May multiply and overgrow on the silicone material, which may lead to malfunction of the prosthesis. Clinician Medical professional or properly licensed speech & language therapist / pathologist who is trained in voice rehabilitation procedures. Esophagus Passage from the throat to the stomach. Fistula Opening between two cavities in the body. Here: TE-fistula. HME Heat and Moisture Exchanger. Device that retains warm moisture in the airways, which otherwise would be lost by breathing through a tracheostoma instead of the nose or mouth. Indwelling prosthesis Voice prosthesis that is prescribed and replaced by a clinician, not by the patient. 6

The part of the throat where the food passes and (if using a voice prosthesis) sound is created by vibration of tissue. Silicone A latex-free material often used in medical devices. Stoma An incised opening, which is kept open for drainage or other purposes. Here: Tracheostoma. TE-fistula Small opening between trachea and esophagus. TEP Tracheo-esophageal puncture: The procedure in which a small opening is created between trachea and esophagus, to allow air to flow into the pharynx to produce sound. Trachea Windpipe. Tracheostoma Breathing opening in the front neck, where the windpipe is connected to the skin. Voice prosthesis A one-way valve with retaining flanges inserted into a TE-fistula to allow speech while reducing the risk of aspiration. 7

8 Descriptive information Purpose of the Provox2 voice prosthesis (Indications for use) The Provox2 voice prosthesis is a device for voice rehabilitation after total laryngectomy. The Provox2 is a one-way valve that keeps a TE-fistula open for speech, but reduces the risk of liquid and food entering the trachea. The Provox2 is an indwelling prosthesis, which means it is inserted and replaced by a clinician (physician or licensed practitioner). Product description The Provox2 voice prosthesis [Fig.1] is a hinged valve with two retention collars [A], made of medical grade silicone rubber. A rigid blue ring [B] sits inside the prosthesis, adding stability and providing an even sealing surface for the valve flap [C]. The ring can also be seen in an x-ray. Accessories to the Provox2voice prosthesis are described below. When the Provox2 should not be used (contraindications) The first Provox2 prosthesis is inserted surgically. Therefore, the decision about its use should be made with your ENT surgeon. If you already wear a voice prosthesis of another brand, discuss the differences with your clinician. Normally there shouldn t be any restrictions to the alternative use of Provox2. Risks and benefits (to make an informed choice) Your surgeon will inform you about the risks and benefits of using a voice prosthesis. The Provox2 voice prosthesis is special with respect to the following: Provox2 is an indwelling prosthesis You will not be able to replace a defective valve yourself. Your clinician must do this for you. This means you may have to visit your clinician more often than if you had a non-indwelling prosthesis. However, the benefits of using a prosthesis like this are: a larger diameter facilitating airflow for voicing; the more stable flanges of Provox2 help to prevent accidental loss of the prosthesis; and Provox2 has no extra safety straps that need to be glued or taped to the skin. Additionally, the device life of an 8

9 indwelling prosthesis, like Provox2, may be much longer than that of a non-indwelling one. Furthermore, there are fewer requirements on your dexterity and visual acuity. If the prosthesis leaks, you can temporarily stop this by putting in the Provox Plug during eating and drinking. You cannot take out the prosthesis for cleaning, but you can clean the inside of the prosthesis with the Provox Brush (and optionally with the Provox Flush) while it is still in place. That means you need to use a mirror or have help from others, but you do not need to take out the whole voice prosthesis, dilate the fistula, clean the prosthesis and insert it again all by yourself. Provox2 is suitable for bi-directional insertion Normally, the Provox2 voice prosthesis is inserted through the tracheostoma. If your clinician cannot reach the fistula from the stoma, or if insertion from the front will not succeed, your clinician can use a special guidewire and insert the prosthesis backward, i.e. let you swallow the prosthesis. How to get a Provox2 inserted and what comes thereafter You can have a Provox2 prosthesis inserted in one of the following ways: Primary insertion. During the surgical removal of the larynx, the tracheo-esophageal puncture is done and a voice prosthesis is fitted immediately. Depending on your physicians order, you can usually start with simple speech training after wound healing (about days after the operation). Primary insertion with temporary stenting. The puncture is done during the larynx surgery, and the surgeon puts in a placeholder or leads the gastric feeding tube through the fistula. You will receive the voice prosthesis at a later date on your physician s order, and normally speech training can start at that time. Secondary insertion. A separate puncture surgery is done at a later date, after your larynx removal. The prosthesis can then be fitted immediately. Some surgeons may choose to put in a spacer and insert the actual prosthesis at a later date. After insertion of the prosthesis, speech training can be normally started the same day. 9

10 Replacement of another voice prosthesis. If you have another type of voice prosthesis, the shaft diameter of the prosthesis determines whether you can receive your new Provox2 prosthesis immediately or if your fistula needs to be dilated. In the latter case it might be necessary for your clinician to use a special instrument to dilate the fistula so that it is large enough for Provox2. The Provox2 is not a permanent implant, and needs periodic replacement. After the prosthesis has been fitted, there should be no leakage while drinking. Occasionally, mild leakage through or around the prosthesis may occur in the first weeks after introduction of a new prosthesis. This often resolves spontaneously and does not require immediate replacement of the prosthesis. For a short time after a prosthesis replacement traces of blood may be found in the sputum. This may result from micro bleedings of the mucous membrane around the fistula. General warnings and precautions WARNINGS: Accidental swallowing of the Provox2 voice prosthesis may occur. If this occurs, contact your physician who will advise you further. Accidental aspiration of the Provox2 voice prosthesis or other components of the Provox2 voice rehabilitation system may occur. Immediate symptoms may include gagging, coughing, choking or wheezing. If this occurs seek immediate medical treatment. A foreign body in your airways may cause severe complications and has to be removed by a physician. Re-use and re-processing may cause crosscontamination and damage to the device, which could cause patient harm. PRECAUTIONS: Consult your clinician if: - There is leakage through or around the prosthesis (coughing). 10

