Διαγωνισμού: Π2012/013 Τελευταία προθεσμία υποβολής προσφορών 21 Σεπτεμβρίου 2012 και ώρα 12.00 μμ. 10 η Σεπτεμβρίου, 2012 RESPONSES ΤΟ QUESTIONS FOR MAMMOGRAPHY TENDER Π2012/013 A. ΑΠΑΝΤΗΣΕΙΣ ΣΕ ΓΕΝΙΚΕΣ ΕΡΩΤΗΣΕΙΣ ΠΡΟΣΦΟΡΟΔΟΤΩΝ 1. Η προθεσμία για την υποβολή των προσφορών παρατείνεται μέχρι το μεσημέρι ( 12:00μμ) της 21 ης Σεπτεμβρίου 2012. 2. Ο προϋπολογισμός τη σύμβασης παραμένει ο ίδιος όπως ανακοινώθηκε στο Παράρτημα 3-6 ΜΕΡΟΣ Α παράγραφος 2.3 δηλ. 380.000,00 Ευρώ, συμπεριλαμβανομένης της συντήρησης για οκτώ (8) έτη μετά από διετή εγγύηση, συμπεριλαμβανομένων των X-Ray tubes, detectors, και όλων των εξαρτημάτων, μη συμπεριλαμβανομένου του Φ.Π.Α. 3. Οι προσφοροδότες πρέπει να αναγράψουν το πραγματικό κόστος που θα τιμολογήσουν τόσο για την αγορά του συστήματος όσο και το πραγματικό κόστος για τη συντήρηση για κάθε ένα από τα οκτώ (8) έτη συντήρησης μετά από δύο έτη εγγύησης. Την οποιαδήποτε αναγωγή θα την κάνει μόνο το Ογκολογικό Κέντρο κατά την διαδικασία αξιολόγησης. 4. Σύμφωνα με το έντυπο ΠΑΡΑΡΤΗΜΑ 3-6 ΜΕΡΟΑ Α, μέσα στον ενιαίο φάκελο της Προσφοράς τοποθετούνται δύο (2) Υποφάκελοι με όλα τα σχετικά με την Προσφορά στοιχεία ως εξής: Α. «Υποφάκελος Δικαιολογητικών Συμμετοχής και Τεχνικής Προσφοράς», ο οποίος περιέχει σε πέντε (5) αντίτυπα. 1

Πρέπει να υποβληθούν επίσης δύο (2) τουλάχιστον επί πλέον αντίτυπα σε ψηφιακή μορφή ( απαραίτητο ψηφιακό αντίτυπο για το έντυπο 8. Δια τα άλλα έντυπα και τεχνικά φυλλάδια επιθυμητό το ψηφιακό αντίτυπο αλλά όχι υποχρεωτικό, αν δεν προσφέρονται σε ψηφιακό μέσο από τον κατασκευαστή). Β. «Υποφάκελος Οικονομικής Προσφοράς», ο οποίος περιέχει, σε τρία (3) αντίτυπα, τα στοιχεία της Οικονομικής Προσφοράς του Προσφέροντα, όπως αυτά προσδιορίζονται στην παράγραφο 8.3.2 του Μέρους Α των Εγγράφων Διαγωνισμού. Οι Υποφάκελοι της Προσφοράς θα είναι σφραγισμένοι και θα φέρουν και τις ενδείξεις του κυρίως φακέλου. Το ένα από τα αντίτυπα θα φέρει την ένδειξη «ΠΡΩΤΟΤΥΠΟ» και θα είναι το επικρατέστερο των άλλων αντιτύπων σε περίπτωση διαφοράς μεταξύ τους. Τα υπόλοιπα αντίτυπα μπορούν να είναι φωτοαντίγραφα, νοουμένου ότι είναι καλής ποιότητας και ευανάγνωστα. Για πληροφορίες για όλα τα πιστοποιητικά που πρέπει να περιέχονται σε κάθε υποφάκελλο, βλέπε ΠΑΡΑΡΤΗΜΑ 3-6 ΜΕΡΟΣ Α 8.3, καθώς και το έντυπο 13 των προσφορών. 5. Γίνεται δεκτή η Εγκύκλιος ΓΛ/ΑΑΔΣ55 της Ελεγκτικής Υπηρεσίας και του Γενικού Λογιστηρίου της Δημοκρατίας ως Αρμόδια Αρχή Δημοσίων Συμβάσεων για προκηρύξεις διαγωνισμών εντός του 2012. Κατά συνέπεια, η εγγύηση συμμετοχής μπορεί να είναι προσωπική επιταγή του Προσφοροδότη Η εγγύηση πιστής εκτέλεσης από τον επιτυχόντα προσφοροδότη όμως, πρέπει να είναι τραπεζική εγγύηση που εκδίδεται από πιστωτικά ιδρύματα ή άλλα νομικά πρόσωπα που λειτουργούν νόμιμα στην Κύπρο, ή σε άλλες χώρες της Ευρωπαϊκής Ένωσης (ΕΕ) 6. Τα πιστοποιητικά που επιβεβαιώνουν τις προϋποθέσεις συμμετοχής (6.2.1α ζ) πρέπει να υποβληθούν με την προσφορά. Επίσης, τα πιστοποιητικά που επιβεβαιώνουν τις προϋποθέσεις συμμετοχής (6.2.3.1 7 πρέπει να υποβληθούν με την προσφορά. Για περαιτέρω διευκόλυνση, τα πιστοποιητικά που αναφέρονται στις παραγράφους 1 και 2 στο έντυπο 13, πρέπει να δοθούν με την προσφορά ενώ αυτά της παραγράφου 3, με την κατακύρωση της προσφοράς. 