Οι ενδείξεις ηπατεκτομής στην αντιμετώπιση κακώσεων των χοληφόρων είναι σπάνιες, αλλά έχουν αυξηθεί σημαντικά μετά το 2000, ίσως λόγω της πολυπλοκότητας των κακώσεων. Η ηπατεκτομή απαιτήθηκε σε περιπτώσεις σύγχρονης αγγειακής και υψηλής κάκωσης του χοληφόρου δένδρου. Τα ποσοστά θνητότητας είναι συγκρίσιμα με αυτά της ηπατικο νηστιδικής αναστόμωσης Η επείγουσα ηπατεκτομή θα πρέπει να αποφεύγεται, εκτός αν υπάρχει κάκωση της πυλαίας φλέβας ή αρτηριακή κάκωση. 51 Transplant indications after major BDI: 1)Lesions of the bile duct leading to recurrent cholangitis, chronic cholestasis, and secondary biliary cirrhosis 2)Hilar vascular lesions (almost always arterial) associated with fulminant hepatic failure 52 26
