2 The procedure describes the steps we shall follow in order to cancel an insurance policy. Following the procedure correctly is of vital importance since there are insurance policies which are obligatory by law. Η διαδικαςία αυτι περιγράφει τα βιματα που πρζπει να ακολουκθκοφν για τθν ακφρωςθ ενόσ ςυμβολαίου αςφάλιςθσ. Η ςωςτι τιρθςθ τθσ διαδικαςίασ είναι υψίςτθσ ςθμαςίασ αφοφ υπάρχουν αςφάλειεσ οι οποίεσ είναι υποχρεωτικζσ από τθν νομοκεςία. 2

3 The insured person departed from Cyprus The Medical for Aliens policy may be cancelled in case that the alien has departed from Cyprus. In such a case, the associate or the customer (who is also the Proposer of the policy) shall bring to us a copy of the air ticket of the departure in order to proceed with the cancellation. The copy of the air ticket is a basic requirement and this shall be scanned in the company software together with the completed cancellation form. Despite the above in cases that there is no way to bring us the copy of the air ticket we may proceed to cancel a policy with the cancellation form only. In terms of direct customers no cancellation shall take place unless we have the signature of the Proposer. Αναχώρηςη του αςφαλιςμζνου προςώπου από την Κφπρο Η αςφάλιςθ Ιατροφαρμακευτικισ Περίκαλψθσ Αλλοδαπϊν δφναται να ακυρωκεί ςε περίπτωςθ που ο αλλοδαπόσ ζχει αναχωριςει από τθν Κφπρο. Σε τζτοια περίπτωςθ ο ςυνεργάτθσ ι ο πελάτθσ (ο οποίοσ είναι και ο Προτείνοντασ του ςυμβολαίου) κα πρζπει να προςκομίςει αντίγραφο του ειςιτθρίου αναχϊρθςθσ για να γίνει ακφρωςθ. Το αντίγραφο του ειςιτθρίου είναι βαςικι προχπόκεςθ και κα πρζπει να ςαρϊνεται ςτο λογιςμικό τθσ εταιρείασ ταυτόχρονα με τθ ςυμπλθρωμζνθ φόρμα ακφρωςθσ. Παρόλα αυτά ςε περίπτωςθ που δεν υπάρχει θ δυνατότθτα προςκόμιςθσ του αντίγραφου του ειςιτθρίου μποροφμε να προχωριςουμε ςτθν ακφρωςθ μόνο με τθ φόρμα ακφρωςθσ. Στισ περιπτϊςεισ των απευκείασ πελατϊν δεν κα γίνεται ακφρωςθ χωρίσ τθν υπογραφι του Προτείνοντα ςε καμία περίπτωςθ. 3

4 The insured person has changed employer In case that the insured person has moved to a different employer and the new employer did not issue an insurance policy with our company, our associate shall bring to us a copy of the new insurance policy in order to proceed with the cancellation. In such a case the associate shall sign the cancellation from. In a similar case of a direct customer (employer) in order to proceed with cancellation they need to bring to us a copy of the new insurance policy or a copy of the new employment contract. In addition, the customer (employer) shall sign the cancelation form. All documents shall be scanned in the company software. Το αςφαλιςμζνο πρόςωπο ζχει αλλάξει εργοδότη Σε περίπτωςθ που το αςφαλιςμζνο πρόςωπο ζχει αλλάξει εργοδότθ και ο καινοφριοσ εργοδότθσ δεν ζχει κάνει αςφάλεια ςτθ δικι μασ εταιρεία ο ςυνεργάτθσ μασ κα πρζπει να προςκομίςει αντίγραφο τθσ καινοφριασ αςφάλειασ για να προχωριςουμε ςε ακφρωςθ. Σε τζτοια περίπτωςθ ο ςυνεργάτθσ κα πρζπει να υπογράψει και τθ φόρμα ακφρωςθσ τθσ αςφάλειασ. Στθν ίδια περίπτωςθ αλλά αν ζχουμε κοντά μασ κάποιον απευκείασ πελάτθ (εργοδότθ) για να προχωριςει ςτθν ακφρωςθ κα πρζπει να προςκομίηεται ςε μασ αντίγραφο τθσ καινοφριασ αςφάλιςθσ ι αντίγραφο του καινοφριου ςυμβολαίου εργοδότθςθσ. Επιπρόςκετα, ο πελάτθσ (εργοδότθσ) κα πρζπει να υπογράψει τθ φόρμα ακφρωςθσ. Όλα τα ζντυπα πρζπει να ςαρωκοφν ςτο λογιςμικό τθσ εταιρείασ. 4

