In patients with a coronary stent who are receiving dual antiplatelet therapy and require surgery DAPT be administered for at least 1 month after BMS implantation in stable CAD,for 6 months after newgeneration DES implantation, and for up to 1 year in patients after ACS, irrespective of revascularization strategy A minimum of 1 (BMS) to 3 (new-generation DES) months of DAPT might be acceptable, independently of the acuteness of coronary disease, in cases when surgery cannot be delayed for a longer period Ηowever, such surgical procedures should be performed in hospitals where catheterization laboratories are available Independently of the timeframe between DES implantation and surgery, single anti-platelet therapy (preferably with aspirin) should be continued

glycoprotein inhibitors
For patients with a very high risk of stent thrombosis, bridging therapy with intravenous, reversible glycoprotein inhibitors, such as eptifibatide or tirofiban, should be considered Cangrelor, the newreversible intravenous P2Y12-inhibitor, has been shown to provide effective platelet inhibition but is not yet available The use of low-molecular-weight heparin LMWH for bridging in these patients should be avoided

Preoperative bridging with a glycoprotein IIb/IIIa inhibitor does not abolish the risk of perioperative stent thrombosis (although it is associated with a low perioperative stent thrombosis rate), and may carry increased risk for bleeding

At this point, the question is where do we go from here? Based on currently available data and guidelines, it seems reasonable to defer purely elective surgery for at least 12 months after DES For patients requiring urgent surgery within 12 months of DES, and especially for those within 6 months of DES, it might be reasonable to consider preoperative OCT (optical coherence tomography) to assess stent endothelialization; those with incomplete stent endothelialization might be reasonable candidates for continuation of DAPT if feasible, monotherapy with aspirin, or bridging therapy with GPI
Ιnitiation of antiplatelet treatment In patients who had interrupted the aspirin or clopidogrel before surgery we recommend antiplatelets initiation in the first 24 hours after operation or in the next morning if there is adequate post-operative hemostasis (Grade 2C) In patients with DES < 12 μήνες in whom clopidogrel was interrupted before operation, the charge dose of clopidogrel (300mg) is recommended when there is adequate hemostasis post-operative Dual anti-platelet therapy should be resumed as soon as possible after surgery and, if possible, within 48 hours. Reversal of antiplatelet treatment Regional anaesthesia For patients receiving anti-platelet therapy, who have excessive or life-threatening perioperative bleeding, transfusion of platelets is recommended Regional anaesthesia No contraindications with aspirin Thienopyridine derivatives (clopidogrel and prasugrel) should be discontinued clopidogrel 5 7 days, prasugrel 7 days, and ticlopidine 14 days before procedure GP IIb/IIIa inhibitors should be discontinued to allow recovery of platelet function before procedure (8 h for tirofiban and eptifibatide)
Take home message Επέμβαση σε ασθενή με ασπιρίνη ιακοπή της ασπιρίνης εάν ο αιμορραγικός κίνδυνος είναι μεγαλύτερος από το θρομβοεμβολικό ιακοπή τουλάχιστον για 7 ημέρες Επέμβαση σε ασθενή με αναστολείς PY12 ιακοπή εάν ο αιμορραγικός κίνδυνος είναι μεγαλύτερος από το θρομβοεμβολικό ιακοπή τουλάχιστον για 5 ημέρες για την κλοπιδογρέλη/τικαγρελόρη και 7 ημέρες για την πρασουγρέλη
Take home message Επέμβαση σε ασθενή με αντιαιμοπεταλιακά και stent Περίμενε! (4 wks BMS (ideally for up 3 mts), 3-12 mts DES) Απέφυγε τη διακοπή της ασπιρίνης Aν όχι. Συνέχισε τη διπλή αγωγή Αν δεν μπορείς να περιμένεις και δεν μπορείς να συνεχίσεις την διπλή, σταμάτησε την κλοπιδογρέλη 5 ημέρες πριν, χορήγησε θεραπεία γεφύρωσης και επανέναρξη κλοπιδογρέλης όσο πιο σύντομα γίνεται Η χορήγηση ΜΜΒΗ δεν αντικαθιστά τα αντιαιμοπεταλιακά Ενημέρωση συγκατάθεση Παρουσία αιμοδυναμικού εργαστηρίου στο νοσοκομείο
