ΕΙΣΑΓΩΓΗ Οι συγγενείς ανωµαλίες των στεφανιαίων αρτηριών, που αφορούν την έκφυση ή/και την πορεία τους είναι σπάνιες και συνήθως δεν επηρεάζουν το προσδόκιµο επιβίωσης. Ωστόσο, συγκεκριµένοι τύποι (πορεία του ανώµαλου αγγείου ανάµεσα στην ανιούσα αορτή και τον κορµό της πνευµονικής αρτηρίας) έχουν ενοχοποιηθεί για επεισόδια αιφνίδιου καρδιακού θανάτου και για πρόκληση µυοκαρδιακής ισχαιµίας ακόµη και επί απουσίας σηµαντικών αθηροσκληρυντικών αλλοιώσεων. Σε εργασίες που έχουν δηµοσιευθεί η επίπτωσή τους κυµαίνεται από 0,6 ως 1,3%. Είναι προφανές ότι η αναγνώριση συγγενών ανωµαλιών των στεφανιαίων αγγείων κατά τη διάρκεια καρδιακού καθετηριασµού αποτελεί προϋπόθεση για τη σωστή λήψη αποφάσεων και διαχείριση των ασθενών.
ΥΛΙΚΟ-ΜΕΘΟ ΟΙ Μελετήσαµε αναδροµικά τα αρχεία 4535 ασθενών που υποβλήθηκαν σε στεφανιογραφικό έλεγχο, στην κλινική Euromedica - Κυανούς Σταυρός της Θεσσαλονίκης, από 1/1/2004 30/4/2009. Στον υπό µελέτη πληθυσµό η επίπτωση των συγγενών ανωµαλιών ήταν 1,35% (61/4535)

Ο πιο συχνός τύπος ήταν η έκφυση της περισπώµενης αρτηρίας από το δεξιό κόλπο του Valsalva σε ποσοστό 31,1% (19/61) ή 0,42% (19/4535) επί του συνόλου των ασθενών. Στην κατηγορία αυτή παρατηρήθηκαν τρεις ανατοµικές παραλλαγές
Ξεχωριστή έκφυση της ΠρΑ και της ΣΑ εντός του ΚV (8/19, 42,1%) RCA Cx
Κοινό στόµιο της ΠρΑ και της ΣΑ (6/19, 31,6%) RCA Cx
Έκφυση της ΠρΑ από το εγγύς τµήµα της ΣΑ (5/19, 26,3%) Cx RCA
Ο δεύτερος πιο συχνός τύπος αφορούσε την ξεχωριστή έκφυση της ΠρΑ και του ΠΚΚ εντός του ΑΚV σε ποσοστό 26,2% (16/61) ή 0,35% (16/4535) επί του συνόλου των ασθενών. (Double barrel anatomy or absent left main trunk)