7. Η τοποθέτηση του Ογκολογικού Κέντρου Τράπεζας Κύπρου σχετικά με τα updates και upgrades, τροποποιείται ως ακολούθως: All software and hardware Updates that correct existing problems and/or improve productivity of the mammography system to be purchased, and released during the two year warranty and eight year service contract periods must be delivered and installed free of charge. All software and hardware Upgrades which are release during the first six months of the warranty period and provide new capabilities for the mammography system, must be delivered and installed free of charge. 8. Αναστολή Εκτέλεσης (ΠΑΡΑΡΤΗΜΑ Ι: ΓΕΝΙΚΟΙ ΟΡΟΙ ΣΥΜΒΑΣΗΣ Άρθρο 22 και 28) «Η Αναθέτουσα Αρχή δικαιούται να αναστείλει την παράδοση ή και την εγκατάσταση των προϊόντων ή οποιουδήποτε μέρους αυτών για τέτοιο χρόνο και με τέτοιο τρόπο όπως δύναται να θεωρήσει απαραίτητο.» Σύμφωνα και με την εισήγηση της αρμόδιας Αρχής, τα άρθρα 22 και 28 παραμένουν ως έχουν. Αυτά δεν είναι ετεροβαρή αφού οι λόγοι ενεργοποίησης τους πρέπει να είναι πάντοτε σοβαροί και η προσφυγή τους να γίνεται με καλή πίστη. Σε αντίθετη περίπτωση, 2

ο αντισυμβαλλόμενος μπορεί να προσφύγει στη δικαιοσύνη και να διεκδικήσει αποζημιώσεις. Σε περίπτωση τερματισμού από την Αναθέτουσα Αρχή η σχετική εγγύηση πιστής εκτέλεσης να επιστρέφεται εντός 30 ημερών από την ημερομηνία προειδοποίησης. 9. Η διάρκεια εκτέλεσης της σύμβασης καθορίστηκε σε τρεις μήνες από την ημερομηνία υπογραφής της Σύμβασης. Εάν όμως, λόγω οικοδομικών εργασιών χρειασθεί παράταση ενός ή δύο μηνών, ο επιτυχών προσφοροδότης θα ενημερωθεί και η ημερομηνία αποστολής του συστήματος και εγκατάστασης θα καθορισθεί όχι αργότερο της 30 ης Νοεμβρίου 2012. 10. With regard to the availability of spare parts for ten years from the date of installation, BOCOC agrees to delete the wording referring to discontinual of the production of such spare parts, provided the manufacturer will provide verification letter indicating that they undertake to provide spare parts for the duration of the warranty and service contract period. 11. Εις περίπτωση που ο προσφοροδότης πιστεύει ότι το σύστημα κλιματισμού του Ογκολογικού Κέντρου δεν είναι ικανοποιητικό και ότι οι αλλαγές στη θερμοκρασία και υγρασία στο δωμάτιο του μαστογράφου μπορούν να επηρεάσουν το detector του συστήματος με συνέπεια την αύξηση του κόστους συντήρησης, τότε ο επιτυχών προσφοροδότης πρέπει να εγκαταστήσει και να φροντίζει για την συντήρηση ξεχωριστού συστήματος κλιματισμού (closed control unit) στο δωμάτιο του μαστογράφου, με δικά του έξοδα, καθόλη την περίοδο της διετούς εγγύησης και την περίοδο της οκταετούς συντήρησης που θα αναλάβει με την υπογραφή της σύμβασης. Οι προσφοροδότες πρέπει να συμπληρώσουν το Exhibit 6 που αναφέρεται στο Παράρτημα ΙΙΙ (Environmental parameters required by the manufacturer specifications) και να υποβάλει αντίγραφο αυτού μαζί με την προσφορά του μέσα στον ίδιο υποφάκελλο με το έντυπο 8. 12. Σε περίπτωση που ο Αντισυμβαλλόμενος είναι Δημόσιος Φορέας που δεν συντάσσει πιστοποιητικά, η δεν παρέχει στοιχεία τεκμηρίωσης της επιτυχούς υλοποίησης των συμβάσεων (π.χ. Υπουργείο Υγείας και ΗΜΥ), όπως απαιτείται (Μέρος Α Παράγραφος 8.3.1.1.5(δ) σελίδα 22 από 31), τότε το ΟΚΤΚ θα δεχθεί αντίγραφα των τιμολόγιων παράδοσης που έχουν εκδοθεί προς αυτές της Δημόσιες Αρχές. 13. Ο Υπεύθυνος του Ανάδοχου πρέπει να κατέχει τίτλο σπουδών στην Ιατροφυσική ή Βιοϊατρική Μηχανική από αναγνωρισμένο ίδρυμα, καλές επικοινωνιακές ικανότητες, καλή γνώση της ελληνικής και αγγλικής γλώσσας και κατά προτίμηση γνώση της γλώσσας στη χώρα του κατασκευαστή. ( βλ.παράρτημα ΙΙ Όροι εντολής παρ. 4.1) Δια της παρούσης δηλώνεται ότι πτυχίο στο Medical Systems Engineering (BEng) είναι αποδεκτό. 