Biological Activity Insulin is synthesized in the betacells of the islets of Langerhans as a precursor molecule, proinsulin. In the secretory granules of the betacells, proinsulin is cleaved into insulin and into a 31aminoacid peptide, called the Connecting Peptide or CPeptide. Insulin and CPeptide are secreted in equimolar amounts. However, because of its longer halflife, the plasma concentration of Cpeptide is higher than that of insulin. The determination of plasma CPeptide allows an assessment of the endogenous insulin production, even in the presence of exogenous insulin administration or in the presence of circulating antiinsulin antibodies. Moreover, the determination of CPeptide in urine provides a reliable index of the insulin production when blood sampling is difficult or when an integrated estimation of CPeptide secretion over a period of several hours is requested. B. Clinical applications. Assessment of residual betacell function in diabetics under insulin therapy. Detection and monitoring of the remission phase of type I diabetes. Adjunct in the differential diagnosis between type I (insulindependent) and type II (noninsulindependent) diabetes. Diagnosis of insulininduced factitious hypoglycaemia. Contribution to the diagnosis of insulinoma (insulin suppression test). Prognostic index of foetal outcome in pregnant diabetic women. Evaluation of insulin secretion in liver disease. Monitoring of pancreatectomy

4 IV. PRINCIPLES OF THE METHOD VI. SUPPLIES NOT PROVIDED The BioSource CPEPEASIA is a solid phase Enzyme Amplified Sensitivity Immunoassay performed on a microtiterplate. A fixed amount of CPEPTIDE labelled with horseradish peroxidase (HRP), compete with unlabelled C PEPTIDE present in the calibrators, controls and samples for a limited number of binding sites on a specific antibody. After 2 hours incubation at room temperature, the microtiterplate is washed to stop the competition reaction. The chromogenic solution (TMB H 2O 2) is added and incubated for 30 min. The reaction is stopped with the addition of Stop Solution and the microtiterplate is then read at the appropriate wavelength. The amount of substrate turnover is determined colourimetrically by measuring the absorbance, which is inversely proportional to the CPEPTIDE concentration. A calibration curve is plotted and CPEPTIDE concentration in samples is determined by interpolation from the calibration curve. V. REAGENTS PROVIDED Reagents Microtiterplate with 96 anti C PEPTIDE coated wells 96 tests Kit Color Code Reconstitution 96 wells blue Ready for use The following material is required but not provided in the kit: 1. High quality distilled water 2. Pipettes for delivery of: 50 μl, 100 μl, 0 µl, 500 µl, 1 ml and 2 ml (the use of accurate pipettes with disposable plastic tips is recommended) 3. Vortex mixer 4. Magnetic stirrer 5. Horizontal microtiterplate shaker capable of 700 rpm ± 100 rpm 6. Washer for microtiterplates 7. Microtiterplate reader capable of reading at 450 nm and 650 nm (or 630 nm) VII. REAGENT PREPARATION A. Calibrators : Reconstitute the zero calibrator with 2.0 ml distilled water and the other calibrators with 1.0 ml distilled water. B. Controls : Reconstitute the controls with 0.5 ml distilled water. C. Working CPeptideHRP Conjugate : pipette 50 µl of the conjugate (concentrated CPeptideHRP solution) into 5 ml of conjugate buffer for 96 wells used. Extemporaneous preparation is recommended D. Working Wash solution : Prepare an adequate volume of Working Wash solution by adding 199 volumes of distilled water to 1 volume of Wash Solution (0x). Use a magnetic stirrer to homogenize. Discard unused Working Wash solution at the end of the