Παρόν.αλλαγή της HIV demenwa με HAART 5 μήνες επιβίωση το 1993-1995 έως 38.5 μήνες το 1996-2000. (Dore, AIDS 2003) Προ HAART: υποφλοιώδεις βλάβες : απάθεια και σοβαρή ψυχοκινητική επιβράδυνση, απώλεια μνήμης. Εξελισσόμενη. Εγκεφαλίτιδα από πολυπύρηνα γιγαντοκύτταρα και απώλεια νευρώνων. Μετά HAART: Mεικτή φλοιώδης και υποφλοιώδης προσβολή, ηπιότερος φαινότυπος, εξάρσεις και υφέσεις. Βλάβη συναπτοδενδριτική με λιγότερο ιϊκό πολ/σμό στο ΚΝΣ.

HIV και Aging Σε πρόσφατες µελέτες HIV και aging, οι καλύτεροι δείκτες πτωχής νευρογνωσιακής λειτουργίας είναι δείκτες πρώιµης αγγειακής εγκεφαλικής νόσου και όχι το HIV status. Στην µετά HAART εποχή, φαίνεται ότι η HIV λοίµωξη να µην αποτελεί σηµαντικό παράγοντα νευρογνωσιακών διαταραχών, τουλάχιστον σε ασθενείς µε πρόσβαση και λήψη κατάλληλων φαρµάκων. Οι παράγοντες κινδύνου για νευρογνωσιακές διαταραχές σε µεγαλύτερης ηλικίας HIV ασθενείς είναι ίδιοι µε αυτούς του γενικού πληθυσµού (Becker, 2009; Sacktor, 2009)