11 - Speaking becomes difficult (higher effort). - There are any signs of inflammation or tissue changes at the fistula tract or the stoma region (pain, heat, swelling). Operating information How to use Provox2 for speaking Optimal speech quality depends on proper closure of the tracheostoma with a finger, preferably from your non-dominant hand, i.e. your left hand if you are right-handed or vice versa. Avoid leakage of air between your finger and the edges of the stoma. The use of a Heat and Moisture Exchanger, like the Provox HME (see below the chapter What related products may be used ), facilitates voicing. Results are often improved by cooperative efforts between the physician, the speech therapist and you. [Fig 2] How to clean the prosthesis Regular cleaning of the prosthesis helps to prolong device life. Clean the prosthesis at least twice a day and after each meal with the Provox Brush. For Provox2 voice prostheses longer than 10mm, use the Provox Brush XL. Insert the brush into the prosthesis and move it gently back and forth with a twisting movement. Remove the brush, clean with gauze, and repeat the procedure until the prosthesis is clean. If the prosthesis moves outwards during cleaning, it may be held in place with a finger. For details and how to clean the brush see Instructions for use that accompanies the Provox Brush. The brush can be used up to one month (maximum). In addition to using the Provox Brush, you may also use the Provox Flush. The Provox Flush can be used with drinking water or air. It allows you to flush drinking water or air with some pressure through the lumen of the prosthesis. For details and how to clean the Provox Flush. See Instructions for Use that accompanies the Provox Flush. In addition to using the Provox Brush, you may also use the Provox Flush. The Provox Flush can be used with drinking water or air. See Instructions for Use that accompanies the Provox Flush. Because of the wide lumen of the shaft of the prosthesis, 11

12 you may also be able to clean the prosthesis by forced expectoration with the stoma closed, i.e. closing the stoma with a finger and pushing air forcefully to your throat. Note: No instruments other than the Provox Brush and Flush should be used, when cleaning the prosthesis, since they may damage the valve function! [Fig 3] Cleaning and disinfection of the accessories The accessories should be cleaned after each use and disinfected at least once a day according to their Instructions for Use. Who replaces the Provox2 if needed? Replacement of the device is carried out by a physician or licensed practitioner who may, in his or her independent medical judgment and in accordance with the law of the state in which he or she practices, consider delegating the replacement of the prosthesis to a clinician trained in the procedure. How long does the Provox2 prosthesis last and how to know that it needs to be replaced? The prosthesis is not a permanent implant, and needs periodic replacement. The most frequent indication for replacement of the Provox2 voice prosthesis is leakage of fluid through the valve. You may notice drops of your drinking liquids entering the windpipe, making you cough. The main reason for this leakage through the valve is that the prosthesis is overgrown by candida yeasts, preventing the valve flap from closing properly. It is normal that candida yeasts develop over time on foreign bodies in the mouth, throat or esophagus. If the device life becomes too short, medical anti-candida treatment could be considered, but has to be done as preventive action. Once candida has grown on the prosthesis, you cannot remove it. Some dietary measures, like the daily intake of yogurt and skim milk containing natural milk bacteria, can be helpful against excessive candida growth. The average device life differs from individual to individual and from country to country. Depending on the biological circumstances, the device life can vary between a few days to several years. 12

13 What accessories exist and how they are used The Provox Brush [Fig 3] is used to clean the inside of the prosthesis, while the prosthesis remains in the TEP fistula. One Provox Brush comes with the prosthesis. See Cleaning instructions above. The Provox Flush [Fig 4] is an additional and optional cleaning device that offers greater comfort. It allows you to flush drinking water or air with some pressure through the lumen of the prosthesis. This helps clean the prosthesis from debris and also flushes mucus away from the esophagus. The Provox Flush is available separately. Order size: 1 piece, non-sterile, reusable. Read instructions. The Provox Plug [Fig 5] is a First Aid tool that allows you to temporarily stop leakage through the prosthesis. If you notice that your Provox2 voice prosthesis is starting to leak, insert the Provox Plug into the opening of the Provox prosthesis with help of the smooth shaft end of a Provox Brush. Bending the shaft of the brush a little sometimes facilitates the insertion of the plug. Speaking is not possible while the Provox Plug is in place, but it is after you remove the Plug by pulling on the safety strap. The Provox Plug is available separately. Order size: 1 Provox Plug and 1 Provox Brush. What related products may be used Provox HME [Fig 6]: Heat- and Moisture Exchanger can partially restore lost nasal functions (air heating, humidifying, breathing resistance). Nasal functions are lost after total laryngectomy because air is inhaled and exhaled through the stoma in the neck, totally bypassing the upper airways (the nose and the throat). The lack of nasal functions is one reason for increased mucus production, frequent coughing, fatigue, sleeping problems and anxiety. Provox HME is especially designed for use after total laryngectomy. Depending on your skin condition, it can be attached to the stoma by means of different Provox Adhesive base plates [A] (self-adherent) or, if needed, by a Provox LaryTube (see further below). The core part of the Provox HME is the HME cassette [B], which is inserted into the base plate. The HME cassette makes it easier to close the stoma with your finger for speech. You press the top lid on the cassette to close the stoma, and a spring in 13

