2 Regarding the announced Tender (Π2014/002) for the Supply, Installation, Acceptance testing, Commissioning, Training and Maintenance of a Technetium Dispensing Isolator tender for the Hot-Lab of Nuclear Medicine in BOCOC, please note the following points either remain or are modified as described below: ΕΝΤΥΠΟ 8 (Technical Specifications and statement of conformity with specifications) Section : Technetium Dispensing Isolator technical specifications and safety aspects Specification no. 4: Tenders to verify from manufacturer, whether the offered isolator is upgradable or not, in order to handle ALSO high energy radiopharmaceuticals, like 18 F-FDG for application in PET-CT. This will be considered an advantage in the evaluation of the tender. If due to safety reasons a specific isolator cannot be upgraded then the tenderer should provide the corresponding safety document from the manufacturer and this will be evaluated accordingly. Specification no.6: The generators to be handled in a safe and ergonomic way. Tenderers to explain this procedure. Specification no.11: The system to have at least two ports for two gloves for the Processing Chamber, so that the system can be operated even by one person. The glove ports to have sliding shutters shielded with approximately 10mm of lead, for the abdominal protection or a local shielded viewing barrier to be positioned inside the isolator between the glove ports. Exact lead thickness of the sliding shutters or of the internal shielded barrier to be specified. The factory should verify that the applied system provides adequate shielding for the abdominal area during dispensing. Specification no. 20: Tenders to state three institutions in Cyprus or abroad where similar Nuclear Medicine Isolators or Clean Room Isolators have been successfully installed. Σελίδα 2 από 6

3 This is a requirement for all equipment installed at the BOCOC. So far no manufacturer has refused to give us such details. If a certain manufacturer considers this as sensitive information then they should state this and the tendered will be evaluated accordingly. Specification no.22a: Specify whether the isolator has adjustable height. Tenderers to forward from manufacturer the reasons why the offered isolator has this facility or not. Specification no.22b: Indicate if the base frame has lockable castors. Tenderers to forward from manufacturer the reasons why the offered isolator has this facility or not. Specification no.21: The length of the isolator not to exceed 2500 mm. Section: Warranty, Spare parts, Maintenance Contract and manufacturer support for the Technetium Dispensing Isolator Specification no. 1a: Verify that the system offered will have a two year full warranty, including service and all spare parts. All Medical equipment purchased by the BOCOC is covered by a two year warranty period by both the manufacturer and the representative in Cyprus. This has never been a problem and we must insist on this. Specification no. 1b: Verify that the Tenderer is willing to undertake and provide an eight year service contract following the two year warranty period for the isolator offered. This service contract must cover all spare parts. It must also include an annual Preventive Maintenance and Quality Assurance session performed by a Service Engineer from the factory, as well as semiannual maintenance service provided by the local engineers trained by the manufacturer. 4 Please note that what is stated here is that all Spare Parts and not Consumables be covered by the preventive maintenance service contract. Specification no. 2a: The Tenderers to provide a list of the most important electronic/mechanic/ pneumatic components, HEPA-filters and other spare parts as suggested by the manufacturer, and these items must be kept in stock or/and be available at the premises of the tenderer. These should be available throughout the contract period agreement. Not all spare parts are requested to be kept in Cyprus. What is required here is that the manufacturer recommends what spare parts should be kept in its representative s premises. These spare parts should be kept in Cyprus and the list of these spare parts should be made available to the BOCOC. Specification no.6 The Tenderers to specify in Entypo 11B what is considered as a consumable and accessory. Clarify the quantity needed and the price for each consumable based on the Model of the Isolator based on a workload of 7-8 examinations per day for 240 working days per