day. VIII. STORAGE AND EXPIRATION DATING OF REAGENTS Ag Conjugate: HRP labelled C PEPTIDE (HPLC grade) in TRIS HCl buffer with bovine serum albumin and thymol CONJ Conjugate buffer : in TRIS MALEATE buffer with bovine casein and thymol CAL HRP 0 BUF CONC 1 vial 0.2 ml 1 vial 5.5 ml 1 vial lyophilized red red yellow Dilute 100 x with conjugate buffer Ready for use Add 2.0 ml distilled water Before opening or reconstitution, all kit components are stable until the expiry date, indicated on the vial label, if kept at 2 to 8 C. Unused strips must be stored, at 28 C, in a sealed bag containing a desiccant until expiration date. After reconstitution, calibrators and controls are stable for 1 week at 2 to 8 C. For longer storage periods, aliquots should be made and kept at C. Avoid successive freeze thaw cycles. The concentrated Wash Solution is stable at room temperature until expiration date. Freshly prepared Working Wash solution should be used on the same day. The Working CPEPTIDEHRP conjugate is stable for 18 hours at 2 to 8 C. Alterations in physical appearance of kit reagents may indicate instability or deterioration. Zero calibrator in human serum and thymol CAL N Calibrator N = 1 to 5 (see exact values on vial labels) in human serum and thymol 5 vials lyophilized yellow Add 1.0 ml distilled water IX. SPECIMEN COLLECTION AND PREPARATION Serum must be kept at 2 8 C. If the test is not run within 24 hours, storage in aliquots at C is recommended. Avoid subsequent freeze thaw cycles. Prior to use, all samples should be at room temperature. It is recommended to vortex the samples before use. Do not use haemolysed samples. Add 0.5 ml distilled water Ready for use Ready for use Note: 1. Use the zero calibrator for sample dilutions ng of the calibrator is equivalent to 1 ng of the NIBSC IRR 84/510. X. PROCEDURE A. Handling notes Do not use the kit or components beyond expiry date. Do not mix materials from different kit lots. Bring all the reagents to room temperature prior to use. Thoroughly mix all reagents and samples by gentle agitation or swirling. Perform calibrators, controls and samples in duplicate. Vertical alignment is recommended. Use a clean plastic container to prepare the Wash Solution. In order to avoid crosscontamination, use a clean disposable pipette tip for the addition of each reagent and sample. For the dispensing of the Chromogenic Solution and the Stop Solution avoid pipettes with metal parts. High precision pipettes or automated pipetting equipment will improve the precision. Respect the incubation times. To avoid drift, the time between pipetting of the first calibrator and the last sample must be limited to the time mentioned in section XIII paragraph E (Time delay). Prepare a calibration curve for each run, do not use data from previous runs. Dispense the Chromogenic Solution within 15 minutes following the washing of the microtiterplate. During incubation with Chromogenic Solution, avoid direct sunlight on the microtiterplate.

5 B. Procedure 1. Select the required number of strips for the run. The unused strips should be resealed in the bag with a desiccant and stored at 28 C. 2. Secure the strips into the holding frame. 3. Pipette 100 µl of each Calibrator, Control and Sample into the appropriate wells. 4. Pipette 50 µl of diluted anticpeptidehrp conjugate into all the wells. 5. Incubate for 2 hours at room temperature on a horizontal shaker set at 700 rpm ± 100 rpm. 6. Aspirate the liquid from each well. 7. Wash the plate 3 times by: dispensing 0.4 ml of Wash Solution into each well aspirating the content of each well 8. Pipette 100 µl of the chromogenic solution into each well within 15 minutes following the washing step. 9. Incubate the microtiterplate for 30 minutes at room temperature on a horizontal shaker set at 700 rpm ± 100 rpm, avoid direct sunlight. 