ΕΠΙΔΗΜΙΟΛΟΓΙΑ Σακχαρώδης Διαβήτης Σύμφωνα με πρόσφατες εκτιμήσεις της Διεθνούς Ομοσπονδίας Διαβήτη (IDF 2013), ο αριθμός των ενήλικων ατόμων με σακχαρώδη διαβήτη παγκοσμίως ανέρχεται ήδη στα 382 εκατομμύρια και αναμένεται να αυξηθεί σε πάνω από 592 εκατομμύρια σε λιγότερο από 25 χρόνια. Ο παγκόσμιος επιπολασμός του διαβήτη ανέρχεται στο 8,3%. Η αύξηση του επιπολασμού αφορά όλους τους τύπους διαβήτη, συγκεκριμένα όμως για τα άτομα με σακχαρώδη διαβήτη τύπου 2 αναμένεται αύξηση του αριθμού τους κατά 55% έως το 2035. Σύμφωνα με τη Διεθνή Ομοσπονδία Διαβήτη (IDF) αλλά και τον Παγκόσμιο Οργανισμό Υγείας (WHO) η γεωγραφική περιοχή με το μεγαλύτερο αριθμό ατόμων με σακχαρώδη διαβήτη είναι η περιοχή του Δυτικού Ειρηνικού με την Κίνα και την Ινδία να αντιμετωπίζουν τις μεγαλύτερες προκλήσεις (IDF 2013, Parvez et. al. 2007).
Πέρα όμως από το ανθρώπινο κόστος που έχει ο σακχαρώδης διαβήτης, υπάρχει και τεράστιο οικονομικό κόστος. Έτσι, το έτος 2013 η δαπάνη για το σακχαρώδη διαβήτη ανήλθε σε 548 δισεκατομμύρια δολάρια, ποσό που αντιστοιχεί στο 11% της συνολικής παγκόσμιας δαπάνης (IDF 2013). ΕΠΙΔΗΜΙΟΛΟΓΙΑ Σακχαρώδης Διαβήτης Η πλειοψηφία των ατόμων με σακχαρώδη διαβήτη παγκοσμίως είναι ηλικίας μεταξύ 40 και 59 ετών ενώ υπάρχει αυξητική τάση ανάπτυξης διαβήτη σε ολοένα και νεότερα άτομα. Όσον αφορά στο φύλο, ο επιπολασμός του σακχαρώδη διαβήτη παγκοσμίως είναι παρόμοιος στους άντρες και στις γυναίκες, αλλά στους άντρες είναι κάπως υψηλότερος σε ηλικίες κάτω των 60 ετών, ενώ στις γυναίκες σε μεγαλύτερες ηλικίες. Συνολικά, ο επιπολασμός του σακχαρώδη διαβήτη είναι υψηλότερος στους άντρες αλλά οι γυναίκες με σακχαρώδη διαβήτη είναι περισσότερες από τους άντρες. Υπολογίζεται ότι κάθε έξι δευτερόλεπτα ένας άνθρωπος πεθαίνει από σακχαρώδη διαβήτη παγκοσμίως. Οι θάνατοι που αποδόθηκαν στο σακχαρώδη διαβήτη το έτος 2013, ανέρχονται σε 5,1 εκατομμύρια.

Evidence for blood pressure goals By some standards, the Action to Control Cardiovascular Risks in Diabetes Study (ACCORD) was a disappointment. ACCORD (published in the New England Journal of Medicine in 2008 ) was a large well-designed trial that attempted to study the effects of tight control of blood sugar, hypertension, and lipids in patients with type 2 diabetes mellitus. 4,733 participants in ACCORD were randomly assigned to intensive blood pressure therapy, targeting a systolic pressure of <120 mm Hg or standard therapy targeting a systolic pressure of <140 mm Hg. The mean follow-up was 4.7 years. Serious adverse events attributed to antihypertensive treatment occurred in 77 of the 2,362 participants in the intensive therapy group (3.3%) and 30 of the 2,371 participants in the standard therapy group (1.3%). In ACCORD, lowering systolic blood pressure from the mid-130s to 120 mm Hg did not further reduce cardiovascular events, with the possible exception of stroke.
INVEST (INternational VErapamil- SR/Trandolapril STudy) Of the 22576 participants (who were recruited because they had both coronary disease and hypertension), 6400 (28%) had diabetes. These patients were evaluated for the effects of achieved systolic blood pressure on the risk of cardiovascular events. Patients were categorized into three groups on this basis: tight (<130), usual (130-<140), and uncontrolled ( 140) mm Hg achieved systolic blood pressure. Tight control was not associated with improved cardiovascular outcome compared to usual control. A J-shaped relationship was observed for patients.

Beta-Blockers Newer b - blockers that cause vasodilatation appear to not have the deleterious effects on insulin sensitivity and glucose metabolism. Carvedilol, a non-selective b - blocker with a1-blocking properties has been found to improve insulin sensitivity. In two trials comparing carvedilol with metoprolol, the Glycemic Effects in Diabetes Mellitus: Carvedilol-Metoprolol Comparison in Hypertensives (GEMINI) trial and the Carvedilol or Metoprolol European Trial (COMET), the carvedilol group had decreases in both insulin resistance and HbA1c while the metoprolol group had an increase in HbA1c and no change from baseline in insulin resistance (GEMINI), and improved rates of survival and cardiovascular hospitalizations (COMET). In addition to carvedilol, vasodilating b - blockers available are labetalol and nebivolol.

TAKE HOME MESSAGES Non-pharmacological measures (particularly weight loss and reduction in salt intake) should be encouraged in all patients with diabetes, independently of the existing blood pressure. The goal blood pressure to aim at during behavioural or pharmacological therapy <140/85 mmhg. In diabetic patients with high blood pressure, who may achieve blood pressure goal by monotherapy, the first drug to be tested should be a blocker of the renin angiotensin system (ACE & ARBS). The finding of microalbuminuria in type 1 or 2 diabetics is an indication for antihypertensive treatment, especially by a blocker of the renin angiotensin system. To reach this goal, most often combination therapy will be required.