Έκτοπη ΣΑ (πάνω από το ΚV) παρατηρήθηκε στο 22,9% (14/61) ή 0,31% (14/4535). Αo RCA RCA Η ΣΑ εκφύεται από τα εγγύς 2 cm της ανιούσης αορτής - κάθετη πορεία αρχικού τµήµατος.
Συγγενής έλλειψη της ΠρΑ ή υπερεπικρατούσα ΣΑ διαπιστώθηκε σε 3 ασθενείς (4,9%). RCA LCA Υπερεπικρατούσα ΣΑ περνά τον crux, ανέρχεται στην αριστερή κολποκοιλιακή αύλακα και αιµατώνει το οπισθοπλάγιο και πλάγιο τοίχωµα.
Ανώµαλη έκφυση της ΣΑ από τον ΑΚV βρέθηκε σε συχνότητα 4,9% (3/61) ή 0,06% (3/4535). Η ΣΑ εκφύεται έµπροσθεν του στοµίου της ΑΣΑ και ακολουθεί συνήθως πορεία ανάµεσα στην ρίζα της αορτής και την πνευµονική αρτηρία πριν φθάσει στη δεξιά κολποκοιλιακή αύλακα.
Η δεξιά στεφανιαία αρτηρία εκφύεται από τον αριστερό στεφανιαίο κόλπο. Έγινε δυνατός ο καθετηριασµός µε καθετήρα ALII. RCA Αo LCA Left sinus of valsalva Η µορφολογία του στοµίου και η αρχική γωνίωση προκαλεί δυσκολίες στον εκλεκτικό καθετηριασµό και προδιαθέτει σε ισχαιµία και αιφνίδιο καρδιακό θάνατο. ύο ασθενείς µε σηµαντικές βλάβες στο εγγύς τµήµα της ανώµαλης ΣΑ αντιµετωπίστηκαν µε διαδερµική στεφανιαία παρέµβαση.
Σε δύο ασθενείς το αριστερό στεφανιαίο σύστηµα εκφύονταν από το ΚV, σχηµατίζοντας µια µονήρη στεφανιαία αρτηρία (συχνότητα 3,3% ή 0,04% επί του συνόλου των ασθενών).
Το 1979 οι Lipton et al πρότειναν µια αγγειογραφική ταξινόµηση η οποία τροποποιήθηκε το 1990 από τους Yamanaka and Hobbs R (right)-type / L (left)-type Ανάλογα µε την έκφυση της µονήρους αρτηρίας από το ΚV ή τον ΑΚV
Groups I, II, III ανάλογα µε την ανατοµική πορεία της αρτηρίας Group I Η µονήρης αρτηρία ακολουθεί την πορεία της ΑΣΑ ή της ΣΑ L-I pattern: υπερεπικρατούσα ΠρΑ απουσία ΣΑ R-I variant: υπερεπικρατούσα ΣΑ, συνεχίζει σε ΠΚΚ Group II Η ανώµαλη στεφανιαία αρτηρία εκφύεται από το εγγύς τµήµα της άλλης φυσιολογικής στεφανιαίας αρτηρίας R-II type / L-II type Group III ΠρΑ και ΠΚΚ εκφύονται χωριστά από το εγγύς τµήµα της φυσιολογικής στεφανιαίας αρτηρίας

Dot and Eye Method-Septal course The left main runs an intra-myocardial course through the ventricular septum along the floor of the right ventricular outflow tract. It then surfaces to a normal epicardial position in the mid-septum, where it divides into the LAD and circumflex. ΣΥΜΠΕΡΑΣΜΑΤΑ Η επίπτωση συγγενών ανωµαλιών των στεφανιαίων αγγείων σε αυτό το δείγµα ασθενών από τη Βόρεια Ελλάδα είναι παρόµοια µε αυτή άλλων δηµοσιευθέντων εργασιών. Η γνώση αυτή ίσως είναι σηµαντική καθότι διαφορές στη συχνότητα και τους τύπους των συγγενών ανωµαλιών έχουν παρατηρηθεί σε διαφορετικούς γεωγραφικούς πληθυσµούς.

Coronary arterial anomalies occur in approximately 1% of the population without other congenital cardiac malformations The incidence of anomalous origin of the right coronary artery out of the right sinus of Valsalva ranges from under 0,01-0,09% It usually arises from the left sinus of Valsalva or the ascending aorta above it, and in most reported cases it courses between the great vessels. Rarely the anomalous origin is found to be from the posterior sinus or another coronary artery
It has been suggested that the abnormal origin and course of anomalous coronary arteries could make them more prone to atherosclerosis due to altered flow patterns. However there are no definitive data on whether anomalous coronary arteries are more or less prone to atherosclerosis than normally arising ones The anomalous origin of RCA from the left sinus of Valsalva is difficult to reach with standard catheters and PCI requires more secure guiding catheter support compared to the normally located RCA. Choosing the appropriate guiding catheter is the most important factor in determining procedural success. A guiding catheter with a curve that seats against the posterior wall of the aorta such as the Amplatz, XB or the Voda catheters, provide the reliable backup needed for the intervention3 The choice of guidewire can also affect the final outcome