14. Ο Επιτυχών Προσφοροδότης θα πρέπει να καθορίσει τουλάχιστον δύο μηχανικούς που βρίσκονται στην υπηρεσία του, οι οποίοι έχουν εκπαιδευτεί στο εργοστάσιο κατασκευής του μαστογράφου, για τη συντήρηση και επιδιόρθωση του συστήματος. Ο πρώτος μηχανικός θα πρέπει να έχει συμπληρώσει την εκπαίδευσή του και να παρουσιάσει σχετικό πιστοποιητικό μέχρι τον χρόνο παράδοσης και διενέργειας των ελέγχων τελικής αποδοχής του συστήματος. Ο δεύτερος μηχανικός θα πρέπει να παρουσιάσει το πιστοποιητικό της εκπαίδευσής του εντός έξι μηνών από την ημερομηνία εγκατάστασης του συστήματος (Παράρτημα ΙΙ Όροι εντολής παρ. 4.2) Δια της παρούσης 3

δηλώνεται ότι στα δυο άτομα αυτής της παραγράφου μπορεί να συμπεριληφθεί και ο υπεύθυνος μηχανικός της Εταιρείας 15. Στο Έντυπο 5 Πίνακας Β: Θα πρέπει στην στήλη «Σύνολο» κάθε χρονιάς να συμπληρώσετε τον συνολικό αριθμό του προσωπικού σας και στην στήλη «Αριθμός προσωπικού απασχολούμενου σε εργασίες συναφείς με την προκηρυσσόμενη», να δηλώσετε μόνον τον αριθμό των μηχανικών / τεχνικών που απασχολούνται στην εταιρεία σας. 16. Ο Συντελεστής βαρύτητας της 3D/Tομοσύνθεσης (3D/TOMOSYNTHESIS) τροποποιείται από 7% σε 12 % και ο συντελεστής βαρύτητας δια το Digital Flat Panel Detector τροποποιείται από 20 % σε 15 % 17. Τα Exhibit 1 Exhibit 6 που αναφέρονται στο Παράρτημα ΙΙΙ είναι έγγραφα που θα πρέπει να υποβάλει ο επιτυχών προσφοροδότης κατά την υπογραφή της σύμβασης, εκτός από το Exhibit 6 (Environmental parameters required by the manufacturer specifications), αντίγραφο του οποίου πρέπει να υποβάλουν όλοι οι προσφοροδότες μαζί με την προσφορά τους, μέσα στον ίδιο υποφάκελλο με το έντυπο 8. 18. Σε περίπτωση που ένας προσφοροδότης θέλει να προσφέρει σαν options, οτιδήποτε πέρα από αυτά που ζητούνται στα έντυπα της προσφοράς, τότε μπορεί να χρησιμοποιήσει το έντυπο 11Α που είναι αντίγραφο του εντύπου 11 19. Το δωμάτιο όπου πρόκειται να εγκατασταθεί ο νέος μαστογράφος στο Ογκολογικό Κέντρο θα έχει Ηλεκτρική Παροχή για τον μαστογράφο: Single Phase 240 VA - Maximum Line Impedance not to exceed 0.2 Ω, - Circuit Breaker at least 50A Type C B. ΑΠΑΝΤΗΣΕΙΣ ΣΕ TEXNIKES ΕΡΩΤΗΣΕΙΣ ΠΡΟΣΦΟΡΟΔΟΤΩΝ (ENtypo 8) 20. The Unit offered System must be able to perform 2D and 3D / Tomosynthesis studies. The software for this capability must be FDA approved and/or CE Marked by the time of signing the purchase agreement, which is expected to be before the end of the year 2012 21. Δια ερωτήσεις που αφορούν UPS, παραπέμπονται οι προσφοροδότες στο Parartima II, Όροι εντολής, παράγραφος 2: Ο προσφοροδότης αναμένεται να παράσχει στοιχεία που αφορούν τις απαιτήσεις του αντικειμένου της σύμβασης για UPS και να περιλάβει το απαιτούμενο UPS στην προσφορά. Εις απάντηση των ερωτήσεων και σχολίων των προσφοροδοτών, το λεχτικό στις τεχνικές προδιαγραφές, έντυπο 8, παράγραφος 61, τροποποιείται ως εξής: An online UPS of suitable rating must be supplied for the computer system of the acquisition and independent review workstation. Verify that this is included. 4

