Seven areas to be examined for abnormal cardiovascular pulsations by inspection and palpation. 1, sternoclavicular; 2, aortic; 3, pulmonic; 4, left parasternal; 5, apical; 6, epigastric; 7, ectopic (abnormal impulse, which may be in variable locations).
Top. Normal physiologic splitting. During expiration, the aortic second sound (A 2 ) and the pulmonic second heart sound (P 2 ) are separated by less than 30 ms and are appreciated as a single sound. During inspiration, the splitting interval widens, and A 2 and P 2 are clearly separated into two distinctly audible sounds. Bottom. Audible expiratory splitting. In contrast to normal physiologic splitting, two distinct sounds are easily heard during expiration. Wide physiologic splitting is caused by delay in P 2. Reversed splitting is caused by delay in A 2, resulting in paradoxical movement; that is, with inspiration, P 2 moves toward A 2, and the splitting interval narrows. Narrow physiologic splitting is seen in pulmonary hypertension, and both A 2 and P 2 are heard during expiration at a narrow splitting interval as aresult of an increased intensity and high-frequency composition of P 2. The differential diagnosis of the innocent murmur versus the pathologic systolic murmur is made by the "company the murmur keeps." The innocent murmur must be found in the setting of an otherwise normal cardiovascular examination. A 2, aortic second sound; AR, aortic regurgitation; AVES, aortic valvular ejection sound; C, midsystolic nonejection sound; M 1, mitral first sound; P 2, pulmonic second heart sound; PVES, pulmonic valvular ejection sound; S 1, first heart sound; S 2, second heart sound; S 3, third heart sound; S 4, fourth heart sound; T 1, tricuspid first sound.
A midsystolic nonejection sound (C) occurs in mitral valve prolapse and is followed by a late systolic murmur that crescendos to a first heart sound (S 1 ). With assumption of the upright posture, venous return decreases, the heart becomes smaller, thec moves closer to S 1, and the mitral regurgitant murmur has an earlier onset. With prompt squatting, both venous return and afterload increase, the heart becomes larger, the C moves toward a second heart sound (S 2 ), and the duration of the murmur shortens.
