterisierung und Weiterverfolgung von Stadien der Glucoseintoleranz;. zur Diagnose und Untersuchung bei Fällen von Insulinresistenz;. zur Diagnose des Insulinoms und anderer Uraschen der Hypoglykämie.

12 IV. GRUNDSÄTZLICHES ZUR DURCHFÜHRUNG VII. VORBEREITUNG DER REAGENZIEN Der DIAsource INS-EASIA ist ein solid phase-enzyme Amplified Sensitive Immunoassay (EASIA) in einer portionierbaren Mikrotiterplatte. Der Assay benutzt monoklonale Antikörper (MAk), die gegen verschiedene Epitope von Insulin gerichtet sind. Kalibratoren und Proben reagieren mit dem primären monoklonalen Antikörper (MAk 1), mit dem die Wells der Mikrotiterplatte beschichtet sind, und mit einem monoklonalen Antikörper (MAk 2), der mit Meerrettich-Peroxidase (MRP) markiert ist. Nach einer Inkubationsphase bildet sich ein Sandwich-Komplex: MAk 1 - Humaninsulin - MAk 2 - MRP; nicht gebundene enzymbeschriftete Antikörper werden durch Waschen der Mikrotiterplatte entfernt. Gebundene enzymbeschriftete Antikörper werden durch eine Farbreaktion gemessen. Farblösung (gebrauchsfertiges TMB) wird hinzugefügt und inkubiert. Die Reaktion wird durch Hinzufügen einer Stopplösung beendet und die Mikrotiterplatte wird bei adäquater Wellenlänge ausgewertet. Die Menge an Substratumsatz wird kolorimetrisch durch Messung der sorption bestimmt, die proportional zur Insulinkonzentration ist. Es wird eine Kalibrationskurve erstellt und die INS-Konzentration in den Proben wird durch Interpolation von der Kalibrationskurve bestimmt. V. Hochwertiges destilliertes Wasser 2. Pipetten: μl, 0 μl und 2 ml (Verwendung von Präzisionspipetten mit Wegwerf-Plastikspitzen wird empfohlen) 3. Vortex Mixer 4. Magnetrührer 5. Waschgerät für Mikrotiterplatten 6. Mikrotiterplatten-Lesegerät zur Auswertung bei 4 nm und 6 nm (Bichromatische Auswertung) A. Kalibratoren: Rekonstituieren Sie den Null-Kalibrator mit 2.0 ml dest. Wasser, die anderen Kalibratoren mit 1 ml dest. Wasser. B. Kontrollen: Rekonstituieren Sie die Kontrollen mit 1 ml dest. Wasser. C. Waschlösung: Bereiten Sie ein angemessenes Volumen Waschlösung aus einem Anteil Waschlösung (200x) mit 199 Anteilen dest. Wasser zu. Verwenden Sie einen Magnetrührer zum gleichmäßigen Durchmischen. Verwerfen Sie die nicht benutze Waschlösung am Ende des Tages. VIII. AUFBEWAHRUNG UND LAGERUNG DER REAGENZIEN Vor dem Öffnen oder Rekonstitution sind die Reagenzien des Kits bis zum laufdatum (Angaben auf Etikett) bei Lagerung bei 2 C bis 8 C stabil. Nicht verwendete Wells sollten dicht in der Folie verschlossen mit Trockenmittel bei 2 bis 8 C gelagert werden bis zum Verfallsdatum. Nach der Rekonstitution sind die Kalibratoren und Kontrollen bei 2 C bis 8 C 7 Tage stabil. Aliquots müssen bei längerer Aufbewahrung bei -20 C eingefroren werden, dan sind Sie 3 Monate haltbar. Vermeiden Sie wiederholtes Einfrieren und Auftauen. Die konzentrierte Waschlösung ist bei Raumtemperatur bis zum Verfallsdatum haltbar. Frisch zubereitete Waschlösung sollte am selben Tag benutzt werden. Nach der ersten Benutzung ist das Konjugat bei Aufbewahrung im Originalgefäß und bei 2 bis 8 C bis zum laufdatum stabil. Veränderungen im Aussehen der Kitkomponenten können ein Anzeichen für Instabilität oder Zerfall