14 the cassette opens it again immediately after you release the button. In addition to partially restoring lost nasal functions, Provox HME may also improve speech by increasing dynamic range (loudness range) and / or maximum phonation time (time you can speak on a single breath). Provox FreeHands HME [Fig 7]: A Heatand Moisture Exchanger (see above) combined with an automatic tracheostoma valve, is designed to allow hands-free speech. Please ask your clinician for more information. Provox LaryTube [Fig 8]: Soft silicone tracheal cannula. If your physician prescribes a cannula for you because your stoma tends to shrink, he/she may prescribe a Provox LaryTube. Unlike most other tracheal cannula, Provox LaryTube has a special retainer/ adapter for Provox HME cassettes. This may allow you to use the Provox HME (see above) even if you cannot wear an adhesive base plate. The finger-operated button of the Provox HME works identically if used with the Provox LaryTube. Since this product is only available by prescription, your physician must choose the appropriate Provox LaryTube for you. Troubleshooting information Symptom: Coughing (triggered by drinking or eating) while the prosthesis is in place. Most common reasons: Leakage through or around the valve. Measures: 1. Clean the prosthesis with the Provox Brush and/or the Provox Flush. There may be food remnants preventing the valve from proper closure. If that does not help: 2. Close the prosthesis with the Provox Plug (see above Instructions on Accessories). If the leakage stops, the prosthesis needs to be replaced. While eating and drinking use the 14

15 Provox Plug to prevent leakage. Notify your clinician that you need a new prosthesis. If the problem persists, even with the Plug in place: 3. You may have some leakage around the prosthesis. Notify your clinician immediately. Symptom: Speaking becomes more and more difficult. Most common reasons: Valve obstruction, tissue swelling in the throat (e.g. during radiotherapy) Measures: 1. Clean the prosthesis with the Provox Brush and the Provox Flush. There may be food remnants increasing the airflow resistance. If that does not help: 2. Notify your clinician. Symptoms: Pain, heat, swelling, and rash (together or alone) in the area of the fistula or stoma. Most common reasons: Tissue inflammation and infections Measures: Notify your clinician immediately. Additional information Clinical studies Information about relevant clinical studies, both on the Provox voice prosthesis and on Provox HME, are available upon request from our customer service department. Disease and self-care information Speech Therapy under guidance of a trained clinician is highly advisable to acquire fluent speech. Product information on the Internet: Adverse events Adverse events may include dysfunction of the valve, tissue reactions around the prosthesis and loss of the prosthesis (see Warnings). In case of an adverse event your clinician has to determine the appropriate treatment. 15

16 Travel or international use Provox voice prostheses are used in many countries worldwide. However, not all medical professionals have detailed knowledge about the Provox system, or the prosthesis may not be available in a specific territory. Before traveling, you should ask your clinician whether he/she can recommend some professional in the destination area. For back-up purposes you should at least have your Provox Plug with you. Silikon Ein latexfreies Material, das häufig für Medizinprodukte verwendet wird. Stoma Eine operativ angelegte Öffnung, welche dauerhaft offen gehalten wird. Hier: Tracheostoma. Tracheo-ösophageale Fistel Kleine Verbindung zwischen Trachea und Ösophagus. Tracheo-ösophageale Punktion Das Verfahren, bei dem eine kleine Öffnung zwischen Luftröhre und Speiseröhre angelegt wird, die den Luftdurchfluss in den Rachen und somit die Stimmbildung ermöglicht. Trachea Luftröhre. Tracheostoma Atemöffnung im vorderen Halsbereich, wo bei Laryngektomierten die Luftröhre mündet. 19 Beschreibende Informationen Bestimmung der Provox2 Prothese (Indikationen) Die Provox2 Stimmprothese dient Wiederherstellung der Stimme nach totaler Laryngektomie. Die Provox2 ist ein Ventil, welches die tracheo-ösophageale Fistel zur Stimmbildung offen hält, jedoch das Risiko, dass Flüssigkeiten oder Nahrung in die Luftröhre gerät, minimiert. Die Provox2 ist eine Verweilprothese, d.h. sie wird durch einen Arzt eingesetzt und ausgewechselt. Produktbeschreibung Die Provox2 Stimmprothese [Abb. 1] ist eigentlich ein Ventil mit zwei Halteflanschen [A]. Sie besteht aus medizinischem Silikon. Ein fester blauer Ring [B] befindet sich an der Innenseite der Prothese. Er dient der Stabilität und stellt eine ebene, gut abdichtende Oberfläche für die Ventilklappe zur Verfügung [C]. Der Ring ist auch auf einem Röntgenbild sichtbar. Das Zubehör für die Provox2 Stimmprothese wird unten ausführlich beschrieben. Wann die Prothese nicht zum Einsatz kommen sollte (Kontraindikationen) Die erste Provox2 Prothese wird im Rahmen einer Operation eingesetzt. Daher sollte die Entscheidung über deren Einsatz zusammen mit dem HNO-Chirurgen getroffen werden. Wenn Sie bereits eine Stimmprothese eines anderen Herstellers tragen, besprechen Sie die Unterschiede mit Ihrem Arzt. Normalerweise gibt es keine Gründe, die verhindern, dass stattdessen eine Provox2 Prothese eingesetzt wird. Risiken und Vorteile Ihr HNO-Arzt wird Sie über die Risiken und Vorteile einer Stimmprothese informieren.die Provox2 Stimmprothese hat folgende spezielle Eigenschaften: Provox2 ist eine Verweilprothese Ein defektes Ventil können Sie nicht selbst ersetzen. Dies muss der Arzt tun. Das bedeutet, dass Sie Ihren Arzt evtl. häufiger aufsuchen müssen als mit einer Wechselprothese. Die Vorteile einer Verweilprothese bestehen 19