year. The prices should be kept constant throughout the warranty and the first 3 years of the maintenance contract periods (total of 5 years). The annual cost of the necessary consumables will be covered by BOCOC. The consumables in Entypo 11B will be evaluated as part of the financial proposal of the Tenderer. Section: Trainings for the Technetium Dispensing Isolator Specification 1a: Modify to read: At least two service engineers to be trained by the manufacturer for servicing the offered system. If there is no engineer trained by the manufacturer at the time of placing the confirmed order of the isolator, then the Tenderer must make arrangements for the training of at least the first engineer to start immediately the training. It is mandatory for the training of the first engineer to be completed before the installation of the product. The training of the second engineer must be completed no later than six months from the installation of the product. It is also acceptable that the training of the two service engineers can be performed either in the factory or locally during the installation of the equipment by a manufacturer s qualified service engineer, provided that the manufacturer agrees and also that this will not affect in any way the delivery time schedule and/or the commitioning time schedule of the isolator to BOCOC. The associated cost for any training of the Tenderer service engineers shall be covered by the Tenderer. Certificates of the training must be provided upon the Acceptance Testing of the equipment. 5 Also modified based on the clarification of Section: Trainings for the Technetium Dispensing Isolator, Specification 1a is the following ΠΑΡΑΡΤΗΜΑ 3-6 ΠΑΡΑΡΤΗΜΑ ΙΙΙ MAINTENANCE AGREEMENT NUCLEAR MEDICINE ISOLATOR 2014: 2.3 The Tenderer shall maintain at least two fully trained engineer employees who will have been trained in a Manufacturer Training Centre for the Product and will be fully capable of rendering the services set forth in this Agreement. The names of such engineers and evidence in support of the above, including the certificates of training at all levels of competency provided by the Manufacturer, shall be communicated in writing by the Tenderer to the Director of the Medical Physics Department of the Hospital within 3 days of the signing of this Agreement. If there is no engineer trained by the manufacturer for the Product at the time of signing this agreement, then the Tenderer must make arrangements for the training of at least the first engineer to start immediately after signing this agreement. It is mandatory for the training of the first engineer to be completed before the installation of the Product. The training of the second engineer must be completed no later than six months from the installation of the Product. Throughout the period of this Agreement, should any of the trained service engineers leave the employment of the Tenderer, he/she shall be replaced, and the replacement engineer shall be fully trained at the Manufacturer s Training Centre, with the transition to be completed no later than six months from the departure of the trained engineer. Beyond the six month grace period, the service fees payable by the Hospital pursuant to this Agreement shall be reduced by 25% of the pro rata monthly service fee for every month or part thereof that the Tenderer employs less than the two fully trained engineers provided for in this paragraph 2.3. ΠΑΡΑΡΤΗΜΑ 3-6 ΜΕΡΟΣ Α: ΟΔΗΓΙΕΣ ΠΡΟΣ ΟΙΚΟΝΟΜΙΚΟΥΣ ΦΟΡΕΙΣ Παράγραφος 2.25: Διάρκεια εκτέλεσης της Σύμβασης Modify to read: Τέσσερεις (4) μήνες από την ημερομηνία υπογραφής της Σύμβασης Παράγραφος 2.3: Προϋπολογισμός Σύμβασης Remains as is: Παράγραφος 6.2.3: Τεχνικές και επαγγελματικές ικανότητες, Σημείο 3: Modify to read: «Να έχει ολοκληρώσει επιτυχώς κατά τα τελευταία δέκα (10) έτη, τουλάχιστον μία (1) σύμβαση, προϋπολογισμού δαπάνης που ανέρχεται τουλάχιστον στο 70% του Προϋπολογισμού της Σύμβασης, με αντικείμενο την εγκατάσταση Κουβουκλίων Ασφαλείας είτε για Παρασκευή τεχνητίου σε θερμό εργαστήριο πυρηνικής ιατρικής είτε για ανασύσταση κυτταροστατικών φαρμάκων σε «Καθαρό Δωμάτιο» σε νοσοκομεία ή στη φαρμακευτική βιομηχανία ακολουθώντας διαδικασίες σύμφωνα με αναγνωρισμένα διεθνή πρότυπα και με ποσοστό συμμετοχής τουλάχιστον 80%. Η έννοια του όρου «ολοκληρώσει» σημαίνει κατά ποσοστό υλοποίησης της σύμβασης τουλάχιστον 80%. Επίσης θα πρέπει να παρέχει επιτυχή συντήρηση του εξοπλισμού σύμφωνα με τη σύμβαση που υπογράφηκε.» Σελίδα 5 από 6
