Questions to be answered Is our patient ill enough to require MCS? Is our patient well enough to go through surgery? Which type of support (Right, Left OR both)? Can the patient and the family cope with the MCS requirements?

What type of support? Destination therapy is mainly thought of as LV support High complication rate of concomitant RV support reflecting mainly worse clinical status of the patients Most devices are customised and licenced for LV support Biventricular support is only as bridge to transplant, when high probability of a donor heart is expected, or as bridge to recovery in acute HF of previously well patients
Is RV good enough to tolerate LV support? Pulmonary Hypertension and RV dysfunction is usually secondary to LV failure (increase in PVR) This can be expected to improve after LV support RV dysfunction may also result from a primary cause (ie RV infarct) or may be irreversible and severe following prolonged pressure and volume overload
primary management goal is improvement in rightsided circulatory flow, which is primary determined by two factors: Right ventricular (RV) function Pulmonary vascular resistance Since left ventricular end-diastolic pressure will drop after the insertion of an LVAD, the afterload of the RV should fall. As a result, even poor RV function before LVAD implantation often improves
Increased pulmonary vascular resistance and right ventricular dysfunction during or early after insertion of the LVAD can often be successfully treated with : Hyperventilation maximal oxygenation inotropes pulmonary vasodilators such as nitric oxide
Is RV good enough to tolerate LV support? It is imperative we take the right decision regarding the RV status. Post op RV dysfunction is very difficult to treat and bail-out RV support post-op is expected to have dismal prognosis

INTERMACS (Interagency Registry of Mechanically Assisted Circulatory Support) study has delineated clinical profiles of advanced heart failure
Patients depending on intravenous inotropic therapy have a dismal outcome and should be offered devices if eligible (profiles 2,3) the broader target for current mechanical circulatory support is the ambulatory heart failure population who are not yet dependent on inotropic therapy or continued hospitalization but have very limited functional capacity and poor prognosis for survival (profiles 4 to 6)

Conclusions MCS has provided a feasible alternative to cardiac transplant for patients in end-stage or advanced heart failure Patient selection is of paramount importance as it dictates prognosis. Emphasis should be placed on EARLY referrals of patients. Careful multidisciplinary evaluation of the patient and the family is mandatory Patient and family education requires close follow up.
