Stroke Epidemiology u u u u u > 700,000 new or recurrent strokes occur per year Accounts for > 50% of all hospitalizations for acute neurologic disease ~ 4 million Americans are living with neurologic deficits due to stroke Leading cause of serious, long-term disability Risk and mortality increase with age
Stroke Ú 3rd leading cause of death in the United States Ú 2nd most common cause of cardiovascular death Ú #1 cause of death from a neurologic disorder
Ischemic stroke (80-85% of all stroke events) Ú Large vessel disease (extra and intracranial) develops from atherosclerosis and thrombosis (50%) Ú Small vessel ( size < 1.5 cm ) disease or lacunar infarction (20-25%) Ú Embolism (20%) Atherothromboembolism or artery to artery embolism Cardioembolism -dysrhythmias ( AF ) -structural changes
Hemorrhagic stroke Ú Intracerebral hemorrhage ( ICH ) occurs when the hemorrhage penetrates into the parenchyma of the brain. It results most commonly from hypertension. This may from amyloid angiopathy or vascular malformation Ú Subarachnoid hemorrhage (SAH) occurs when the site of hemorrhage is in the subarachnoid spaces surrounding the brain. It is usually caused by a ruptured aneurysm

Natural History-I Ú Cumulative risk for stroke at 5 years after a TIA is 30-50%. Ú 1/3 patients die within 5y of TIA, usually of CAD. Ú Risk for stroke following TIA 10-30% in first year, 6% risk subsequent years. Ú Only 10% of stroke patients have had a TIA prior.
Natural History-II Ú Ú Ú Risk of stroke is proportional to degree of stenosis In patients with more than 75% stenosis 1 year 18% with ischemic events 2 years 22% with ischemic events In patients with less than 75% stenosis 1 year 3% with ischemic events 2 years 6% with ischemic events
Natural History-III Ú 35% of patients with a TIA will have a stroke in their lifetime 50% of these will occur in the year following first TIA After first year stroke risk is 5% per year Ú After stroke, 20-30% mortality, risk of recurrent is 5-40%, with 30% of these fatal. only 50% of stroke victims will be alive at five years.

Peripheral Arterial Disease (PAD)
Peripheral Arterial Disease (PAD) Vascular disease can block circulation to the legs, leading to serious disability. Asymptomatic PAD Symptomatic PAD Intermittent claudication limiting the amount you can walk, Critical ischemia leading to gangrene and leg amputation.
Prevalence of PAD variation according to diagnostic criterion 6.3 million individuals with symptomatic, established PAD are diagnosed in the USA and EU1 Epidemiological studies imply that real* prevalence may be approx. 20 million (= 9.5% of the population > 50 years old) In 613 men and women (mean age 66 years), real prevalence was found to be underestimated by two- to seven-fold2 Even people with mild PAD are three times as likely to die of heart attacks and strokes as those without the disease. 1 17 Western European countries. n n Epidemiology of PAD effect of age and gender Epidemiological data on PAD vary according to: Population studied Method of diagnosing PAD Incidence and prevalence of intermittent claudication* increase with age n Prevalence in men aged 45 50 years is 1% n Prevalence is 3 3.5% in men aged > 50 years n Similar trend in women, increase with age n More common in men than in women Twice as many men as women aged > 50 years have intermittent claudication (3.5% and 2%, respectively) Predominance in males disappears after age of 70 Weitz JI et al.
