Package Outline of Specifications The items of group A1 in «ΕΝΤΥΠΟ 8» are the most frequently used and will be purchased fron the Tenderer on a regular basis, who will provide the lowest total cost for all items of this group. The Tenderers must also specify which of the Optional items in the remaining groups they can provide, and indicate the cost per item. If a Tenderer can provide lower cost for any or all optional items, as well as lower transportation costs, because that Tenderer was successful in winning the Tender for the items of group A1, then this reduction of costs must be indicated clearly in entypa 10A. The Bank of Cyprus Oncology Center reserves the right to purchase items from the Optional groups (radionuclides or cold kits etc.) from diferrent Tenderers, on the basis of availability and costs. Supply and transportation conditions The production and transportation of all the radioactive materials and reagent kits must follow all the International and European Regulations for medically used radiopharmaceuticals. The requirements for keeping the above radiopharmaceuticals, such as temperature, humidity and other conditions must be provided by the manufacturers. Any accessories, tubing or special apparatus needed for the administration of these radiopharmaceuticals to patients, must be indicated and described clearly, and must be supplied in the proper quantities, together with the radiopaharmaceuticals. GROUP I : SECTION A1: RADIOPHARMACEUTICALS (RADIONUCLIDES AND COLD KITS) NEEDED FOR THE UNINTERRUPTED OPERATION OF NUCLEAR MEDICINE REQUIRED ITEMS ΟΜΑΔΑ Ι : ΕΝΟΤΗΤΑ Α1: ΡΑΔΙΟΝΟΥΚΛΙΔΙΑ/ΡΑΔΙΟΦΑΡΜΑΚΑ (ΥΠΟΧΡΕΩΤΙΚΑ) The Tenderers must specify if they can provide the Technetium Generators and other Radionuclides needed and included in this group. The tenderers must indicate both the cost per Unit / application and the cost for each individual radionuclide in Eντυπο 10, based on the expected consumption per year. Delivery will be made once per week for all the items ordered for that week, and the weekly shipping costs must be indicated separately in Εντυπο 10. It is expected that if any items of this group are ordered together with other optional items of another group, from the same Tenderer for a given week, then shipping costs will be charged only once for that week and will be the same as the standard weekly shipping charge quoted in Εντυπο 10 of this Tender. (Ιf the company does not provide 12 GBq generators, indicate the closest activity above 12 GBq. (Provide available generator activities and price for each generator of different activity in Εντυπο 10). Also provide the total cost for 48 such generators in Εντυπο 10. (anticipated number of generators needed per year) The tenderers must indicate if they can provide Sodium Iodide (I 131 ) Therapy Capsules. Please indicate available activities in capsule form and provide minimum and maximum activities. Provide corresponding cost in Εντυπο 10. Expected consumption: 130 capsules per year, 100 of which will be of 3700 MBq (100 mci) activity, and the rest will be 5550 Mbq (150 mci) activity. GROUP II : RADIONUCLIDES/RADIOPHARMACEUTICALS - OPTIONAL ITEMS ΟΜΑΔΑ ΙΙ : ΡΑΔΙΟΝΟΥΚΛΙΔΙΑ/ΡΑΔΙΟΦΑΡΜΑΚΑ - ΠΡΟΑΙΡΕΤΙΚΑ The items in GROUP II, SECTIONS A2, Α3, B1, Β2, C1, C2, and C3, are optional. Although for these items no expected consumption is indicated, however the tenderers must still provide the cost per unit in Εντυπο 10A, and these prices will remain fixed until the end of this tender. The Bank of Cyprus Oncology Center will rely on the prices offered, to order some of the optional items within two years from the announcement of this Tender. For this reason, the Tenderers must indicate prices for all the optional items, and these prices will be binding for the Tenderers. The BOCOC reserves the right to order all such optional items from one or more manufacturers based on the price offered by each one. SECTION A2 : RADIONUCLIDES/RADIOPHARMACEUICALS (OPTIONAL) (For treatment and Diagnosis) ΕΝΟΤΗΤΑ Α2: ΡΑΔΙΟΝΟΥΚΛΙΔΙΑ/ΡΑΔΙΟΦΑΡΜΑΚΑ ( ΠΡΟΑΙΡΕΤΙΚΑ ) ( Για Θεραπεία και Διάγνωση) The Tenderers must specify if they can provide the Radionuclides listed below for diagnostic tests and therapy applications. The tenderers must indicate both the cost per Unit / application in Eντυπο 10A. The expected consumption is limited from one to five per year. Delivery should be made for the week ordered, and