20 darin, dass ein größerer Durchmesser den Luftdurchfluss zur Stimmbildung erleichtert. Weiterhin mindern die stabileren Flansche der Provox2 Prothese deutlich das Risiko, die Prothese versehentlich zu verlieren. Provox2 hat keinen Sicherungsfaden, der an der Haut befestigt werden muss. Außerdem kann die Lebensdauer einer Verweilprothese deutlich länger sein als die einer Wechselprothese und es werden nicht so hohe Anforderungen an die Geschicklichkeit und Sehfähigkeit des Patienten gestellt. Falls die Prothese undicht ist, kann zur vorübergehenden Abhilfe während der Nahrungsaufnahme der Provox Plug (Stopfen) eingesetzt werden. Die Prothese wird zur Reinigung nicht herausgenommen; sie verbleibt in der Fistel. Die Protheseninnenseite wird mit der Provox Brush (Reinigungsbürste) und zusätzlich nach Wahl mit dem Provox Flush gereinigt. Dafür benötigen Sie einen Spiegel oder es muss Ihnen jemand behilflich sein, aber Sie müssen die Prothese nicht - wie bei einigen anderen Prothesenmodellen - selbst herausnehmen, die Fistel aufweiten, die Prothese reinigen, um sie dann wieder einzusetzen. Provox2 ist für bi-direktionales Einführen geeignet (durch den Mund oder durch das Stoma) Normalerweise wird die Provox2 Stimmprothese durch das Tracheostoma eingeführt. Falls Ihr Arzt die Fistel vom Stoma aus nicht erreichen kann oder wenn das Einführen von vorne nicht gelingt, kann der Arzt einen speziellen Führungsdraht benutzen und die Prothese von hinten einführen, d.h. Sie müssen die Prothese schlucken. Einsetzen der Provox2 und Nachsorge Eine Provox2 Prothese kann auf folgende Weise implantiert werden: Primäreinsatz: Während der chirurgischen Entfernung des Kehlkopfes wird eine kleine Verbindung zwischen Luftröhre und Speiseröhre angelegt (tracheo-ösophageale Punktion). In diese Fistel wird sofort eine Stimmprothese eingesetzt. Nach Maßgabe des Arztes können Sie nach Abheilung der Wunde (ca Tage nach der Operation), mit einfachen Übungen zur Stimmbildung beginnen. Primäreinsatz mit vorübergehender Schienung: Auch in diesem Fall wird die 20

21 Punktion während der Kehlkopfoperation vorgenommen, aber der Arzt setzt nach der Punktion einen Platzhalter in die Fistel ein oder lässt die Ernährungssonde hindurch laufen. Die Stimmprothese erhalten Sie, gemäß ärztlicher Entscheidung, später. Dann kann auch mit den Stimmübungen begonnen werden. Sekundäreinsatz: In diesem Fall findet nach der Kehlkopfentfernung noch eine zweite Operation statt, während derer die Fistel angelegt wird. Dabei wird die Prothese sofort eingesetzt. Einige Ärzte setzen möglicherweise zunächst einen Platzhalter ein und die Prothese dann zu einem späteren Zeitpunkt. Nach Einsatz der Prothese kann in der Regel noch am gleichen Tag begonnen werden. Ersetzen einer Stimmprothese eines anderen Herstellers: Falls Sie eine andere Stimmprothese haben, ist der Durchmesser dieser Prothese ausschlaggebend dafür, ob Sie Ihre neue Provox 2 Stimmprothese sofort bekommen können, oder ob erst die Fistel geweitet werden muss. Im letzteren Fall ist es möglich, dass Ihr Arzt ein spezielles Instrument benutzt, um die Fistel so zu weiten, dass sie groß genug für die Provox 2 Prothese ist. Bei der Provox2 handelt es sich nicht um ein Dauerimplantat. Sie muss bei Bedarf gewechselt werden. Nach dem Einsetzen der Prothese sollte beim Trinken keine Undichtigkeit auftreten. Hin und wieder kann in den ersten Wochen nach dem Einsetzen einer neuen Prothese eine leichte Undichtigkeit durch die Prothese oder um die Prothese herum auftreten. Dies hört meist von allein auf und es ist nicht notwendig, die Prothese sofort auszuwechseln. Nach dem Einsetzen der Prothese kann sich für kurze Zeit etwas Blut im Hustensekret befinden, welches von minimalen Blutungen der Schleimhaut um die Fistel herrühren kann. Warninweise / Vorsichtsmassnahmen WARNINWEISE: Es kann zu versehentlichem Verschlucken der Provox2 Stimmprothese kommen. In diesem Fall setzen Sie sich bitte mit Ihrem Arzt in Verbindung, der über die weitere 21

22 Vorgehensweise entscheidet. Es kann zu versehentlichem Einatmen (Aspiration) der Provox2 Stimmprothese oder anderen Bestandteilen des Provox Stimmrehabilitationssystems kommen. Unmittelbare Symptome dafür können Würgen, Husten, Erstickungsanfälle oder keuchende Atemgeräusche sein. In diesem Fall müssen Sie sofort medizinische Hilfe in Anspruch nehmen. Ein Fremdkörper in den Atemwegen kann zu schweren Komplikationen führen und muss vom Arzt entfernt werden. Wiederverwendung und Wiederaufbereitung können eine Kreuzkontamination und eine Beschädigung des Produktes verursachen, was für den Patienten negative Folgen haben könnte. VORSICHTMASSNAHMEN: Suchen Sie Ihren Arzt auf, wenn: - Undichtigkeit durch oder um die Prothese herum auftritt (Symptom: Husten) - größere Anstrengungen zum Sprechen nötig sind - wenn Zeichen einer Entzündung oder sonstiger Gewebeveränderungen an der Fistel oder dem Stoma bestehen (Schmerzen, Hitzegefühl, Schwellung). Informationen zur Anwendung Sprechen mit der Provox2 Um eine optimale Stimmqualität zu erreichen, muss das Tracheostoma dicht mit dem Finger verschlossen werden, vorzugsweise von der nicht-dominanten Hand, d.h. von Ihrer linken Hand wenn Sie Rechtshänder sind bzw. umgekehrt. Vermeiden Sie das Austreten von Luft zwischen dem Finger und dem Rand des Stomas. Die Benutzung eines HME (wie z.b. dem Provox HME) erleichtert die Stimmbildung. Die Stimme kann häufig verbessert werden, wenn Arzt, Logopäde und Sie zusammen arbeiten. [Abb. 3] Reinigung der Prothese Regelmäßige Reinigung der Prothese trägt zu längerer Haltbarkeit bei. Die Prothese mindestens zweimal täglich sowie nach jeder Mahlzeit mit der Provox Brush 22

