NOACs: A Case Example Approach Mary, a 75-year-old woman, was diagnosed with newonset AF after routine ECG. Type 2 DM with moderate renal dysfunction and a creatinine clearance of 45 ml/min. She has a history of falls but not of bleeding. She does not drive and lives on her own in an area where public transport is limited. You perform a comprehensive physical exam, including an assessment of her risk of stroke.

So which is the best practice to prevent stroke for Mary? ASA, DAPT Warfarin NOACs
Camm J. ES, Barcelona, August 30 2009. Warfarin: Battling All Comers for Over a Decade Warfarin vs. Placebo/control Warfarin vs. low-dose warfarin + ASA Warfarin vs. ASA VKA vs. ASA + clopidogrel 0 0.3 0.6 0.9 1.2 1.5 1.8 2.0 Favors Warfarin Favors Alternative
Warfarin: Advantages 1. INR assesses anticoagulant level 2. Multiple antidotes available 3. Omitting one or two doses usually is not clinically problematic 4. Introduced in 1954. Has stood the test of time. 5. No liver toxicity 6. Ability to maintain target INR is improving (Now > 60% in top facilities) 7. Inexpensive

Mary cannot FU INR. Maybe DAPT better than ASA alone?

Dg vs. Rvx/Apx Patras University Hospital 11
In choosing between the two new drugs so far available dabigatran and rivaroxaban Some people prefer a once-daily dosage (ie, rivaroxaban). Dabigatran should be avoided in severe renal failure. Renal function is not so much of a problem with rivaroxaban but it still needs to be considered. Only the low dose of dabigatran should be used with the P- glycoprotein (Pgp) inhibitor verapamil Close surveillance recommended especially in pts with renal impairment with quinidine and amiodarone.
Dabigatran can be dialysed out of the system; rivaroxaban cannot. For ACS patients, there are better data for rivaroxaban (in low dose) from the ATLAS trial. There is concern over the higher dose of dabigatran in the elderly. "Although physicians traditionally like to err away from bleeding risk, I would recommend 150 mg as the standard dose of dabigatran, with 110 mg reserved for patients with reduced renal function or those over 80 years old."

PHARMACO- GENETIC NOMOGRAM NEJM 2009; 360: 753-764
RE-LY: Analysis and Comments RE-LY participants who were randomly assigned to receive warfarin would have needed to have an INR time within the therapeutic range (TTR) approximately 79% of the time to have a stroke rate as low as that in the group receiving 150 mg of dabigatran. Even with diligent, patient self-monitoring or pharmacogenetic dosing, such tight control is unlikely in real world practice.