5 The insured person did not depart from Cyprus and stays illegally A CSA shall never cancel the policy. When facing such a case the CSA shall send an to the Managing Director of the company in order to have his written approval before he/ she proceed with the cancellation. Only the employer (Proposer) or a person acting with his written consent may proceed with the cancellation. Το αςφαλιςμζνο πρόςωπο δεν ζχει αναχωρήςει από την Κφπρο και μζνει παράνομα Σε καμία περίπτωςθ δεν μπορεί να γίνει ακφρωςθ τθσ αςφάλειασ από κάποιον CSA. Όταν ζχουμε μια τζτοια περίπτωςθ ο CSA κα πρζπει να ςτζλνει θλεκτρονικό μινυμα ςτο Γενικό Διευκυντι τθσ εταιρείασ οφτωσ ϊςτε να εξαςφαλίςει γραπτι ζγκριςθ για να μπορζςει να προχωριςει ςε ακφρωςθ. Σε ακφρωςθ μπορεί να προχωριςει μόνο ο εργοδότθσ (Προτείνοντασ) ι άτομο που ενεργεί με τθ γραπτι του ςυγκατάκεςθ. 5

6 Employers Liability Insurance for Housemaids In case that the housemaid leaves the house the insurance policy cannot be cancelled. Despite this if the employer wishes to issue a new policy for his/ her new housemaid and would like to have one date of expiration date for his employer s liability policy as well as the medical for Alien policy then the policy may be cancelled (there will be no refund) and a new policy may be issued. The employer s liability policy must be cancelled in case the employer has passed away. In such a case the death certificate shall be presented to us and a cancellation form shall be completed. Αςφάλεια Ευθφνησ Εργοδότη για Οικιακζσ Βοηθοφσ Σε περίπτωςθ που θ οικιακι βοθκόσ φφγει από τθν οικία απαςχόλθςθσ θ αςφάλεια δεν ακυρϊνεται. Παρόλα αυτά αν ο εργοδότθσ επικυμεί να εκδϊςει καινοφρια αςφάλεια για τθ νζα του οικιακι βοθκό και επικυμεί να ζχει μια θμερομθνία λιξθσ για το ςυμβόλαιο Ευκφνθσ Εργοδοτι και το ςυμβόλαιο ιατροφαρμακευτικισ περίκαλψθσ, τότε ακυρϊνεται θ αςφάλιςθ Ευκφνθσ Εργοδότθ (δεν επιςτρζφεται οποιοδιποτε ποςό) και εκδίδεται καινοφριο ςυμβόλαιο. Η αςφάλιςθ ευκφνθσ εργοδότθ για οικιακι βοθκό πρζπει να ακυρωκεί ςε περίπτωςθ που ο εργοδότθσ ζχει αποβιϊςει. Σε τζτοια περίπτωςθ κα πρζπει να προςκομιςκεί ςε μασ πιςτοποιθτικό κανάτου κακϊσ και να ςυμπλθρωκεί φόρμα ακφρωςθσ ςυμβολαίου. 6

7 Employer s Liability Insurance on a specific name For all occupations with the exception of that of the housemaid, in case that the employer s liability policy and the medical for Αliens policy are issued at the same time, the policy is issued on the name of the specific employee. Such policy may be cancelled provided that they bring to us a copy of the departure air ticket or if they proceed in issuing a new policy at the same time. Ονομαςτική Αςφάλιςη Ευθφνησ Εργοδότη Για τα επαγγζλματα εκτόσ από αυτό τθσ οικιακισ βοθκοφ, ςε περίπτωςθ που το ςυμβόλαιο ευκφνθσ εργοδότθ εκδίδεται ταυτόχρονα με τθν ιατροφαρμακευτικι περίκαλψθ, τότε γίνεται ονομαςτικά για τον ςυγκεκριμζνο εργοδοτοφμενο. Τα ςυμβόλαια αυτοφ του είδουσ μποροφν να ακυρωκοφν νοουμζνου ότι κα προςκομιςκεί ςε μασ αντίγραφο ειςιτθρίου ι κα εκδοκεί ταυτόχρονα καινοφριο ςυμβόλαιο. 7