23 (Bürste) reinigen. Benutzen Sie für Provox2 Stimmprothesen der Größe 12,5 und 15 mm die Provox XL. Führen Sie die Bürste in die Prothese ein, bewegen Sie sie vorsichtig vor und zurück und drehen Sie sie dabei zwischen den Fingern. Die Bürste entfernen und mit einer Gazekompresse abwischen. Diesen Vorgang wiederholen, bis die Prothese sauber ist. Falls sich die Prothese beim Reinigen etwas nach außen bewegt, kann Sie mit dem Finger fest gehalten werden. Nähere Einzelheiten und Angaben zur Reinigung der Bürste finden sich in der Bedienungsanleitung für die Provox Brush (Bürste). Die Bürste kann bis zu einen Monat lang benutzt werden (Maximum). Zusätzlich zur Provox Brush (Bürste) kann auch der Provox Flush benutzt werden. Der Provox Flush kann mit Trinkwasser oder Luft verwendet werden. Damit kann Trinkwasser oder Luft unter einem gewissen Druck durch das Lumen der Prothese gespült werden. Nähere Einzelheiten und Angaben zur Reinigung des Provox Flush finden sich in der Bedienungsanleitung für den Provox Flush. Wegen der weiten Öffnung des Ventils der Prothese kann diese auch durch kräftiges Husten bei geschlossenem Stoma gereinigt werden. d.h. Sie schließen das Stoma mit dem Finger und drücken Luft durch den Rachen. Achtung: Zur Reinigung der Prothese dürfen nur Provox Brush (Bürste) und Provox Flush benutzt werden. Andere Instrumente können die Ventilfunktion beschädigen! [Abb. 2] Reinigung und Desinfektion des Zubehörs Das Zubehör ist nach jedem Gebrauch zu reinigen und mindestens einmal täglich gemäß der Bedienungsanleitung zu desinfizieren. Wer wechselt bei Bedarf die Prothese? Der Wechsel der Stimmprothese wird vom Arzt oder von entsprechend ausgebildetem medizinischem Personal vorgenommen, das mit der Methode vertraut ist und vom Arzt dazu ermächtigt wurde. Wie lange hält die Provox2 Prothese und wann muss sie ersetzt werden? Die Prothese ist kein Dauerimplantat, sondern muss bei Bedarf ausgewechselt werden. Der häufigste Grund für den Wechsel der Provox2 23

24 Stimmprothese ist ein Durchsickern von Flüssigkeit durch die Prothese. In diesem Fall bemerken Sie, dass Tropfen der Trinkflüssigkeit in die Luftröhre gelangen, was einen Hustenreiz hervorruft. Der Hauptgrund für eine solche Undichtigkeit ist, dass die Prothese von Candida überwachsen ist, was dazu führt, dass die Ventilklappe nicht mehr vollständig schließt. Es ist normal, dass Fremdkörper (wie z.b. eine Stimmprothese), die in Mund, Kehle oder Speiseröhre sind, von Candida überwachsen werden. Falls die Lebenszeit der Prothese zu kurz ist, kann eine medikamentöse Anti-Candida-Behandlung in Erwägung gezogen werden. Diese muss jedoch präventiv geschehen, d.h. bevor sich der Candida-Pilz ausbreitet. Wenn sich einmal Candida auf der Prothese entwickelt hat, kann er nicht mehr entfernt werden. Einige Ernährungsgewohnheiten, wie z.b. täglicher Konsum von Joghurt oder fettarmer Milch, die natürliche Milchbakterien enthalten, können gegen übermäßigen Candidabewuchs hilfreich sein. Die durchschnittliche Lebensdauer der Prothese ist bei jedem Menschen sowie auch in verschiedenen Ländern unterschiedlich lang. Abhängig von den biologischen Verhältnissen kann die Lebensdauer der Prothese einige Tage oder einige Jahre betragen. Zubehör und dessen Anwendung Die Provox Brush (Bürste) [Abb. 3] wird zur Reinigung der Innenseite der Prothese benutzt, wobei die Prothese in der tracheo-ösophagealen Fistel verbleibt. Dabei verbleibt die Prothese in der tracheo-ösophagealen Fistel. Eine Provox Brush liegt jeder Stimmprothese bei. Beachten Sie die Reinigungshinweise oben sowie die dem Produkt beiliegenden Anwendungshinweise. Der Provox Flush [Abb. 4] ist ein zusätzliches, optionales Reinigungsgerät, welches noch größeren Komfort bietet. Damit kann Trinkwasser oder Luft unter einem gewissen Druck durch das Lumen der Prothese gespült werden. Der Provox Flush ist separat erhältlich. Bestellhinweis: 1 Stk., nicht steril, wiederverwendbar. Beachten Sie die dem Produkt beiliegenden Anwendungshinweise. Der Provox Plug (Stopfen) [Abb. 5] ist ein Hilfsmittel zur vorübergehenden Abhilfe bei Undichtigkeit der Prothese. Falls Sie bemerken, dass Ihre Provox2 Prothese anfängt, undicht 24

