Progressive renal impairment leads to multiple changes in the management of patients with T2DM Physiological challenges: 1 4 Blood glucose levels can fluctuate widely HbA1c level measurement can be falsely high Other considerations: 1 4 Renal impairment is common in diabetic patients Age related decline in renal function In patients with renal impairment it is important to establish the safety and efficacy of antidiabetic medication 3,4,7 Therapeutic challenges: 1 6 Metformin: Either contraindicated (creatinine clearance <60 ml/minute) or dose reduction SU: Either contraindicated (severe renal impairment) or dose reduction to avoid hypoglycaemia TZDs: Propensity for fluid retention AGIs: Contraindicated in patients with severe renal impairment because of a lack of data Insulin: Clearance not predictable, making dose adjustments difficult SGLT-2 inhibitors: Efficacy is dependent upon kidney function and declines progressively with advancing renal impairment. Άρα όταν λαμβάνουμε τροφή από το στόμα εκκρίνονται από το έντερο κάποιες ορμόνες, οι οποίες στη συνέχεια δρουν στο πάγκρεας και αυξάνουν την έκκριση ινσουλίνης Οι ορμόνες αυτές ονομάζονται ινκρετίνες Οι ινκρετίνες διεγείρουν την έκκριση της ινσουλίνης μόνο όταν τα επίπεδα γλυκόζης είναι αυξημένα. Η πρώτη ορμόνη που βρέθηκε ότι έχει ιδιότητες ινκρετίνης ήταν το GIP (Glucose dependent Insulinotropic Polypeptide). Το 1983 ανακαλύπτεται μία νέα ορμόνη, το GLP-1 (Glucagon Like Peptide) που διαπιστώνεται ότι παρουσιάζει επίσης ιδιότητες ινκρετίνης όπως και το GIP. 1996 Οι ινκρετίνες αποδομούνται από το ένζυμο διπεπτιδυλική πεπτιδάση (DPP-IV).
Η φαρμακευτική βιομηχανία ανέπτυξε φάρμακα που αναστέλλουν το ένζυμο DPP-IV (DPP-IV αναστολείς) Με στόχο τη μείωση της αποδόμησης των ινκρετινών που παράγονται σε μειωμένη ποσότητα και την μερική αποκατάσταση των χαμηλών επιπέδων των ινκρετινών που υπάρχουν στον ΣΔ τύπου 2 Οι αναστολείς της DPP-IV αναστέλλουν την ταχεία in vivo αδρανοποίηση των GLP-1 και GIP σε αδρανείς μεταβολίτες και έτσι επιτυγχάνουν τη διατήρηση της ινκρετινικής δράσης των ορμονών αυτών.