Η τροποποίηση αυτή συνδέεται άμεσα με την απάντηση 45 που αφορά την πρώτη ερώτηση στο κεφάλαιο Reliability, Warranty, Spare parts, Maintenance Contract, and Training of Service Engineers for the Digital mammography system (βλέπε και απάντηση με αριθμό 45) 22. Η παράγραφος 3 του Εντύπου 8 τροποποιείται ως ακολούθως: The unit must provide a fully automatic operation mode, which allows automatic selection of target (where applicable), filter, kv and mas based on breast thickness and density, for optimum image quality. Indicate if this requirement is satisfied 23. The Wording of paragraph 4 of entypo 8 remains unchanged. Please state which of the following modes of operation are available: automatic, semi-automatic and manual operation. 24. The Wording of paragraph 7 of entypo 8 remains unchanged. Please Indicate the power output of the unit currently offered and comment on status/progress in contrast studies 25. The Wording of paragraph 9 of entypo 8 is modified as follows: Dual focal spot of 0.3mm and 0.1 mm for each target material is needed, in compliance with IEC-336. Smaller focus provides better resolution. 9.1 Indicate the actual size of the large and small focus of the unit offered 9.2 Indicate if the unit is capable of providing at least 100mA on the large focus, and at least 30 ma on the small focus, respectively 26. The Wording of paragraph 28 of entypo 8 is modified as follows The Detective Quantum Efficiency (DQE) at 1 lp / mm for an entrance skin exposure of 15 mr (surface air kerma of 132μGy) to be 0.45 or higher, and at 5 lp / mm for the same exposure to be 0.24 or higher. 27. The Wording of paragraph 38 remains unchanged. If there is ongoing research on tissue contrast but it is not yet possible to offer the module now as commercial product or indicate its future cost, Tenderers please state so. 28. The Wording of paragraph 41 remains unchanged. Ethernet adapter 1000 is required. 29. Since the localization software is usually on the operator s acquisition workstation, the wording of 52 of entypo 8 is modified as follows: Indicate if the independent Review workstation offered has the following Display functions: a) Zoom/Roam b) Invert Image c) Windowing / Image Contrast & Brightness adjustments d) Automatic Contrast setting e) Flip/Rotate/Pan f) Magnify g) Measurements (distance, angle, ROI, area, statistics etc) h) Image Annotations and comment i) Application oriented Look Up Table for contrast, brightness, edge enhancement, noise reduction etc. j) Input of patient data via keyboard k) Preselection of patient orientation for automatic image orientation 30. The wording of paragraph 55 of entypo 8 is modified as follows: 5

55.1 Image communication through Ethernet interface in DICOM 3 format for network connection. The following DICOM compliance statements must be available on the acquisition workstation: a) Push b) Send-Receive c) Print d) Storage service class e)archive f) Modality Work List Management g) Query/retrieve service class. Verify availability of the above and on the acquisition station. 55.2 Verify that the following are available on the independent Diagnostic Workstation: Print, b) Aechive, c) Storage Service class and d) Querie/retrieve service class Other DICOM Conformance statements to be stated. 55.3 Indicate if the system provides the software and an integrated screen sensor for automatic calibration and QC of the 5 MP monitors 31. The wording of paragraph 60 of entypo 8 remains unchanged 32. The wording of paragraph 61 of entypo 8 is modified as follows: An online UPS of suitable rating must be supplied for the computer system of the acquisition station and the independent diagnostic workstation. Verify that this is included. 