10. Pipette 100 µl of Stop Solution into each well. 11. Read the absorbencies at 450 nm (reference filter 630 nm or 650 nm) within 3 hours and calculate the results as described in section XI. C. Read the plate at 450 nm against a reference filter set at 650 nm (or 630 nm). 2. Calculate the mean of duplicate determinations. 3. Calculate for each calibrator, control and sample: Serum DILUTION TEST OD B/B0 = (Calibrator, Control OD (Zero Calibrator ) or Sample) x Using either linearlinear of semilogarithmic graph paper, plot the (B/B0) values for each calibrator point as a function of the C PEPTIDE concentration of each calibrator point. Reject obvious outliers. 5. Computer assisted methods can also be used to construct the calibration curve. If automatic result processing is used, a 4parameter logistic function curve fitting is recommended. 6. By interpolation of the sample (B/B0 ) values, determine the C PEPTIDE concentrations of the samples from the calibration curve. Sample Dilution Theoretical Concent. INTERNAL QUALITY CONTROL The following data are for illustration only and should never be used instead of the real time calibration curve. If desirable, each laboratory can make its own pools of control samples, which should be kept frozen in aliquots. Controls that contain azide will interfere with the enzymatic reaction and cannot be used. Acceptance criteria for the difference between the duplicate results of the samples should rely on Good Laboratory Practises It is recommended that Controls be routinely assayed as unknown samples to measure assay variability. The performance of the assay should be monitored with quality control charts of the controls. It is good practise to check visually the curve fit selected by the computer. XIII. PERFORMANCE AND LIMITATIONS A. Detection Limit Twenty zero calibrators were assayed along with a set of other calibrators. The detection limit, defined as the apparent concentration two standard deviations above the average OD at zero binding, was 0.01 pmol/ml. B. Specificity The specificity was estimated by spiking a pool of CPeptide serum with concentration lower than 0.03 pmol/ml with the following peptides : Compound Quantity added (ng/ml) Crossreaction XV. The human blood components included in this kit have been tested by European approved and/or FDA approved methods and found negative for HBsAg, anti HCV, antihiv1 and 2. No known method can offer complete assurance that human blood derivatives will not transmit hepatitis, AIDS or other infections. Therefore, handling of reagents, serum or plasma specimens should be in accordance with local safety procedures.

6 All animal products and derivatives have been collected from healthy animals. Bovine components originate from countries where BSE has not been reported. Nevertheless, components containing animal substances should be treated as potentially infectious. Avoid any skin contact with all reagents, Stop Solution contains HCl, the chromogen contains TMB and H 2O 2. In case of contact, wash thoroughly with water. KLINISCHER HINTERGRUND A. Biologische Aktivität Insulin wird in den BetaZellen der LangerhansInseln als ein Vorläufermolekül, Proinsulin, synthetisiert. In den Sekretgranula der BetaZellen wird Proinsulin in Insulin und in ein Peptid mit 31 Aminosäuren, das so genannte CPeptid ( Connecting Peptide ) gespalten. Insulin und CPeptid werden in äquimolaren Mengen sezerniert. Wegen seiner längeren Halbwertszeit ist die Plasmakonzentration von CPeptid jedoch höher als jene von Insulin. Die Bestimmung von