πζηάζεηο γηα ηε ρξήζε επείγνπζαο κνξηαθήο δηάγλσζεο θπκαηίσζεο (Mycobacterium Direct Testing, ΑMTD) 3. ην εξγαζηήξην ε εμέηαζε εθηειείηαη πάληα ζην πξώην δείγκα αζζελνύο κε ζεηηθή κηθξνζθνπηθή εμέηαζε. Όηαλ ην απνηέιεζκα ηεο κνξηαθήο εμέηαζεο είλαη ζεηηθό, ν αζζελήο ζεωξείηαη όηη πάζρεη από θπκαηίωζε θαη γίλεηαη έλαξμε αληηθπκαηηθήο αγωγήο ελ αλακνλή ηωλ απνηειεζκάηωλ ηεο θαιιηέξγεηαο. ε απηά ηα δείγκαηα δηελεξγείηαη θαη κνξηαθόο έιεγρνο επαηζζεζίαο ζηα αληηθπκαηηθά θάξκαθα θαη ην απνηέιεζκα ζηέιλεηαη ζηελ θιηληθή όηαλ είλαη πιεξνθνξηαθό (~85% ησλ πεξηπηώζεσλ). 4. ε θάζε άιιε πεξίπησζε, ε εμέηαζε εθηειείηαη κεηά από παξαγγειία ηνπ θιηληθνύ ηαηξνύ επί πςειήο θιηληθήο ππνςίαο θπκαηίσζεο. 5. Πεξηπηώζεηο κε πςειή θιηληθή ππνςία θπκαηίσζεο αιιά κε αξλεηηθή κηθξνζθνπηθή εμέηαζε πξέπεη λα αμηνινγνύληαη αλά πεξίπησζε. Με βάζε ηηο αλαζεωξεκέλεο νδεγίεο ηνπ Ακεξηθάληθνπ Κέληξνπ ειέγρνπ ινηκώμεωλ (CDC), ζε κηθξνζθνπηθά αξλεηηθά/κνξηαθά ζεηηθά δείγκαηα, ζα πξέπεη λα δηεξεπλάηαη ε έλαξμε αληηθπκαηηθήο αγωγήο κε βάζε ηηο θιηληθέο ελδείμεηο ελ αλακνλή ηωλ απνηειεζκάηωλ ηεο θιαζηθήο θαιιηέξγεηαο. Η απνζηνιή επηπξόζζεηωλ δεηγκάηωλ γηα κνξηαθή εμέηαζε επαθίεηαη ζηελ θξίζε ηνπ θιηληθνύ ηαηξνύ. Έλαο αζζελήο κπνξεί λα ζεωξεζεί όηη πάζρεη από θπκαηίωζε, εάλ 2 ή πεξηζζόηεξα αξλεηηθά κηθξνζθνπηθά δείγκαηα, είλαη ζεηηθά ζηε κνξηαθή εμέηαζε. 6. Σε πεξηπηώζεηο κε ζεηηθό κηθξνζθνπηθό απνηέιεζκα θαη αξλεηηθή κνξηαθή εμέηαζε δηελεξγείηαη δνθηκαζία γηα παξνπζία αλαζηνιέσλ (3-7% ηωλ πεξηπηώζεωλ) Α. Δάλ ππάξρνπλ αλαζηνιείο, ε κνξηαθή εμέηαζε δελ έρεη δηαγλωζηηθή αμία. Θα πξέπεη λα εμεηάδεηαη ε παξαγγειία επηπιένλ δηαγλωζηηθώλ δνθηκαζηώλ θαη λα δηεξεπλάηαη ε έλαξμε αληηθπκαηηθήο αγωγήο κε βάζε ηηο θιηληθέο ελδείμεηο θαη ζε αλακνλή ηωλ απνηειεζκάηωλ ηεο θιαζηθήο θαιιηέξγεηαο. Β. Δάλ δελ ππάξρνπλ αλαζηνιείο, ζα πξέπεη λα δηεξεπλάηαη ε έλαξμε αληηθπκαηηθήο αγωγήο κε βάζε ηηο θιηληθέο ελδείμεηο θαη ζε αλακνλή ηωλ απνηειεζκάηωλ ηεο θιαζηθήο θαιιηέξγεηαο. Ο αζζελήο κπνξεί λα ζεωξεζεί όηη παξνπζηάδεη ινίκωμε από άηππα κπθνβαθηεξίδηα εάλ έλα δεύηεξν δείγκα είλαη κηθξνζθνπηθά ζεηηθό/κνξηαθά αξλεηηθό θαη δελ ππάξρνπλ αλαζηνιείο. 7. Σε πεξηπηώζεηο κε αξλεηηθό κηθξνζθνπηθό απνηέιεζκα θαη αξλεηηθή κνξηαθή εμέηαζε ζα πξέπεη λα δηεξεπλάηαη ε έλαξμε αληηθπκαηηθήο αγωγήο κε βάζε ηηο θιηληθέο ελδείμεηο ελ αλακνλή ηωλ απνηειεζκάηωλ ηεο θιαζηθήο θαιιηέξγεηαο θαη λα εμεηάδεηαη ε παξαγγειία επηπιένλ δηαγλωζηηθώλ δνθηκαζηώλ. Με δεδνκέλε ηε ρακειόηεξε επαηζζεζία ηεο κεζόδνπ ζηα κηθξνζθνπηθά αξλεηηθά δείγκαηα, δελ κπνξεί λα απνθιεηζηεί ε δηάγλωζε ηεο θπκαηίωζεο. 8. Σηα εμσπλεπκνληθά δείγκαηα (θαη ηδηαίηεξα ζε δείγκαηα πιεπξηηηθνύ θαη αζθηηηθνύ πγξνύ, ΕΝΥ, θιπ) ε κέζνδνο έρεη ρακειόηεξε επαηζζεζία ή ελδέρεηαη λα ππάξρνπλ αλαζηνιείο. Η ειάρηζηε πνζόηεηα ΔΝΤ πνπ γίλεηαη δεθηή από ην εξγαζηήξην είλαη 0.5 ml. H ιήςε κεγαιύηεξεο πνζόηεηαο ή πνιιαπιώλ δεηγκάησλ απμάλεη ηε πηζαλόηεηα αλίρλεπζεο ηνπ κπθνβαθηεξηδίνπ. 9. Γείγκαηα αίκαηνο ή εκθαλώο αηκαηεξά δείγκαηα είλαη αθαηάιιεια γηα ηελ κέζνδν AMTD ηηο αθόινπζεο πεξηπηώζεηο, ε εμέηαζε AMTD δελ πξέπεη λα δεηείηαη σο εμέηαζε ξνπηίλαο: 1. Αζζελείο κε γλσζηό AMTD απνηέιεζκα. 2. Αζζελείο κε ρακειή θιηληθή ππνςία γηα ινίκσμε κε M. tuberculosis. 3. It has similar sensitivity to culture, targets Mycobacterium tuberculosis specifically and enables simultaneous detection of rifampicin resistance via the rpob gene. Training requirements are generally 1-2 days, and the 1-step external sample preparation is extremely simple. The closed system ensures that there is no risk of contamination and no requirement for bio-safety facilities. Test results can be obtained in just 90 minutes.

Preferential pricing will be granted to 116 lowand middle- income countries where TB is endemic, with additional reduction in price once there is significant volume of demand."there has been a strong commitment to remove any obstacles, including financial barriers, that could prevent the successful roll-out of this new technology," said Dr Giorgio Roscigno, FIND's Chief Executive Officer.