25 zu werden, führen Sie den Provox Plug mittels des glatten Stiels der Provox Brush in die Öffnung der Provox Prothese ein. Durch leichtes Biegen des Bürstenstiels kann das Einführen des Stopfens erleichtert werden. Während der Provox Plug sich in der Stimmprothese befindet, können Sie nicht sprechen. Wenn Sie ihn herausziehen, ist das Sprechen aber wieder möglich. Der Provox Plug ist separat erhältlich. Bestellhinweis: 1 Provox Plug und 1 Provox Brush (Bürste). Weitere Produkte aus dem Provox-System Provox HME [Abb. 6]: Wärme- und Feuchtigkeitstauscher (HME) können die verlorene Nasenfunktion (Erwärmen, Befeuchten, Atemwiderstand) teilweise wieder herstellen. Nach der Kehlkopfentfernung sind die Nasenfunktionen nicht mehr vorhanden, da die Luft nunmehr direkt unter Umgehung der oberen Luftwege (Nase und Rachen) durch das Stoma im Hals eingeatmet und wieder ausgeatmet wird. Die fehlende Nasenfunktion ist einer der Gründe dafür, dass verstärkt Sekret produziert wird und häufiger Hustenreiz auftritt. Weitere Symptome sind Müdigkeit, Schlafprobleme und Angstgefühle. Provox HME wurden speziell für die Benutzung nach totaler Laryngektomie entwickelt. Abhängig von der Hautbeschaffenheit können Provox HME mittels unterschiedlicher selbstklebender Provox Adhesive Pflaster [A] direkt an der Haut befestigt werden, oder falls notwendig in eine Provox LaryTube (siehe unten) eingesetzt werden. Kern der Provo HME ist die HME Cassette [B], welche in das Pflaster eingesetzt wird. Die HME Cassette erleichtert den Stomaverschluss mit dem Finger. Zum Sprechen drücken Sie einfach auf die Kassette, die sich dadurch zum Sprechen verschließt. Wenn Sie die Kassette wieder los lassen, öffnet Sie sich sofort wieder. Provox HME können zusätzlich zur Wiederherstellung der Nasenfunktion auch die Stimme verbessern, indem die Lautstärke vergrößert wird und/oder die Tonhaltedauer, d.h. die Zeit, in der Sie mit einem einzigen Atemzug sprechen können, verlängert wird. 26 Kataloge zusendet. Provox FreeHands HME [Abb. 7]: Ein automatisches Sprechventil, welches mit einem Wärme- und Feuchtigkeitstauscher (HME) kombiniert wurde. FreeHands ermöglicht Ihnen fingerfreies Sprechen. d.h. Sie müssen zum Sprechen das Stoma nicht mehr mit dem Finger zuhalten. Weitere Informationen erhalten Sie von Ihrem Arzt oder direkt von unserem Kundenservice (siehe unten), der Ihnen gern auch entsprechende Kataloge zusendet. Provox LaryTube [Abb. 8]: Weiche Trachealkanüle aus Silikon. Wenn Ihnen Ihr Arzt eine Kanüle verschreibt, weil Ihr Stoma schrumpfen könnte, kann er Ihnen eine Provox LaryTube verschreiben. Anders als die meisten anderen Trachealkanülen, kann die Provox LaryTube eine Provox HME Cassette direkt aufnehmen. So können Sie die Provox HME (siehe oben) benutzen, selbst wenn Sie kein Pflaster tragen können. Genau wie beim Pflaster wird die Kassette, auch wenn sie in der LaryTube sitzt, mit dem Finger verschlossen. Ihr behandelnder Arzt sucht die für Sie richtige Provox LaryTube aus und verordnet Sie Ihnen. Problembeseitigung Symptom: Hustenreiz (ausgelöst beim Essen oder Trinken). Häufigste Ursachen: Undichtigkeit durch die Prothese oder an der Prothese vorbei. Maßnahmen: 1. Reinigen Sie die Prothese mit der Provox Brush und/oder dem Provox Flush. Es könnten kleine Nahrungsmittelbestandteile in die Prothese gelangt sein, die dazu führen, dass das Ventil nicht mehr richtig schließt. Falls dadurch keine Besserung erzielt wird: 2. Verschließen Sie die Prothese mit dem Provox Plug (siehe oben). Sollte die Undichtigkeit damit beseitigt sein, muss die Prothese gewechselt werden. Benutzen Sie während des Essens und Trinkens vorübergehend den Provox Plug, um die Undichtigkeit zu beseitigen. Setzen Sie Ihren Arzt davon in Kenntnis, dass Sie eine neue Prothese benötigen. Wenn das Problem weiter besteht, selbst wenn der Provox Plug in der Prothese ist: 26

27 3. Es besteht eine Undichtigkeit an der Prothese vorbei. d.h. die Flüssigkeit kann zwischen Prothese und Gewebe in die Luftröhre fließen. in der Umgebung der Prothese. Setzen Sie Ihren Arzt davon sofort in Kenntnis. Symptom: Das Sprechen wird immer schwieriger. Häufigste Ursachen: Die Stimmprothese ist verstopft oder das Gewebe ist angeschwollen (z.b. durch Bestrahlungen). Maßnahmen: 1.Reinigen Sie die Prothese mit der Provox Brush und dem Provox Flush. Nahrungsmittelreste können den Luftdurchfluss behindern. Falls das nicht hilft: 2.Benachrichtigen Sie Ihren Arzt. Symptome (einzeln oder gemeinsam auftretend): Schmerzen, Hitzegefühl, Hautrötungen in der Umgebung der Fistel oder des Stomas. Häufigste Ursachen: Entzündungen des Gewebes und Infektionen. Maßnahmen: Benachrichtigen Sie umgehend Ihren Arzt. Zusätzliche Informationen Klinische Studien Informationen über relevante klinische Studien sowohl über die Provox Stimm-prothese als auch über Provox HME können Sie von unserem Kundenservice anfordern. Informationen über die Krankheit und Selbsthilfe Eine Stimmtherapie zusammen mit einem entsprechend ausgebildeten Therapeuten (Logopäde) ist sehr zu empfehlen, damit eine flüssige Stimme erlernt wird. Produktinformationen im Internet: Komplikationen Komplikationen können sein: Fehlfunktion des Ventils, Gewebereaktionen in der Umgebung der Prothese, Verlust der Prothese (siehe Warnhinweise). Im Falle einer Komplikation muss Ihr Arzt über eine angemessene Behandlungsweise entscheiden. 27