CPeptid im Plasma ermöglicht eine Beurteilung der endogenen Insulinproduktion, auch bei Vorliegen exogener Insulinverabreichung oder in Anwesenheit zirkulierender AntiInsulinAntikörper. Darüber hinaus bietet die Bestimmung von CPeptid im Harn einen zuverlässigen Index der Insulinproduktion, wenn die Abnahme von Blutproben schwierig ist oder wenn eine integrierte Schätzung der Sekretion von CPeptid über einen Zeitraum von mehreren Stunden erforderlich ist. 8 IV. GRUNDSÄTZLICHES ZUR DURCHFÜHRUNG VII. VORBEREITUNG DER REAGENZIEN Der BioSource CPEPEASIA ist ein solid phaseenzyme Amplified Sensitive Immunoassay (EASIA) im Mikrotiterplattenformat. Eine festgesetzte Menge an mit MeerrettichPeroxidase (MRP) markiertem CPEPTIDE konkurriert mit dem zu messenden, in der Probe oder in dem Kalibrator vorhandenen CPEPTIDE um eine festgesetzte Menge an Antikörperbindungsstellen. Nach 2stündiger Inkubation bei Raumtemperatur wird die Mikrotiterplatte gewaschen, um die Konkurrenzreaktion zu stoppen. Farblösung (TMB H 2O 2) wird hinzugefügt und inkubiert. Die Reaktion wird durch Hinzufügen einer Stopplösung beendet und die Mikrotiterplatte wird bei adäquater Wellenlänge ausgewertet. Die Menge an Substratumsatz wird kolorimetrisch durch Messung der Absorption bestimmt, die proportional zur C PEPTIDEKonzentration ist. Eine Kalibrationskurve wird gedruckt und die C PEPTIDEKonzentrationen der Proben werden über Dosis Interpolation der Kalibrationskurve bestimmt. V. Hochwertiges destilliertes Wasser 2. Pipetten: 50 μl, 100 µl, 0 µl, 500 μl, 1 ml und 2 ml (Verwendung von Präzisionspipetten mit Einwegplastikspitzen wird empfohlen) 3. Vortex Mixer 4. Magnetrührer 5. Horizontaler Schüttler für Mikrotiterplatte Kap. 700 rpm ± 100 rpm 6. Waschgerät für Mikrotiterplatten 7. MikrotiterplattenLesegerät zur Auswertung bei 450 nm und 650 nm (oder 630 nm) A. Kalibratoren: Rekonstituieren Sie den NullKalibrator mit 2,0 ml dest. Wasser, die anderen Kalibratoren mit 1,0 ml dest. Wasser. B. Kontrollen: Rekonstituieren Sie die Kontrollen mit 0,5 ml dest. Wasser. C. Gebrauchsfertiges CPEPTIDEHRPKonjugat: Lösen Sie 50 µl des konzentrierten CPEPTIDEHRPKonjugats in 5 ml Konjugatpuffer (für 96 Wells). Es wird empfohlen, die Lösung frisch zuzubereiten. D. Waschlösung: Bereiten Sie ein angemessenes Volumen Waschlösung aus einem Anteil Waschlösung (0 x) mit 199 Anteilen dest. Wasser zu. Verwenden Sie einen Magnetrührer zum gleichmäßigen Durchmischen. Werfen Sie die nicht benutzte Waschlösung am Ende des Tages weg. VIII. AUFBEWAHRUNG UND LAGERUNG DER REAGENZIEN Vor dem Öffnen oder der Rekonstitution sind die Reagenzien des Kits bis zum Ablaufdatum (Angaben auf Etikett) bei Lagerung bei 2 C bis 8 C stabil. Nicht verwendete Mikrotiterstreifen sollten bis zum Verfallsdatum dicht in der Folie verschlossen mit Trockenmittel bei 2 bis 8 C gelagert werden. Nach der Rekonstitution sind die Kalibratoren und Kontrollen bei 2 C bis 8 C 7 Tage stabil. Aliquots müssen bei längerer Aufbewahrung bei C eingefroren werden, dann sind Sie 3 Monate haltbar. Vermeiden Sie wiederholtes Einfrieren und Auftauen. Die konzentrierte Waschlösung ist bei Raumtemperatur bis zum Verfallsdatum haltbar. Frisch zubereitete Waschlösung sollte am selben Tag benutzt werden. Das gebrauchsfertige CPEPTIDEHRPKonjugat ist bei 28 C 18 Stunden lang stabil. Veränderungen im Aussehen der Kitkomponenten können ein Anzeichen für Instabilität oder Zerfall sein. IX. PROBENSAMMLUNG UND VORBEREITUNG Serumproben müssen bei 28 C aufbewahrt werden. Falls der Test nicht innerhalb von 24 Std. durchgeführt wird, ist die Aufbewahrung bei C erforderlich. Vermeiden Sie wiederholtes Einfrieren und Auftauen. Vor Gebrauch müssen alle Proben Raumtemperatur