Inhibition of cell wall synthesis Vancomycin acts by binding to the D-alanyl-D-alanine peptide termini of the nascent peptidoglycan-lipid carrier. β-lactams inhibit PBP by bonding to its D-alanyl--Dalanine binding pocket. MRSA has acquired a gene for PBP2 which has a very low binding affinity to b-lactam antibiotics. As a result the production of a cell wall can continue. This is the basis for Methicillin resistance in S. Aureus.

Βακτηριοκτόνα - Βακτηριοστατικά Βακτηριοκτόνα. Ονομάζονται τα αντιβιοτικά που προκαλούν γρήγορα τον θάνατο των μικροβίων (πενικιλλίνες, κεφαλοσπορίνες, αμινογλυκοσίδες, βανκομυκίνη κ.α.). Βακτηριοστατικά. Ονομάζονται τα αντιβιοτικά που προκαλούν αναστολή της ανάπτυξης των μικροβίων (σουλφοναμίδες, τετρακυκλίνες, χλωραμφαινικόλη, ερυθρομυκίνη, κλινταμυκίνη κ.α.).

Τύποι αντοχής στα αντιβιοτικά Ενδογενής αντοχή Μερικά μικρόβια είναι γενετικά ανθεκτικά σε ορισμένα αντιβιοτικά πχ, Enterococci και aminoglycosides. Επίκτητη αντοχή Μεταλλάξεις (ESBLs, PBPs). Ανταλλαγή γενετικού υλικού με plasmids, transposons (VanA) ή μετασχηματισμό. Επαγώμενη αντοχή με μεταβολή της έκφρασης γονιδίων (πχ αποκαταστολή στο Enterobacter έπειτα από χρήση cephalosporin).

Μέθοδος διαδοχικών αραιώσεων Minimum Bactericidal Concentration (MBC) : Ως ελάχιστη βακτηριοκτόνος συγκέντρωση ορίζεται η ελάχιστη συγκέντρωση των αντιβιοτικών που είναι αναγκαία για να φονεύσει ένα μικρόβιο (επιβιώνει <0.1% των μικροβίων). Minimum Inhibition Concentration (MIC): Ως ελάχιστη ανασταλτική συγκέντρωση ορίζεται η ελάχιστη συγκέντρωση των αντιβιοτικών που είναι αναγκαία για να αναστείλει την ανάπτυξη ενός μικροβίου.
