For patients with AF who have an ischemic stroke or TIA despite therapeutic anticoagulation, there are no data to indicate that either increasing the intensity of anticoagulation or adding an antiplatelet agent provides additional protection against future ischemic events. In addition, both of these strategies are associated with an increase in bleeding risk. Kernan et al, Stroke 2014

The combination of oral anticoagulation (ie, warfarin or one of the newer agents) with antiplatelet therapy is not recommended for all patients after ischemic stroke or TIA but is reasonable in patients with clinically apparent CAD, particularly an acute coronary syndrome or stent placement (Class IIb; Level of Evidence C) Kernan et al, Stroke 2014

our findings did not show any benefit of NOACs in secondary prevention of ischemic stroke. Sardar et al, PLOS ONE 2013
1. Patients with cerebral bleeding were excluded from the trials. 2. Stroke within 14 days and severe stroke within 6 months before screening were exclusion criteria in the trials investigating dabigatran and rivaroxaban. 3. There is no antidote for reversal and no reliable laboratory monitoring of the anticoagulant effect for emergency situations. Stollberger et al, 2013

Άλλες πηγες About one third of patients who present with AF and an ischemic stroke will be found to have other potential causes for the stroke such as carotid stenosis. For these patients, treatment decisions should focus on the presumed most likely stroke origin. In many cases, it will be appropriate to initiate anticoagulation, because of the AF, and additional therapy (such as CEA).
