1.2 million MI/year occur in USA. Acute chest pain is one of the most common reasons for presentation to Emergency Department, accounting in USA for 7 million ED visits/year. Only 15 25% of patients with acute chest pain ultimately prove to be ACS. On the other hand, 2-4% of patients with true ACS are mistakenly turned away as noncardiac chest pain. This increases their mortality 2 fold.
ECG: Within 10 minutes of first patient contact. Serial ECG repeated when pain recurs or intensifies and every 4-6 hours irrespective of whether pain remained or resolved. If chest discomfort is present: New 1 mm ST elevation prevalence of acute MI is 80% New ST depression or T inversion prevalence of acute MI is 20% Normal ECG and history of coronary HD prevalence of acute MI is 4% Normal ECG and no history of coronary HD prevalence of acute MI is 2% ECG abnormalities appear and evolve in the absence of new symptoms
The ECG in ACS In patients with ischemic chest pain, ST elevation has: 50% sensitivity 90% specificity Circumflex artery distribution is notoriously silent in the ECG. When you use ST elevation or depression, Q-wave and LBBB: 69% sensitivity 82% specificity Normal ECG identifies low risk cases with 1-5% risk of MI and 4-23% of unstable angina. A single ECG detects <50% of MI ( At the presentation). Never use ECG alone to EXCLUDE ACS.
ST depression has worse prognosis (5.1% 30 D mortality) than T inversion only (1.7% 30 D mortality). When significant ( 0.5 mm) shift in ST occurs during pain, and disappears with its disappearance this strongly suggest true ACS. Availability of previous ECG improves the accuracy of diagnosis.
Approximately 23% of myocardial infarction are not recognised because of: 1) No symptoms 2) Atypical symptoms 3) Typical symptoms ignored or misinterpreted by patients

The Role of T Wave T wave inversion of 1 mm have modest role in predicting subsequent death or MI. T wave inversion of 3 mm has a prognostic value.

ST elevation of early repolarisation: Largely confined to right precordial leads. Common in young patients. Diminishes or becomes undetectable when heart accelerates, so if appropriate let the patient do sit ups to increase heart rate.

ΕΠΙΣΗΜΑΝΣΕΙΣ - ΣΥΜΠΕΡΑΣΜΑΤΑ Η διαχείριση ασθενών µε τυπικό προκάρδιο άλγος και θετικό ηλεκτροκαρδιογράφηµα για ΟΣΣ είναι εύκολη και διαχειρίσιµη 5% των ασθενών που προσέρχονται µε πόνο έχουν STEMI και >25% non STEMI Παρουσία νεοεµφανιζοµένων αποκλεισµών σκελών στο ΗΚΓµα, προσδιορίζει άτοµα υψηλού κινδύνου. Ασθενείς µε προηγούµενο ιστορικό Σ/Ν, Σ/Δ, ηλικιωµένοι, γυναίκες και νέα άτοµα <40 ετών, παρουσιάζουν ιδιαίτερο ενδιαφέρον ως προς τη διαχείριση, ( συµπτωµατολογία, ΗΚΓµα, ένζυµα)
ΕΠΙΣΗΜΑΝΣΕΙΣ ΣΥΜΠΕΡΑΣΜΑΤΑ Διαχωρισµός ασθενών προσερχοµένων µε άτυπα προκάρδια άλγη ή προκάρδια δυσφορία µε µη ειδικές ΗΚΓφικές διαταραχές και µε αρνητική ενζυµ ική κίνηση (παρακολούθηση, επανειληµµένα ΗΚΓτα και ένζυµα) Ιδιαίτερη προσοχή στους ενδιαµέσου κινδύνου ασθενείς, οι οποίοι παρακολουθούνται επί µακρόν στις µονάδες βραχείας νοσηλείας, λαµβάνοντας επιπλέον υπ` όψιν τους παράγοντες κινδύνου Οι χαµηλού κινδύνου ασθενείς αποτελούν πρόκληση για τον κλινικό καρδιολόγο. Η πλειονότητα αυτών των ατόµων δεν έχουν κάτι σηµαντικό. Εν τούτοις καλείται ο κλινικός καρδιολόγος των ΤΕΠ να διαχωρίσει αυτούς τους ασθενείς που πρέπει να τύχουν επείγουσας θεραπευτικής αντιµετώπισης. Η αποτυχία ανίχνευσης ασθενών µε ΟΣΣ από ανεπαρκή εκτίµηση, ανέρχεται στο 2-4% και αυτοί οι ασθενείς παρουσιάζουν διπλάσια θνητότητα.