sein. IX. PROBENSAMMLUNG UND -VORBEREITUNG Serumproben müssen bei 2-8 C aufbewahrt werden. Falls der Test nicht innerhalb von 24 Std. durchgeführt wird, ist die Aufbewahrung bei -20 C erforderlich. Vermeiden Sie wiederholtes Einfrieren und Auftauen. Vor Gebrauch müssen alle Proben Raumtemperatur erreichen. Vortexmischen der Proben wird vor Gebrauch empfohlen. Keine hämolytischen Proben benutzen X. DURCHFÜHRUNG A. Bemerkungen zur Durchführung Verwenden Sie den Kit oder dessen Komponenten nicht nach laufdatum. Vermischen Sie Materialen von unterschiedlichen Kit-Chargen nicht. Bringen Sie alle Reagenzien vor der Verwendung auf Raumtemperatur. Mischen Sie alle Reagenzien und Proben gründlich durch sanftes Schütteln oder Rühren. Führen Sie Kalibratoren, Kontrollen und Proben doppelt aus. Vertikale Ausrichtung wird empfohlen. Verwenden Sie zur Zubereitung der Waschlösung reinen Kunststoffbehälter. Verwenden Sie saubere Wegwerf-Pipettenspitzen, um Kreuzkontamination zu vermeiden. Verwenden Sie zur Pipettierung der chromogene Lösung und der Stopplösung keine Pipetten mit Metallteilen. Präzisionspipetten oder ein automatisches Pipettiersystem erhöhen die Präzision. Achten Sie auf die Einhaltung der Inkubationszeiten. Zur Vermeidung eines Drifts dürfen zwischen der Pipettierung des ersten Kalibrators und der letzten Probe nicht mehr als 30 Minuten verstreichen. Erstellen Sie für jeden Durchlauf eine Standardkurve, verwenden Sie nicht die Daten von früheren Durchläufen. Pipettieren Sie die chromogene Lösung innerhalb von 15 Minuten nach dem Waschen der Mikrotiterplatte. Während der Inkubation mit der chromogene Lösung ist die Mikrotiterplatte vor direktem Sonnenlicht zu schützen. B. Durchführung 1. Wählen Sie die erforderliche Anzahl der Wells für den Lauf aus. Nicht verwendete Wells sollten wieder dicht in der Folie verschlossen mit Trockenmittel bei 2 bis 8 C gelagert werden. 2. Befestigen Sie die Wells im Halterahmen. 3. Pipettieren Sie jeweils µl Kalibrator, Kontrolle und Probe in die entsprechenden Wells. 4. Pipettieren Sie µl Anti-INS-MRP Konjugat in alle Wells. 5. Inkubieren Sie 30 Minuten bei Raumtemperatur 6. Saugen Sie die Flüssigkeit aus jedem Well ab. 7. Waschen Sie die Platte dreimal: pipettieren Sie 0,4 ml Waschlösung in jeden Well saugen Sie der Inhalt jedes Wells ab 8. Pipettieren Sie 100 µl der chromogene Lösung innerhalb von 15 Minuten nach dem Waschvorgang in jeden Well.

INTERNE QUALITÄTSKONTROLLE Entsprechen die Ist-Werte nicht den auf den Fläschchen angegebenen Soll- Werten, können die Werte, ohne treffende Erklärung der weichungen, nicht weiterverarbeitet werden. Falls zusätzliche Kontrollen erwünscht sind, kann jedes Labor seinen eigenen Pool herstellen, der in Aliquots eingefroren werden sollte. Akzeptanzkriterien für die Differenz zwischen den Resultaten der Wiederholungstests anhand der Proben müssen auf Guter Laborpraxis beruhen. Es wird empfohlen, Kontrollen im Assay routinemäßig wie unbekannte Proben zu behandeln, um die Assayvarianz zu messen. Die Leistung des Assay muss mit den Qualitätskontrollkarten der Kontrollen überprüft werden. Es hat sich bewährt, die durch den Computer ausgewählte Kurvenanpassung visuell zu überprüfen. C.