33. The wording of paragraph 62.c of entypo 8 is modified as follows: Indicate if the system can position the needle either automatically (or motorized) both in the lateral (x-axis) and longitudinal (y-axis) positions, while the vertical (z-axis) can be positioned manually by the person performing the biopsy. Also indicate if the positioning can be done using Cartesian coordinates, 34. The wording of paragraph 64.c of entypo 8 is modified as follows: Automatic localization and automatic (or motorised) positioning of the needle positioner in less than 10 seconds. State if the system satisfies this time limit 35. Response to questions on paragraph 66 of entypo 8: Since different manufacturers offer different type of vacuum biopsy device and needle attachment, it is preferable to include in the Tender price a more universal device (e.g Vacora type or equivalent), and also include in the tender price enough needles for the first 20 biopsies. Needle lengths of 90mm to 120mm are preferable (e,g,vacora 9G-10G- 12G or equivalent). Further needle attachments and needles can be purchased as consumables in the future. Consequently, the cost of these consumables/needles must be stated in entypo 11A 36. Response to questions on paragraph 67 of entypo 8 : The wording of the paragraph is modified to read as follows: A special biopsy chair that allows biopsies with different patient positions including decubitus, must be included in the offer. A Mammography chair model stereotaxia AK 5010 from A/K/R/U/S would be the most preferable. If its cost is too high for this chair to be included in the Tender price, then the model patientstuhl ak 5003m from the same company would be accepted. If this second one is offered, then the cost of the first one must be shown in entypo 11A as an option. 6

37. The wording of paragraph 71 of entypo 8 is modified as follows: State if the digital mammography system, has the capability of both 2D and 3D acquisitions in one examination, during one compression. 38. Response to questions on paragraph 81 of entypo 8: A dedicated mammography QC kit is required as described in paragraph 81 39. Response to questions on paragraph 82 of entypo 8: A multipurpose QC kit described in queston 82 is required, in addition to the dedicated kit of paragraph 81. If some Tenderers cannot include both QC kits in the Tender price, they may decide to offer one of them in the tender price and the other as option and quote its price in entypo 11A. This will be accounted for, in the financial evaluation 40. The wording of paragraph 83.2 of entypo 8 is modified as follows: 83.2 Verify that the detector(s) offered can cover kv in the mammographic range with accuracy within +/- 1kV, and in the diagnostic range with accuracy better than +/- 2.0 % of the value indicated on the selected setting. State the accuracy of the system offered (examples: RTI Piranha / Piranha premium, RaySafe Solo or RaySafe Xi, IBA MagicMax meter and measuring set IBArad/flu-L, including LXcan) 41. The wording of paragraph 86 of entypo 8 is modified as follows: A Full field low contrast detectability phantom of variable thickness for mammography must be included (examples: CIRS Model 082, Fluke Model 18-252). 