8 All Employer s Liability policies with the exception of what is described above cannot be cancelled unless the Customer Service Advisor has a written approval from the Managing Director. Όλα τα ςυμβόλαια Ευκφνθσ Εργοδότθ εκτόσ από αυτά που περιγράφονται πιο πάνω δεν είναι δυνατό να ακυρωκοφν εκτόσ και αν ο CSA εξαςφαλίςει γραπτι ζγκριςθ από τον Γενικό Διευκυντι. 8

9 Motor Insurance A motor insurance policy can only be cancelled by the policyholder who shall be the owner of the vehicle. The return of the original Certificate of Insurance is a must in order to proceed with the cancellation. In addition the Customer Service Advisor (CSA) shall complete a Cancellation Form according to the issuing Insurance Company. All online policies can be cancelled directly by the CSA. The CSA shall send an to the colleague dealing with the specific insurance company for the cancellation of any manual insurance policy. Αςφάλεια Μηχανοκίνητων Οχημάτων Το ςυμβόλαιο αςφάλιςθσ μθχανοκίνθτου οχιματοσ μπορεί να ακυρωκεί μόνο από τον ιδιοκτιτθ του ςυμβολαίου ο οποίοσ κα πρζπει να είναι και ο ιδιοκτιτθσ του οχιματοσ. Η επιςτροφι του πρωτότυπου Πιςτοποιθτικοφ Αςφάλιςθσ είναι βαςικι προχπόκεςθ για να προχωριςουμε ςε ακφρωςθ του ςυβολαίου. Επιςπρόςκετα ο CSA κα πρζπει να ςυμπλθρϊςει τθ Φόρμα Ακφρωςθσ ανάλογα με τθ εκδότρια αςφαλιςτικι εταιρεία. Όλα τα ςυμβόλαια που εκδίδονται θλεκτρονικά ςτο λογιςμικό τθσ εταιρείασ (online) μποροφν να ακυρωκοφν απευκείασ από τον CSA. Ο CSA κα πρζπει να ςτζλνει μινυμα με θλεκτρονικό ταχυδρομείο ( ) ςτο ςυνάδελφο που αςχολείται με τθ ςυγκεκριμζνθ αςφαλιςτικι εταιρεία, για τθν ακφρωςθ οποιουδιποτε ςυμβολαίου δεν ζχει εκδοκεί θλεκτρονικά από το λογιςμικό τθσ εταιρείασ μασ (manual). 9

10 For any product that had not been mentioned above we only require the completion of Cancellation Form and the signature of the policyholder in order to proceed with the cancellation. Σε ςχζςθ με οποιοδιποτε προϊόν δεν ζχει αναφερκεί πιο πάνω χρειαηόμαςτε μόνο να ςυμπλθρωκεί θ Φόρμα Ακφρωςθσ και να υπογράψει ο ιδιοκτιτθσ του ςυμβολαίου για να προχωριςουμε ςτθν ακφρωςθ. 10

11 All documents referred in the above procedure shall be scanned and uploaded in our system. Original Cancellation forms shall be kept in a file at the branch for one year from the date of cancellation. Όλα τα ζντυπα που αναφζρονται ςτθν πιο πάνω διαδικαςία κα πρζπει να ςαρϊνωνται και να καταχωροφνται ςτο λογιςμιςκό μασ. Η πρωτότυπθ φόρμα Ακφρωςθσ κα πρζπει να διατθρείτε ςε αρχείο ςτο υποκατάςτθμα για ζνα χρόνο από τθν θμερομθνία ακφρωςθσ. 11