the shipping costs must be indicated separately in Εντυπο 10A. It is expected that if any items of this group are ordered together with other items of group A1, then logistics cost will be charged only once for that week and will be the same as the standard weekly charge quoted in Εντυπο 10 of this Tender. The Bank of Cyprus Oncology Center reserves the right to purchase different radionuclides included in this Section from different suppliers, on the basis of costs, relying on the prices given by the suppliers in their proposal for this Tender. (1-1.5 mci per Kg of patient weight, i.e. 75, 100, and 150 mci units. Expected consumption: 1-2 patients per year The tenderers must indicate if they can provide Rhenium -186. Επίσης εντός ενός έτους από την ημερομηνία υπογραφής της σύμβασης θα πρέπει να εξασφαλιστεί η σχετική άδεια μεταφοράς και διακίνησης των ραδιοφαρμάκων όπως προνοείται από την σχετική νομοθεσία του Κράτους. Συστήνεται όπως η παράδωση γίνεται κάθε Παρασκευή ή Σάββατο απόγευμα της εβδομάδος που έγινε η παραγγελία (ή το αργότερο Κυριακή βράδυ μέχρι τις 08:00μμ) ή όπως αλλιώς συμφωνηθεί με το Κέντρο. Οι ενεργότητες (activities) που αναγράφονται στην εβδομαδιαία παραγγελία των ραδιοφαρμάκων θα είναι οι απαιτούμενες κατα την ημέρομηνία χρήσης τους. Αυτές θα λαμβάνονται υπόψη για σκοπούς αξιολόγησης σύμφωνα πάντα με τις προσφερόμενες τιμές (Εντυπο 10 & 10Α). Η ημέρα βαθμονόμησης του κάθε ραδιονουκλιδίου πρέπει να παρουσιαστεί με το τιμολόγιο χρέωσης. ΤΟΠΟΣ ΠΑΡΑΔΟΣΗΣ : Στο θερμό εργασήριο τού Ογκολογικού Κέντρου της Τράπεζας Κύπρου στη Λεωφόρο Ακροπόλεως 32, Στρόβολος, Λευκωσία Για κάθε είδος της ομάδας Α και της ομάδας Β, ο προσφοροδότης θα πρέπει να υποβάλει MA (manufacturing authorization) License of tracer (License of colt kit, αντίστοιχα) από μία από τις Ευρωπαϊκές χώρες. Με την παρουσίαση της ΜΑ άδειας δεν χρειάζεται η Εγγραφή του ραδιονουκλιδίου/ραδιοφαρμάκου ή του σκευάσματος στη Κύπρο. 4 5 1, Όλα τα προϊόντα που θα προσφερθούν πρέπει να συνάδουν με τα μονογράμματα της Ευρωπαϊκής (EP) ή Βρετανικής (BP) ή Αμερικάνικης ( USP) φαρμακοποιίας. 2. Προϊόντα που θεωρούνται φάρμακα σύμφωνα με τις πρόνοιες του Περί Φαρμάκων Ανθρώπινης Χρήσης «Έλεγχος της ποιότητας, προμήθειας και τιμών» Νόμου 70(Ι) του 2001-2002 θα λαμβάνονται υπόψη όταν: Κυκλοφορούν στη Κύπρο ή άλλη χώρα της Ευρωπαϊκής Ένωσης. 3. Για προϊόντα που θα προσφερθούν και δεν κυκλοφορούν στη Κύπρο πρέπει να προσκομίζεται πιστοποιητικό φαρμακευτικού προϊόντος ( CoPP ) ή σχετικό πιστοποιητικό ελεύθερης κυκλοφορίας ( FSC ) ή αντίγραφο της ισχύουσας άδειας κυκλοφορίας που να αποδεικνύουν ότι το προϊόν κυκλοφορεί σε τουλάχιστο μία από τις χώρες της Ευρωπαϊκής Ένωσης. Αναβάθμιση του προϊόντος Εάν, κατά τη διάρκεια χρήσης, υπάρξει οποιαδήποτε αναβάθμιση του προϊόντος, τότε ο επιτυχών προσφοροδότης, θα είναι υπόχρεος να το αντικαταστήσει με το νέο, στην ίδια τιμή. Νοείται ότι θα υπάρξει αξιολόγηση του νέου προϊόντος με δείγματα, που θα υποβληθούν. Αν ο προσφοροδότης προμηθεύει τα προϊόντα από εργοστάσιο παρασκευής στη Κύπρο πρέπει να έχει άδεια παρασκευής Φαρμακευτικών Προϊόντων και πιστοποιητικό GMP ( κανόνες ορθής παρασκευαστικής πρακτικής) από τις Φαρμακευτικές Υπηρεσίες της Κυπριακής Δημοκρατίας. 6 Αν ο προσφοροδότης προμηθεύει τα προϊόντα από εργοστάσιο παρασκευής άλλης Ευρωπαϊκής χώρας πρέπει να έχει άδεια παρασκευής Φαρμακευτικών Προϊόντων και πιστοποιητικό GMP ( κανόνες ορθής παρασκευαστικής πρακτικής) από τις Φαρμακευτικές Υπηρεσίες της αντίστοιχης χώρας κράτους μέλους. Στη στήλη «ΑΠΑΝΤΗΣΗ» σημειώνεται η απάντηση του Προσφέροντα που είτε είναι το συγκεκριμένο προϊόν που προσφέρει είτε έχει τη μορφή ΝΑΙ/ΟΧΙ εάν η αντίστοιχη 22

προδιαγραφή πληρούται ή όχι από την Προσφορά ή ένα αριθμητικό μέγεθος που δηλώνει την ποσότητα του αντίστοιχου χαρακτηριστικού στην Προσφορά. 5. Στη στήλη «ΠΑΡΑΠΟΜΠΗ» θα καταγραφεί η σαφής παραπομπή σε Παράρτημα της Τεχνικής Προσφοράς το οποίο θα περιλαμβάνει αριθμημένα Τεχνικά Φυλλάδια κατασκευαστών, ή αναλυτικές τεχνικές περιγραφές των προσφερόμενων προϊόντων, που κατά την κρίση του Προσφέροντα τεκμηριώνουν τα δηλούμενα στον Πίνακα Στοιχα. Οι παραπομπές πρέπει να είναι συγκεκριμένες (π.χ. Τεχνικό Φυλλάδιο 3, Σελ. 4 Παράγραφος 4, κ.λ.π.), ενώ αντίστοιχα στο τεχνικό φυλλάδιο ή την αναφορά θα υπογραμμίζεται το σημείο τεκμηρίωσης και θα σημειώνεται η αντίστοιχη παράγραφος του Πίνακα στην οποία καταγράφεται το ζητούμενο χαρακτηριστικό. 23