28 Hinweise für unterwegs (auch für Auslandsreisen) Provox Stimmprothesen werden weltweit in vielen Ländern eingesetzt. Dennoch kann nicht jeder einzelne Arzt detailliert über das Provox-System informiert sein. In bestimmten Ländern kann es vorkommen, dass keine Stimmprothese erhältlich ist. Vor einer Reise sollten Sie Ihren behandelnden Arzt fragen, ob er einen Kollegen in der entsprechenden Gegend empfehlen kann. 31 Informazione Descrittiva Indicazioni d Uso (Scopo della protesi vocale Provox2) La protesi vocale Provox2 é un dispositivo per la riabilitazione vocale dopo un intervento di laringectomia totale. La Provox2 é una valvola unidirezionale che mantiene aperta la fistola TE per consentire al paziente di parlare, riducendo il rischio che liquidi e cibo entrino nella trachea. La Provox2 é una protesi fissa; questo significa che deve essere inserita e sostituita da un dottore (medico professionista o logopedista). Descrizione del Prodotto La protesi vocale Provox2 [Fig. 1] é una valvola a cerniera dotata di due flange di trattenimento [A], in gomma di silicone di grado medicale. Un anello rigido di colore blu [B] posizionato dentro la protesi, ne aumenta la stabilità e fornisce una superficie di chiusura liscia per lo sportellino che funge da valvola [C]. L anello può anche essere rilevato in una radiografia. Gli accessori per la protesi vocale Provox2 saranno descritti di seguito. Controindicazioni (Quando non si dovrebbe utilizzare la Provox2) La prima protesi vocale Provox2 si inserisce chirurgicamente. Pertanto, la decisione circa il suo impiego dovrebbe essere presa assieme al proprio medico chirurgo. Se si utilizza già una protesi di altra marca, parlare delle differenze con il vostro medico. Di norma, non dovrebbero esserci restrizioni per l utilizzo alternativo della Provox2. Rischi e Benefici (Per fare una scelta informata) Il vostro medico chirurgo vi informerà sui rischi e benefici di utilizzo di una protesi vocale. La protesi vocale Provox2 é una protesi speciale per i seguenti motivi: Provox2 é una protesi fissa Una valvola difettosa non potrà essere sostituita dal paziente. La sostituzione deve essere eseguita dal vostro medico. Questo significa che potrebbero essere necessarie delle visite più frequenti al vostro medico, rispetto ad una protesi mobile. Tuttavia, i benefici derivanti dall uso di una protesi come la Provox2 sono: 31

32 un diametro più grande che facilita il passaggio di un maggior flusso d aria per la fonazione; le flange più stabili della Provox2 aiutano a prevenire la perdita accidentale della protesi; la Provox2 non ha linguette di sicurezza da incollare o fissare alla pelle. Inoltre, la vita di una protesi fissa, come la Provox2, può essere molto più lunga di quella di una protesi mobile, richiedendo inoltre minor abilità manuale ed acuità visiva al paziente nella manutenzione. Se la protesi perde liquidi, tale perdita può essere temporaneamente bloccata inserendo il Provox Plug mentre si mangia e si beve. La protesi non può essere rimossa per la pulizia, ma è possibile pulirne la parte interna con lo Provox Brush (Spazzolino Provox) (o in alternativa, con il Provox Flush), mentre é ancora sistemata in sede. Tale operazione può richiedere l uso di uno specchio o un aiuto esterno, ma non sarà necessario togliere l intera protesi vocale, dilatare la fistola, pulire la protesi e reinserirla nuovamente, tutto da soli. Provox2 é adatta ad un inserimento bidirezionale Di norma la protesi vocale Provox2 si inserisce attraverso il tracheostoma. Nel caso in cui il medico non possa raggiungere la fistola dallo stoma, o l inserimento frontale non abbia successo, il vostro medico potrà utilizzare un filo guida speciale ed inserire la protesi dall interno, in altre parole, facendola come inghiottire. Come si inserisce una Provox2 e cosa accade dopo Una protesi Provox2 può essere inserita in uno dei seguenti modi: Inserimento primario. Durante l intervento chirurgico di rimozione della laringe, si effettua una puntura tracheoesofagea e si posiziona subito la protesi vocale. Secondo il giudizio del vostro medico, si potrà iniziare una semplice riabilitazione vocale dopo la guarigione della ferita (circa giorni dall intervento). Inserimento primario con temporaneo stabilizzatore. Si esegue la puntura durante l intervento di rimozione della laringe, e il chirurgo posiziona un dilatatore o un catetere naso-gastrico nella fistola. La protesi vocale sarà inserita in un momento successivo, 32