42. The wording of paragraph 93 of entypo 8 remains unchanged 43. The wording of paragraph 94 of entypo 8 is modified as follows: A laptop (with carrying case) for the QC tests on the mammography unit, with pre-installed Windows 7 O/S and the necessary SW packages, where applicable, for all above test devices. The laptop should fulfil the following specifications. (If the laptop offered is of lower specs, give full details for comparison during evaluation): - CPU: Intel Core i5, 2.4GHz or Equivalent - Display: 15.4 1600x900 - Widescreen Display - Memory: 4096 DDR 3 SD RAM - Storage: 500GB - Graphics: - Graphic Accelerator With at Least 256 MB Dedicated Memory - Optical Drive: DVD ±RW Double Layer Suitable software for immediate display of the waveforms and measurements archiving to be provided. Laptop Backpack to be provided 44. Response to questions on paragraph 96 of entypo 8 : The residual value is added in entypo 11 below the costs of the system and the cost of maintenance each year. It is subtracted from the total cost and influences the financial evaluation 45. The wording of paragraph 1 of the section on Reliability, Warranty, Spare parts, Maintenance Contract, and Training of Service Engineers for the Digital mammography system του εντύπου 8, is modified as follows (Βλέπε επισης απάντηση με αριθμό 21 σε ερωτήσεις που αφορούν το UPS) 7

1.1. The Tenderer must state explicitly how the system protects itself (generator, flat panel, patient information and images, etc.) in cases of overvoltages, or power failures 1.2. In case the system does not have self protection as stated in 1,1 above, then the Tenderers must state what type of protection systems they will provide for the protection of the whole unit. 1.3. The Tenderers must state that they undertake the responsibility and will correct without delay any damages caused` from Power Loss or instabilities of the voltage during both the warranty and service contract periods, and no reduction of the agreed uptime will be accepted for such power loss damages 46. Response to questions on paragraph 2 of the section on Reliability, Warranty, Spare parts, Maintenance Contract etc.,, του εντύπου 8: If downtime exceeds 3%, then both the conditions of clause 2.1 and 5.2 in section on Reliability, Warranty and maintenance contract will be applicable. 47. Response to questions on paragraph 6 of the section on Reliability, Warranty, Spare parts, Maintenance Contract,etc. του εντύπου 8: Wording remains unchanged. Tenderers indicate their policy on stocking spare parts for evaluation 48. Response to questions on paragraph 7 of the section on Reliability, Warranty, Spare parts, Maintenance Contract,etc. του εντύπου 8: The wording is modified to read as follows: The Tenderers must indicate the ability of the digital mammography system to accept software and hardware upgrades and state that: 7.1 Any updates correcting problems or improving performance, released during the warranty and service contract periods must be provided free of charge and without delay 7.2 Any hardware and software upgrades released during the first six months after installation (i.e. during first quarter of the warranty period), must be included and installed without any additional cost to BOCOC. 49. Response to questions on paragraph 12 of the section on Reliability, Warranty, Spare parts, Maintenance Contract,etc. του εντύπου 8: Response to 12.1: The wording remains without change. Note: Two trained service engineers are needed to cover each other at times of sickness and vacation, or when one engineer is in another city repairing equipment of another customer etc, The intension is to have less than 1% downtime with minimal discomfort or delays for the patients. The 3% limitis not a cushion for relaxing but a limit indicating the worse service possible.
