ΚΛΙΝΙΚΗ ΠΕΡΙΠΤΩΣΗ Νόσος Πεπτικού έλκους - Σύνδρομο Ζοllinger-Ellison
ΠΕΡΙΓΡΑΦΗ ΤΗΣ ΠΕΡΙΠΤΩΣΗΣ Ένας άνδρας 43 ετών προσέρχεται στον γαστρεντερολόγο με συμπτώματα πεπτικού έλκους. Αναφέρει ότι παρουσιάζει καυστικό άλγος στο μέσον του επιγαστρίου, το οποίο υφίεται μετά την πρόσληψη τροφής και επιδεινώνεται με άδειο στόμαχο. Ο ασθενής καταναλώνει μεγάλες ποσότητες αντιόξινων δίχως αποτέλεσμα. Εμφανίζει υποτροπιάζοντα επεισόδια πεπτικού έλκους τα τελευταία 5 έτη, με ελάχιστη ανακούφιση με τη συνήθη αγωγή. Τα επίπεδα γαστρίνης νηστείας στον ορό ανευρέθησαν εξαιρετικά αυξημένα. Μετά από τον περαιτέρω έλεγχο, ετέθη η διάγνωση ενός όγκου του παγκρέατος που εκκρίνει γαστρίνη.
Περίληψη Ένας άνδρας 43 ετών εμφανίζει υποτροπιάζοντα πεπτικά έλκη που δεν ανταποκρίνονται στη συνήθη αγωγή. Εμφανίζει αυξημένες τιμές γαστρίνης νηστείας στον ορό και διαγιγνώσκεται όγκος παγκρέατος γαστρινοεκκριτικός (σύνδρομο Ζοllinger-Ellison).

ΚΛΙΝΙΚΗ ΣΥΣΧΕΤΙΣΗ (1) Το πεπτικό έλκος αποτελεί συχνή νόσο για τον ιατρό της πρωτοβάθμιας φροντίδας. Τα πεπτικά έλκη δημιουργούνται από τη διάρρηξη της συνέχειας του γαστρικού βλεννογόνου ή/και του βλεννογόνου του δωδεκαδάκτυλου που οδηγούν σε φλεγμονή και άλγος. Το άλγος περιγράφεται από τους ασθενείς συνήθως ως άλγος επιγαστρίου στην εντόπιση και καυστικό στο χαρακτήρα, με επιδείνωση κατά τη νηστεία και με ανακούφιση μετά τη λήψη τροφής. Η λήψη μη στεροειδών αντιφλεγμονωδών φαρμάκων (ΜΣΑΦ) και η λοίμωξη από το Ηelicobacter pylori αποτελούν τις δύο βασικές αιτίες που προκαλούν πεπτικό έλκος. Η παραγωγή γαστρικού οξέος συμβάλλει στη βλάβη του βλεννογόνου, αλλά δεν κατέχει καθοριστικό ρόλο.
ΚΛΙΝΙΚΗ ΣΥΣΧΕΤΙΣΗ (2) Η παραγωγή γαστρικού οξέος στον στόμαχο ρυθμίζεται από ποικίλους παράγοντες, όπως η ορμόνη γαστρίνη, η παρακρινής ισταμίνη και η νευροκρινής ακετυλχολίνη. Ο έλεγχος της παραγωγής γαστρίνης είναι εξαιρετικά ακριβής. Ωστόσο, στους ασθενείς με σύνδρομο Ζοllinger-Ellison, το σοβαρό πεπτικό έλκος είναι αποτέλεσμα της υπερέκκρισης γαστρικού οξέος από μη ελεγχόμενη έκκριση γαστρίνης από ένα μη Β κυτταρικό ενδοκρινή όγκο. Οι ασθενείς με Ζοllinger-Ellison συχνά παρουσιάζονται με συμπτώματα πεπτικού έλκους. Τα επίπεδα γαστρίνης ορού μπορούν να θέσουν τη διάγνωση, αλλά θα απαιτηθούν περαιτέρω εξετάσεις.

The parietal cell is regulated by neural, hormonal, and paracrine pathways. Activation of vagal parasympathetic preganglionic outflow to the stomach acts in three ways to stimulate gastric acid secretion. There is direct neural innervation and activation of the parietal cell via release of acetylcholine (ACh) from enteric neurons, which acts on the parietal cell via muscarinic receptors. In addition, neural activation of the ECL cell stimulates the release of histamine, which acts via a paracrine pathway to stimulate the parietal cell. Finally, G cells located in gastric glands in the gastric antrum are activated by the release of gastrin-releasing peptide from enteric neurons, which acts on the G cell to stimulate the release of gastrin. Gastrin thereafter acts via a humoral pathway to stimulate the parietal cell.