33 secondo il parere del vostro medico, e la normale riabilitazione vocale potrà iniziare da quel momento. Inserimento secondario. La puntura tracheoesofagea è realizzata separatamente, durante un intervento chirurgico successivo a quello di rimozione della laringe. La protesi potrà, in quel momento, essere immediatamente impiantata. Alcuni chirurghi possono scegliere di posizionare un dilatatore e di inserire la protesi successivamente. Dopo l inserimento, si potrà normalmente iniziare la riabilitazione vocale a partire dallo stesso giorno. Sostituzione di un altra protesi vocale. Se si possiede un altro tipo di protesi vocale, il suo diametro determina se sia possibile inserire immediatamente la nuova protesi Provox2 o se occorra dilatare la fistola. In quest ultimo caso il medico potrebbe avvalersi di uno speciale strumento per la dilatazione della fistola, in modo da ingrandirla sufficientemente per la Provox2. La Provox2 non é una protesi permanente, e richiede una sostituzione periodica. Dopo l inserimento della protesi, non dovrebbe avvenire alcuna fuoriuscita di liquidi mentre si beve. Occasionalmente, si può verificare una lieve perdita attraverso o attorno la protesi durante le prime settimane dal posizionamento, che spesso si risolve spontaneamente e non richiede alcuna sostituzione immediata della protesi. Per un breve periodo di tempo dalla sostituzione della protesi, si potrebbero trovare tracce di sangue nell espettorato, dovute probabilmente a piccoli sanguinamenti della membrana mucosa attorno alla fistola. Avvertenze e precauzioni generali AVVERTENZE: In caso di ingestione accidentale della protesi vocale Provox2, contattare il vostro medico per ulteriori consigli. In caso di aspirazione accidentale della protesi vocale Provox2 o di altri componenti del sistema di riabilitazione vocale Provox, i sintomi immediati possono includere: senso di soffocamento, tosse e respiro affannoso. 33

34 Se ciò avviene, consultare immediatamente un medico. Un corpo estraneo nelle vie respiratorie può causare gravi complicazioni e deve essere rimosso da un medico. Il riutilizzo e il ricondizionamento possono causare la contaminazione incrociata e danni al dispositivo, e provocare lesioni ai pazienti. ATTENZIONE: Consultare il vostro medico se: - c è una perdita attraverso o attorno la protesi (tossendo); - la fonazione diventa difficile (lo sforzo richiesto è maggiore); - ci sono segni di infiammazione o cambiamenti del tessuto nel tratto della fistola o nella regione dello stoma (dolore, calore, gonfiore). Informazione sul funzionamento Come usare la Provox2 per parlare Una qualità di fonazione ottimale dipende dalla corretta chiusura del tracheostoma con un dito, preferibilmente della mano non dominante, per esempio della mano sinistra per chi é destro o viceversa. Evitare la fuoriuscita d aria tra il dito e i bordi dello stoma. L uso di uno scambiatore di calore e umidità come, per esempio, la cassetta Provox HME (vedere il seguente capitolo Quali prodotti correlati si possono utilizzare ), facilita la fonazione. I risultati, spesso, migliorano con la fattiva cooperazione tra medico, logopedista e paziente. [Fig. 2] Come pulire la protesi Una regolare pulizia della protesi aiuta a prolungarne la vita. Pulire la protesi almeno due volte al giorno e dopo ogni pasto con lo spazzolino Provox Brush. Per le protesi vocali Provox2 di lunghezza maggiore di 10 mm, utilizzare lo Provox Brush XL (Spazzolino Provox XL). Inserire lo spazzolino nella protesi e muoverlo dolcemente avanti e indietro con moto rotatorio. Rimuovere lo spazzolino, pulirlo con una garza e ripetere la procedura fino a quando la protesi risulta pulita. Nel caso in cui la protesi si muova verso l esterno durante la pulizia, trattenerla in posizione con un dito. Per dettagli sullo 34

35 spazzolino e su come pulirlo, consultare le istruzioni per l uso in dotazione con lo spazzolino Provox Brush. Lo spazzolino può essere usato per un mese (al massimo). Oltre a usare lo spazzolino Provox Brush, è possibile usare l irrigatore Provox Flush. L irrigatore Provox Flush può essere usato con acqua fresca o aria. Esso consente di sciacquare con acqua fresca o con aria, con una certa pressione, il lume della protesi. Per i dettagli e le modalità di pulizia dell irrigatore, consultare le istruzioni per l uso in dotazione con l irrigatore Provox Flush. Lo spazzolino, dopo l uso, può essere pulito con acqua, asciugato con una garza e riposto per l impiego successivo. Non occorre sterilizzare lo spazzolino, che potrà essere adoperato per alcune settimane. Risulta anche possibile, data la larghezza del lume della protesi, pulire il dispositivo mediante un espettorazione forzata tenendo chiuso lo stoma (per esempio, chiudendo lo stoma con un dito e spingendo con forza l aria verso la gola). Nota: Non utilizzare strumenti diversi dallo spazzolino Provox Brush e dall irrigatore Provox Flush per pulire la protesi, in quanto potrebbero danneggiare il funzionamento della valvola. [Fig. 3] Pulizia e disinfezione degli accessori È essenziale pulire gli accessori prima di ogni uso e disinfettarli almeno una volta al giorno in base alle rispettive istruzioni per l uso. Chi sostituisce la Provox2 se necessario? La sostituzione del dispositivo è effettuata dal medico o professionista il quale può, a sua discrezione professionale ed in conformità alle leggi del paese in cui svolge la propria attività, considerare di delegare l intervento di sostituzione della protesi a personale specializzato in tale procedura. Quanto tempo dura la protesi Provox2 e come sapere se occorre sostituirla? La protesi non é un impianto permanente e occorre sostituirla periodicamente. Il più frequente segnale per la sostituzione della protesi vocale Provox2, é la perdita di fluidi attraverso la valvola. Alcune gocce di bevande potrebbero entrare nella trachea, provocando tosse. 35