erreichen. Vortexmischen der Proben wird vor Gebrauch empfohlen. Keine hämolytischen Proben benutzen. X. DURCHFÜHRUNG A. Bemerkungen zur Durchführung Verwenden Sie den Kit oder dessen Komponenten nicht nach Ablaufdatum. Vermischen Sie Materialien von unterschiedlichen KitChargen nicht. Bringen Sie alle Reagenzien vor der Verwendung auf Raumtemperatur. Mischen Sie alle Reagenzien und Proben gründlich durch sanftes Schütteln oder Rühren. Führen Sie Kalibratoren, Kontrollen und Proben doppelt aus. Vertikale Ausrichtung wird empfohlen. Verwenden Sie zur Zubereitung der Waschlösung reinen Kunststoffbehälter. Verwenden Sie saubere Einwegpipettenspitzen, um Kreuzkontamination zu vermeiden. Verwenden Sie zur Pipettierung der Farblösung und der Stopplösung keine Pipetten mit Metallteilen. Präzisionspipetten oder ein automatisches Pipettiersystem erhöhen die Präzision. Achten Sie auf die Einhaltung der Inkubationszeiten. Attività biologica L'insulina viene sintetizzata nelle cellule beta delle isole di Langerhans come una molecola precursore, la proinsulina. 12 IV. PRINCIPIO DEL METODO BioSource CPEPEASIA è un immunosaggio a sensibilità amplificata a fase solida eseguito su piastre di microtitolazione. Una quantità definita di C PEPTIDE marcato con horseradish perossidasi (HRP) compete con il C PEPTIDE presente in calibratore e campioni per un numero definito di siti di un anticorpo. Dopo 2 ore di incubazione a temperatura ambiente, la piastra di microtitolazione viene lavata per arrestare la reazione di competizione. La soluzione chromogena (TMB H 2O 2) viene aggiunta e incubata. La reazione viene interrotta con l aggiunta di Soluzione di arresto; quindi la piastra di microtitolazione viene letta alla lunghezza d onda adeguata. Avvertenze generali Non usare il kit o suoi componenti oltre la data di scadenza. Non mescolare reattivi di lotti diversi. Prima dell uso portare tutti i reattivi a temperatura ambiente. Mescolare delicatamente i campioni per inversione o rotazione. Eseguire calibratori, controlli e campioni in doppio. Si raccomanda l allineamento verticale. Utilizzare un contenitore di plastica pulito per preparare la soluzione di lavaggio. Per evitare crosscontaminazioni, cambiare il puntale della pipetta ogni volta che si usi un nuovo reattivo o campione. L uso di pipette ben tarate e ripetibili o di sistemi di pipettamento automatici migliora la precisione del dosaggio. Rispettare i tempi di incubazione. Aspirare il liquido da ogni pozzetto. 7. Lavare la piastra 3 volte : versando 0,4 ml di soluzione di lavaggio in ogni pozzetto aspirando il contenuto di ogni pozzetto 8. Pipettare 100 µl di soluzione cromogena in ogni pozzetto entro 15 minuti dalla fase di lavaggio. 9. Actividades biológicas La insulina es sintetizada en las células beta de las islas de Langerhans como una molécula precursora, la proinsulina. En los gránulos secretorios de las células beta la proinsulina es escindida en insulina y en un peptido con 31 aminoácidos, el Peptido de Conexión o el PeptidoC. La insulina y el PeptidoC son segregados en cantidades equimolares. Sin embargo, la concentración en plasma del PeptidoC es más elevada que la concentración de la insulina por su mediavida más larga. La determinación del PeptidoC en plasma permite una evaluación de la producción de insulina endógena, incluso en caso de administración de insulina exógena o en caso de presencia de anticuerpos antiinsulina circulantes. Además, la determinación del PeptidoC en orina presenta un índice seguro para la producción de insulina si muestras sanguíneas se obtienen dificilmente o si una evaluación integrada de la secreción del PeptidoC durante un período de unas horas es necesária.
